2. gingivectomy :excision of the gingiva by removing pocket wall 3. Indications 1-Elimination of suprabony pockets[ 2-Elimination of persistent gingival enlargements 3-Elimination of suprabony periodontal abscesses 4-crown lengthening 4. Contraindication: 1-The need for bone surgery 2-Intrabony pocket. 3-Inadequate attached gingiva 4-Esthetic consideration , particularly in the anterior jaw. 5. Advantages: 1-Technically simple , good visual access 2- Complete suprabony pocket elimination 3-Predictable morphological result 6. Disadvantage: 1-Very limited indication 2-Gross wound , postoperative pain 3-Healing is by secondary intention. 4-Danger of exposing bone 5-Loss of attached gingiva 6-Exposes cervical area of tooth (sensitivity (,esthetic,caries 7. Gingivoplasty: A surgical reshaping of the gingiva. 8. Indication of gingivoplasty: 1-thick gingival margin 2-case of gingival cleft 3-unsatisfactory gingival contour following gingivectomy 4-gingival asymmetry 9. Types of gingivectomy : 1-surgical gingivectomy. 2-gingivectomy by chemosurgery. 3-gingivectomy by electrosurgery. 4-gingivectomy by cryosurgery. 5-Gingivectomy by laser. 10. Surgical gingivectomy 11. :gingivectomyforInstrument used 1-peridontal probe 2- pocket marker forceps 4-orban papillae knife 5-bard-parker blades #12 and #15 6-scissors 3-kirkland knife 7-curette 12. Surgical gingivectomy procedure : Steps in surgical gingivectomy: 1-case selection 2- mark the pocket 3- resect the gingiva 4-remove the granulation tissue 5- remove the calculus 6-place the periodontal pack 13. A, Discontinuous incision apical to bottom of the pocket indicated by pinpoint markings. B, Continuous incision begins on the molar and extends anteriorly without interruption 14. Periodontalpack(p.dressing) Types: 1-zinc oxide eugenol pack 2-non eugenol pack Advantage: 1-reduce postoperative pain 2-facillitate the healing 3-prevent plaque accumulation 15. Post operative instruction: 1-ice application immediately after surgery for one hour 2-keep the pack in place until remove after 1week 3-avoid smoking , brushing over the pack , citrus juice and spicy food 4-use chlorhexidine gluconate rinses 5-tell pt. swelling and bloody stain saliva are not unusual5 16. :Healing after surgical gingivectomy :Basic healing is by secondary intention 1-protective surface clot which replaced by granulation tissue (2-24hrs , increase in CT cells (angioblast by third day numerous fibroblast 3-capillaries from blood vessels of PDL migrate to the granulation tissue and within two week connect to the gingival tissue 17. 4- 5-15 days after surgery surface epithelization generally completed 18.
