2. Organization and Scope of Licensed Activities

The Livingston Community Hospital, located at 204 Hillside Avenue, Livingston,New Jersey, is operated under NRC Materials License No. 29-12732-01. The NRClicense expired on November 30,1989. The license was not terminated and closcoutsurvey msults were not submitted to the NRC. In September of 1988, the hospitalfiled for bankruptcy and its propeny was bought by Saint Barnabas Medical Centerlocated at Old Short Hills Road, Livingston, New Jersey. In a letter datedFebruary 8,1993 (Attachment 1), Ira Garelick of Saint Barnabas Medical Centerstated that when Saint Barnabas acquired the propeny, the former licensee had alreadyleft the site. He stated that no radioactive materials were tansferred to, or disposed ,

of, by Saint Barnabas Medical Center.

3. Confirmatory Survey Resul11_

Twelve wipe samples were obtained at the Livingston facility using filter paper disksin the locations identified in Appendix A of this report and placed in distilled water inglass counting vials. The wipe samples were obtained in several areas where licensedmaterial was used and stored. All materials, wastes and equipment had been removedand transferred to Snycor International Corporation, Fairfield, New Jersey. The NRCcontacted the Radiation Safety Officer for Syncor International, K. Wayne Wong, whoremembered receiving materials and equipments from Livingston CommunityHospital, but was unable to give specifics. The samples were analyzed in theRegion I laboratory using a Packard 2250 CA Liquid Scintillation Counter. Noremovable contamination was found above background.

A radiation survey was performed of the former nuclear pharmacy and imaging roomat the Livingston facility. Contact surveys were taken on the sink, piping, cabinet andlead storage containers, and a general radiation survey of the ama was made. Thesurvey instmment used was a Ludlum Model 3, S/N NRC 006314, calibrated onJanuary 14, 1994. None of the measurements indicated radiation levels abovebackground.

Regarding our recent conversations, please be advised that onJanuary 14, 1993, I performed a close-out survey of facilities i

formerly occupied by the above-mentioned licensee, Livingston {Community Hospital. This facility ceased operations at the end of :

1989. Please be advised that when St. Barnabas Medical Center ;

acquired the property, .the former licensee had already left the '

site. At no time has St. Barnabas used the facility as a workinghospital.

A survey and inspection was made of the former nuclear pharmacy and |imaging room. The survey was performed with a Ludlum Model 2 GM- |meter with a pancake probe (calibrated by the manufacturer on 12-

{11-92) ; results showed all areas indistinguishable from background ilevels. A wipe test showed no removable contamination above '|background. The visual inspection revealed no sealed or unsealed }sources of radiation. There were no radiation safety records

j|available to review. I suggest you contact personnel associatedwith the licensee regarding the final disposition of their .ireference sources.
