Dear Friends,

It is with pleasure that we take this time to reflect on theaccomplishments of 2011 and thank all of our generousdonors. Together with our partners, HCP contributed tothe examination and treatment of over 400,000 patientsand provided over 40,000 surgeries. Of equal importance,your support helped strengthen the local ophthalmiccapacity in ten countries in the Himalayan region andSub-Saharan Africa – Nepal, India, Bhutan, China, NorthKorea, Ghana, Ethiopia, Tanzania, Kenya and South Sudan(the world’s youngest country).

After working in mountainous Asia for over fifteen years, itis gratifying to see that high-quality eye care has becomethe standard of care, particularly in Nepal, northern Indiaand Bhutan. It is equally satisfying to see the developmentof quality eye care programs taking hold in Sub-SaharanAfrica, where we are developing critical infrastructure fortwo eye care institutions in east and west Africa. Concurrentto the infrastructure investments, we are focused on skillstransfer training for eye care personnel at all levels-inhospital and non-hospital outreach settings. We arefortunate to have volunteer ophthalmologists who donatetheir time to provide critical follow up training with ourpartners who live and work in places of great need. In 2011,there were 24 weeks of donated ophthalmologist time.

We would also like to take this opportunity to thankUSAID/ASHA for its support of our programs in Nepal andGhana including the Tilganga Expansion, Refractive

Surgery Unit and Renewable Energy Demonstrationprojects at Tilganga, and support to construct and equipthe new training facility at the Komfo Anokye TeachingHospital in Kumasi, Ghana.

Finally, we would like to introduce our newest program,HCP Procurement, which was initiated in November 2011to improve the sourcing of ophthalmic equipment, bothfor HCP and for externalpartners. In its first two monthsof operation, the programsourced over $460,000 inquality equipment for HCPprograms and an additional$15,000 for external parties.The procurement programensures that our partnerfacilities receive qualityequipment at good prices – maximizing every donor dollar.

Since the formal inauguration in April 2009, the Tilganga Institute ofOphthalmology continues to expand its services. With increased capacity,an addition of 110,000 square feet, it can treat more patients, train moreophthalmic personnel at all levels and offer more expertise and in depthsupport to community centers and ophthalmic teams in Nepal and aroundthe world.

Training Profile: SICS TrainingManual Small Incision Cataract Surgery (SICS) with an intraocular lensimplant continues to be the most efficient procedure for reducingcataract blindness in the developing world. In 2011, Tilganga trained 10ophthalmologists in SICS. In addition to supporting the training, HCPprovides equipment and consumable support for the trainee to return tohis or her home hospital ready to provide cataract care.

Training at TilgangaIn 2011, HCP supported the training of:

• 1 full year sub-specialty fellowship.• 10 Ophthalmologists in SICS.• 4 Ophthalmic Assistants (2 from Ethiopia; 2 from Bhutan).• 20 trainees received Primary Eye Care Training.

ObservershipsHCP supports sub-specialty training for Tilganga ophthalmologistsin the US (see our Affiliated Medical Institutions, page 32)through observerships. In 2011, four doctors from Tilganga

received training in retina and neuro-ophthalmology at theJohn A. Moran Eye Center in Utah where Co-Director Dr. GeoffTabin leads the International Division of Ophthalmology.

Eyebanking: Improving Corneal CareTogether with Tilganga and SightLife, HCP continues tosupport strengthened eye banking and cornea care in Nepal.In 2011, 163 corneas were collected for surgery at Tilganga’seye bank, of which 150 were used for transplantation atTilganga, 6 were donated to Bhutan and 7 were distributed toother hospitals in Nepal.

Increased Care at Hetauda – Tilganga’s FirstCommunity Eye HospitalOver 3,300 surgeries were provided at the Hetauda Community Eye Hospital in 2011.Community Eye Hospitals serve as surgical facilities with at least one full time ophthalmologiston staff and a full team of eye care professionals. The team at Hetauda is engaged in extensiveoutreach work, and they have steadily increased the surgical volume each year. The numberof surgeries has more than doubled since 2009 after the HCP investment in infrastructure –see chart below.

Dolakha ExpandsTogether, HCP and TIO have made a long-term commitmentto restoring and reopening the Dolakha Lions CommunityEye Center (DLEC). Over the next several years, HCP and TIOwill increase its support for training and equipment andbring the center under the Tilganga umbrella by cyclingTilganga residents and doctors through the hospital on aregular basis. Each year Tilganga’s MD Residency Programaccepts 5 new residents who cycle through the 9 differentcommunity eyecenters supportedby Tilganga.

In 2011, twooutreach eventstreated 877 patientsand provided 277cataract surgeries.

Outreach Profile: TibetTeams from Tilganga are actively engaged inoutreach efforts throughout the world. In2011, Tilganga’s Dr. Govinda Paudel traveledthe Lhasa Institute of Eye Care (LIEC) inSeptember to direct a 3-day trainingworkshop on advanced cataract techniquesincluding SICS and phacoemulsification.During the workshop, 114 cataractoperations were provided.

“Site Visit by USAIDIn September, HCP and Tilganga hosted two USAID engineers for aninspection of the TIO facility. USAID’s Division of American Schools andHospitals Abroad (ASHA) has provided $3.2 million of the $7 milliontotal Expansion projectbudget and $700,000 of the$1,107,900 total RefactiveSurgery Unit budget. Theinspection consisted of athorough walk-through and detailed file review of the construction andcommodities purchased with USAID funds since 2003.

Himalayas:Increasing Care through Enhanced Trainingand Infrastructure SupportAs the only eye hospital in its district, the Jomgon Kongtrol Eye Centre (JKEC)continues to receive an increasing number of patients, which led to theconstruction of a 5,066 square foot wing to accommodate higherpatient volume.

In collaboration with the Tilganga, the HCP supported two high-volumecataract workshops in Uttar Pradesh that provided 1,252 surgeries. HCP hasbeen working with two charity hospitals run by Jagadguru Kripalu CharityHospitals in rural Uttar Pradesh, a state in Northern India where access toquality eye care is lacking.

Progress in BhutanSince 2000, HCP has partnered with the Royal Government of Bhutan to developa comprehensive system of eye care for the nation of over 700,000, supportingthe sub-specialty training for eight Bhutanese ophthalmologists and dozens ofparamedical staff in addition to providing over 7,000 sight-restoring eye surgeriesand distributing over 10,000 pairs of reading glasses. In December Dr. Bhim Rai,Bhutan’s first vitreo-retina specialist, completed a 15-month fellowship atTilganga and HCP procured specialized equipment for his new vitreo-retinapractice at the National Hospital in Thimpu. As Dr. Rai was preparing to return toBhutan, he overlapped with two Bhutanese colleagues that HCP sent to Tilgangawho arrived for paramedical training in vitreo-retina.

Highlights from 2011:

• Completion of Dr. Bhim Rai’s 15-month vitreo-retinafellowship and procurement of equipment necessaryto start new practice.

• Increased patient care – see table.

• Successful development of Dr. Nor Tsherings CorneaPractice, including developing a system for tissueexchange from the Eye Bank at Tilganga.

• Dr Tabin’s visit in November included an eye campconducted at Paro District hospital where patients receivedspecialized consultation and surgical care on site or areferral to Dr. Nor at Thimpu National Hospital.

• Two ophthalmic assistants graduated from the Royal Institute of Health Sciences in Thimpu.

ETHIOPIAHCP Support in Ethiopia ExpandsArbaminch Hospital, located in southern Ethiopia, is home to Dr. YewubneshHailu who has been working with HCP since 2009. In May, Dr. Matt Oliva,HCP Affiliated Ophthalmologist Dr. Sam Cady, Dr. Tilahun Kiros from QuihaZonal Hospital and sixnurses from Quihatraveled to Arbaminch foran HCP supportedhigh-volume cataractworkshop that provided509 eye surgeries in5 days.

Eye Banking inEthiopiaIn October, HCP, SightLifeand ORBIS Internationalsigned a memorandumof understandingoutlining a five-yearproject to strengthen eye banking in Ethiopia. Through the partnership, theorganizations will work to strengthen the local eye bank, train and equip localcorneal surgeons and provide Ethiopians with access to successful transplants.Ethiopia has one of the highest burdens of corneal blindness in the world, withan estimated population of 300,000 bilaterally corneal blind.

Sub-Saharan Africa:Komfo Anokye Teaching Hospital (KATH) –Eye Care Center of ExcellenceIn 2011 HCP and KATH continued our joint effort to establish an eye surgery train-ing center for West Africa modeled after the Tilganga Institute of Ophthalmologyin Nepal. In addition to administering the USAID supported construction project,HCP has contributed to the training of several KATH clinicians, administrators andresidents through follow-up visits by HCP affiliated ophthalmologists and personal-ized trainings.

Sub-Saharan Africa:Mbola, Tanzania – HCP’s 8th MillenniumVillages Eye Care Assessment & InterventionIn August, Dr. Geoff Tabin worked with the UN Millennium Villages Project toprovide eye care in the Tabora Region of Tanzania. Dr. Tabin was joined byAffiliated Ophthalmologist Dr. Alan Crandall, a team from the ophthalmologydepartment at Weill Cornell Medical College including Drs. Don D'Amico, PaulChan and Grace Sun, two ophthalmic assistants from Tilganga, Mark Orrs, a PhDresearcher from Columbia University, a local cataract surgeon and ophthalmicassistant from the Kiguma Development Foundation, and a resident fromKilimanjaro Christian Medical Centre (KCMC) in Moshi, Tanzania.

The team provided 414 surgeries through the intervention. Almost all of thepatients were totally blind prior to the surgical workshop, and as such, there wasconsiderable joy during the post-operative exams. In addition to the surgicalintervention, Mark Orrs continued with research on the economic impact ofsight-restoration. This research is ongoing and will become part of a larger researchinitiative to be undertaken in Ethiopia in 2012.

Care and Training in KenyaIn Kenya's western Nyanza province, there isone ophthalmologist per million people. In July,HCP International Fellow Dr. Matt Bujakscreened approximately 500 patients andprovided 90 cataract surgeries at an outreachevent organized by the Sabatia Eye Hospital,the largest eye hospital in Western Kenya.

Dr. Bujak is a Toronto-based ophthalmologist who has traveled worldwide as partof an international fellowship supported by the HCP.

Ongoing Training and

To-date, HCP has managed interventionsin Bonsaaso, Ghana; Koraro, Ethiopia;Ruhiira, Uganga; Sauri, Kenya; Pampaida,Nigeria; Mayange, Rwanda; andMwandama, Malawi, andMbola, Tanzania,screening over 21,000 people and providingcataract surgery to over 1,200 people. South SudanHCP succeeded in its long-term goal of enhancing local ophthalmic capacitythrough training and providing much needed care in South Sudan, where it isestimated that 10% of the population is completely blind. Dr. Geoff Tabinworked with colleagues from the John Dau Foundation and the Moran EyeCenter to manage an eye care workshop at the Duk Lost Boys Health Clinic inDuk County. The HCP has worked with the John Dau Foundation since 2008,including training two ophthalmic assistants from Duk County at the TilgangaInstitute of Ophthalmology in Kathmandu, Nepal.

The team provided 200 sight-restoring cataract surgeries and 100 trichiasissurgeries for trachoma in a village with no running water and no electricity.The youngest patient was 14 months old and the oldest was 92.

Fellowships for Sub-Specialty training include three months of academic training and surgery observation at an academic medical center, typicallythe University of Utah’s John A. Moran Eye Center, or a comparable facility, followed by an additional three to twelve months of training in Nepalor India. Specialties include pediatrics, cornea, retina, glaucoma, oculoplastics and uveitis. Each fellow is paired with a Professor who prepares thespecific course of study, appropriate to each fellow’s background and objectives. Accommodation and living stipend are provided.

HCP trains surgical teams in increasing numbers each year, as we believe training local providershas the greatest multiplier effect on eradicating preventable blindness. Training takes the formof fellowships, exchanges and mentoring, both onsite and off.

BBC airs documentary featuring HCP work in NepalOn February 10th, the BBC aired a documentary on the “Human Planet” program highlighting an outreachevent in Nepal. Since the program was broadcast, HCP has received an influx of emails and donations fromthe UK. Thanks to BBC and to all the viewers who responded with letters and support.

Jakarta Post features Dr. Sanduk Ruit’s work in IndonesiaThe Jakarta Post featured an article about Dr. Ruit’s work in Indonesia. In June, Dr. Ruit performednearly 800 cataract surgeries in North Sumatra. Dr. Ruit plans to provide surgical training for 50physicians in Indonesia enabling them to provide sutureless cataract surgery for patients throughoutthe country. According to Dr. Ruit, “some 48 percent of blindness in the world is due to cataracts,especially resulting from negligence and the inability to afford surgery. Cataract blindness can in factbe cured but there are patients who can’t undergo surgery for financial reasons.”

Himalayan Cataract Project’s far-reaching work is profiled inEye World magazineEye World, the magazine of the American Society of Cataract & Refractive Surgery, wrote a profile onHCP's in-depth work throughout the Himalayas and Sub-Saharan Africa. Many of the nonprofit'saccomplishments are due to the infrastructure HCP has worked to implement, the reporter said.

"That's what HCP is really about — infrastructure, training, and education, not just fly-by visits in a vacuum. These doctors dig in, training localpractitioners and leaving behind a legacy of cataract surgery excellence that continues to benefit the area well after the HCP team returns totheir countries."

Dr. Geoff Tabin speaks at Clinton Global Initiative eventDr. Geoff Tabin was the keynote speaker at a dinner event held in conjunction with the Clinton GlobalInitiative (CGI) in New York, whose mission is to inspire, connect and empower a community of globalleaders to forge solutions to the world’s most pressing challenges. The dinner was sponsored by StandardChartered Bank, a long-time supporter of eye care through its “Seeing is Believing” campaign.

Geoff’s talk, Widening Our Lens: Seeing Global Health as an Economic Investment, underscored the economicimplications of blindness. The following day, Standard Chartered Bank announced that it will raise an additional$100 million for eye care from 2012 through 2020 through its “Seeing is Believing” campaign.

HCP donor raises $10,000 to support remote eye campsHCP donor Drew Hosea created a fundraising page to raise money in his sister’s memory for what wouldhave been her 40th birthday. Mary Beth Hosea was nearly blind at the end ofher life due to a malignant brain tumor.

San Francisco Marathon runners raise morethan $11,000 for HCPThe HCP was well represented at the San Francisco Marathon on July 31st.Nine runners volunteered to raise more than $11,000 to support our effort toalleviate unnecessary blindness. Representatives from the HCP included Geoff Tabin, Matt Oliva, Huck Holz, Job Heintz, Emily Newick, PamClapp and Emilia DeMarchis. HCP board and staff send their heart-felt appreciation to Pratik, Karnika, Mike, Gillian, Abbey, Lisa, Oliver, Joshuaand Swikrit, and to all who made donations!
