2. Licensee Action on Previous Inspection Findings

(Closed) Violation (50-407/8201-01): This violation concerned thelicensee's failure to follow procedure and cited three examples of lackof entries and/or lack of operator signature in the operations log bookfor reactor operation as required by the Nuclear Engineering LaboratoryOperations Manual. The licensee's response was to instruct operators toinclude their signature for all operations entered in the log book and torevise the supporting forms for reactor startup listed in Appendix B ofthe Technical Specifications to insure that entries are made in theoperations log at the beginning of checkout for startup and the necessaryannotations are made in the log book if the startup is aborted. Duringthis inspection, the NRC inspector determined that the Appendix B Forms B-1,B-2, and B-3 had been revised and a review of operations logs and thesupporting forms, for operation subsequent to May 1982, indicated that allreactor operations are now noted in the log and all log entries weresigned by the reactor operator.

(Closed) Open Item (50-407/8201-02): This item addressed the need toincorporate a review requirement for completed surveillance tests inlicensee procedures. The NRC inspector determined that the ReactorSafety Committee had stipulated in its written statement of authority," Safety Committee - Rules and Procedures," that all operational recordsof the facility shall be reviewed.

(Closed) Open Item (50-402/8201-03): This item addressed the lack ofdocumentation of oral examinations as part of the committed operatorrequalification training program. The NRC inspector reviewed trainingdocumentation for the past year and determined that both practicalexperience and oral examinations are being documented and certified.

3. Reactor Organization, Loos, Records, and Operating Procedures

The following logs and records for the period May 12, 1982, throughApril 15, 1983, were reviewed:

The NRC inspector determined that the power recorder trace could becorrelated to reactor startup and termination checklists and both thetrace and checklists could be correlated to the reactor operation log.The operator instruction and checklist revisions as committed to by thelicensee in response to violation 50-407/8201-01 had been accomplishedand appeared to be adequate.

There had been no major maintenance performed since the last inspection.

The NRC inspector reviewed the Nuclear Engineering Laboratory OperationManual and determined that it is in need of a thorough review and generalrevision. The facility organization, although still consistent with theTechnical Specifications, had changed and was not reflected in theoperation manual. Other examples of specific areas, requiring revisionare: emergency call list was different; and, forms had been revised.This was discussed with the licensee, and the NRC-inspector advised thelicensee that when action is taken on the pending facility licenserenewal request, a revision will be mandatory since the TechnicalSpecifications are being revised and a new emergency plan will beissued. This is considered and open item pending licensee action to reviewand issue a revised operation manual. (50-407/8301-01)

4. Surveillance

The purpose of this inspection was to ascertain whether or notsurveillance was conducted in accordance with the requirements of theTechnical Specifications, Section 4.

The NRC inspector reviewed the records of surveillance tests conductedsince the last inspection. From this record review, it was determinedthat all required surveillance tests had been completed as required. TheNRC inspector observed the conduct of a " monthly check" procedure by asenior operator. There were no questions about the conduct of the test.

There were no violations or deviations identified in this area of theinspection.

5. Requalification Training

The purpose of the inspection was to verify that the licensee'srequalification training program was conducted in accordance with theapproved training plan and 10 CFR 55.

The NRC inspector reviewed the licensee's training records. It was foundthat the status of annual written examinations was current. There were noquestions concerning the depth or content of written examinations whichhad been given. The licensee had implemented formal documentation andcertification of practical experience and the required annual oralexamination for each operator.

There were no violations or deviations identified in this area of theinspection.

6. Review and Audit

The minutes of the licensee's Reactor Safety Committee (RSC) meetingsconducted since the last inspection were reviewed. The NRC inspectordetermined that the Technical Specification requirements for quorum andfrequency were met.

During the review of the reactor startup and termination checklists,Forms 8.1, B.2, and B.3, the NRC inspector noted that the RSC memberresponsible for review had failed to indicate a review for the periodending December 7, 1982. This was discussed with the licensee and it wasdetermined to be an oversight. A satisfactory review was reported anddocumented by the RSC meeting minutes for December 7, 1982. The NRCinspector also noted that the semi-annual surveillance tests were beingreviewed by the designated RSC member but the RSC meeting minutes did notspecifically identify this review. This was discussed with a licenseerepresentative and will be included in future RSC meeting minutes.

There were no violations or deviations identified in this area of theinspection.

7. Experiments

Experiments performed since the last inspection were found to have beencompleted, reviewed, and approved in accordance with TechnicalSpecifications and Section 2 of the Nuclear Engineering LaboratoryOperations Manual.

There were no violations or deviations identified in this area of theinspection.

8. Radiation Control

The purpose of this inspection was to ascertain whether or not thelicensee's radiation protection and contamination control program wasadequate and consistent with 10 CFR requirements.

The NRC. inspector noted-that reac' tor spaces at the licensee's facilitywere properly posted:and that personnel were using dosimetry equipment.It was also found that radiation monitoring equipment available to theoperators was in calibration at the time of the inspection.

The NRC inspector also noted that there had been no recorded environmentalreleases of radioactive materials and that the licensee was conductingenvironmental monitoring.

There were no violations or deviations identified in this area of theinspection.

9. Emergency Procedures

The NRC inspector noted that emergency call lists were prominently postedin several locations within the laboratory complex. The lists did notreflect a recent change in the radiation safety officer position. Thiswas brought to the attention of the licensee's representative.

The NRC inspector observed activation of and security personnel responsetime to the intruder alarm system.

There were no violations or deviations identified in this area of theinspection.

10. Exit Interview

An exit interview was conducted April 22, 1983, with those personneldenoted in paragraph 1 of this report. At this interview, the NRCinspector summarized the scope and findings of this inspection.
