HOSPICE CARE What is Hospice? Hospice care provides palliative and supportive care for the terminally ill and their families rather than curative treatment for the underlying medical condition. Most of hospice care is provided at home, but short term in patient care is available if the patient’s conditions so requires. Who Qualifies? To receive Medicare coverage for hospice care, a patient must be certified as terminally ill by the patient's physician and/or the hospice staff physician. Additionally, hospice care must be part of a written plan of care established by the attending physician and hospice medical professionals. If coverage conditions are met, Medicare is available for an array of services, including: ✓Nursing care ✓Home health aides/Hospice Aides

✓Physician services ✓Homemaker services

✓Counseling services for ✓Medical social services;

the patient and the family ✓Medical supplies, equipment

✓Short-term inpatient care and pharmaceuticals (including pain medication);

✓Physical, occupational, and speech therapy.

How do you apply for Hospice? A patient certified as terminally ill may elect to opt into hospice coverage. The hospice care must generally be provided by, or under arrangement with, one Medicare-certified hospice program.


What happens to Medicare A & B or Medicare HMO coverage? When a patient elects the Hospice program, they opt out of Medicare A & B or C in regard to the terminal diagnosis only. If the patient requires medical treatment for another independent condition regular Medicare will still be available. How long will Medicare provide Hospice services? Hospice care coverage can be received for two 90-day periods , one 60-day period, and then an unlimited number of additional 60-day periods if approved. Upon the expiration of each coverage period a subsequent period can be approved if a physician certifies that the patient has less than 6 months to live, after each period ends. What happens if I change my mind? A patient may elect to change programs once during each period, and may elect to opt out of the hospice program, and back to regular Medicare coverage, prior to the end of the 90 or 30 day period. If the patient is eligible, the patient can return to the Hospice program at any time. If a patient revokes the hospice care election prior to the end of a period, the patient will be reinstated to the Medicare program they were receiving prior to Hospice. In this case the patient loses the remaining Hospice days in that period.
