    THE SOLDIERS who accompanied the rst settlers are thepredecessors of the thousands of men and women who havedonned uniforms to protect the United States. The region isproud to house bases associated with each branch of the mili-tary, including: Joint Base Langley-Eustis, Fort Monroe, CoastGuard Station Portsmouth and numerous Navy bases includ-ing Naval Station Norfolk, Naval Air Station Oceana, Joint Expe-ditionary Base (JEB) Little Creek-Fort Story.

    With so much presence in the area, the Navy is the heart of theregion. With more than 95,000 uniformed men and women cur-rently stationed here, the Navy has had undeniable impact on theregions history, economy and culture. More than 300,000 resi-dents are part of the Navy family, plus an overwhelming numberof retired or separated Sailors who choose to remain here.

    Naval Station Norfolk is the worlds largest naval base. Morethan 60,000 military and 10,000 civilian personnel work on thebase, which homeports more than 80 Atlantic Fleet ships and16 squadrons.

    The worlds largest amphibious base formerly Naval Am-phibious Base Little Creek and now part of Joint ExpeditionaryBase Little Creek-Fort Story is the homeport for 30 amphibi-ous ships, including patrol aircraft and salvage ships. About 80tenant commands are based at JEB-Little Creek, including Na-val Special Warfare Group II, East Coast-based SEALs and theArmed Forces School of Music.

    JEB-Fort Story is home to the 11th Transportation Battalion,which conducts amphibious operations. It is the Armys onlysaltwater-purication training site. The site offers amphibioustraining for all branches, including explosive ordnance disposalSailors and Navy SEALs.

    Naval Air Station Oceana is the most sophisticated and criti-cally important air station in the Navy, with collectively almost20 squadrons and almost 300 aircraft stationed there. The baseemploys 10,500 military and 2,340 civilian personnel. Dam NeckAnnex, also part of NAS Oceana, hosts the Fleet Combat Train-ing Center, which trains more than 1,000 students each month.Dam Neck employs another 2,412 military and 1,080 civilianpersonnel.

    Naval Weapons Station Yorktown is the Navys premiere weap-ons facility. More than 3,000 military and civilian personnelwork on the 11,500-acre base, maintaining missiles and otherordnance for the Atlantic Fleet.

    PERSONNEL SUPPORT ACTIVITY:The Personnel Support Activity located on Naval Station Norfolkprovides one-stop service personnel reporting to or departingfrom commands in this area.     HOUSEHOLD GOODS:The Consolidated Personal Property Shipping Offices (CPPSO)

    in Norfolk and at Joint Base Langley-Eustis has a full rangeof services available including storage of household goods,baggage and vehicles. Contact the CPPSO upon arrival even ifthe delivery address is unknown. For those who plan to live onthe Southside, call 443-3700 and follow the prompts. For thosewho plan to live on the Peninsula, call 764-2045.

    Before signing delivery documents, note all known loss ordamage sustained to the shipment. If any property wasdamaged or lost, contact the CPPSO as soon as possible tofind out how to file a claim.

    DEFENSE PERSONAL PROPERTY SYSTEM // www.move.milIn an effort to make moving a little bit easier for you, the USDepartment of Defense (DoD) United States TransportationCommand (USTRANSCOM) and the Military SurfaceDeployment and Distribution Command (SDDC) developedthe Defense Personal Property Program, or DP3. As part ofthe DP3 mission, an internet-based system to manage DoDhousehold goods moves was created. Its called the DefensePersonal Property System (DPS).

    Through DPS and the DP3 initiative, you will have the abilityto self-counsel and submit your application online, track yourshipment through DPS or Interactive Voice Response (IVR),have the option to file a claim online, and to settle directly withyour moving company as well as Full Replacement Value (FRV)of lost and damaged goods.

    Command career counselors can set up a quota. The CDRC isavailable for individual services. For service members who arestill unsure about separation or retirement, FFSC offers CareerOptions Navy Skills and Evaluation Program (CONSEP).

    EXCEPTIONAL FAMILYMEMBER PROGRAM:Identifying resources to meet the special needs of familymembers can be overwhelming while moving. Special needsconsist of ongoing requirements for medical treatment,psychological services or special education. FFSC can assistfamilies with the Exceptional Family Member Program (EFMP)enrollment process and find suitable resources for care andinformation.

    SPONSOR TRAINING:This single-session training teaches command personnel toserve as sponsors. Topics include ways to be an effectivesponsor, duties and responsibilities, military and communitysources and FFSC relocation assistance.     For Public Private Venture (PPV) housing, Navy Housingcounselors qualify members for PPV Housing. Housingcounselors will process housing applications for referral to PPVhousing. Lincoln Military Housing is the housing partner whomanagers the PPV homes. Members are also counseled onlease agreements, sales contracts, tenant and landlord issuesand other housing concerns.

    Service members can rent apartments, condominiums, townhomes and houses through the Rental Partnership Program(RPP). Properties extend from Virginia Beach to Williamsburg.The RPP provides affordable housing at a discounted rate.Military members agree to a one-year lease and pay rent byallotment.     CHESAPEAKE was formed in 1963 by a political consolidationof the City of South Norfolk with the former Norfolk County,which dated to 1691. The new city joined the ranks of thecurrent Seven Cities of Hampton Roads which are linked by thecircumferential Hampton Roads Beltway. And just like much ofHampton Roads, the area boasts an impressive history.

    Chesapeakes history goes far back into Virginias colonialroots. On the Atlantic Intracoastal Waterway, which passesthrough Chesapeake, lays the site of the Battle of Great Bridge.On Dec. 9, 1775, the red coats and colonists clashed in theBattle of Great Bridge when the American rebels defeatedthe British and drove them from nearby Norfolk. Today, themunicipal center complex rests a few hundred yards from thathistoric spot.

    Until the late 1980s and early 1990s, much of Chesapeake waseither suburban or rural, serving as a bedroom communityfor the cities of Norfolk and Virginia Beach with residentscommuting to these locations. Beginning in the late 1980s andaccelerating in the 1990s, however, Chesapeake saw significantgrowth, attracting numerous industries and businesses of itsown.

    The business community includes more than 80 foreign-based companies from 19 different countries. This economic

    development and the attractive residential properties havecreated a population boom. Chesapeake has grown from78,000 at its inception to an estimated 222,209 in 2010,according to the U.S. Census Bureau.

    Today, Chesapeake is a vibrant community that enjoys the bestof two worlds one of urban variety, culture and excitementand another of rural landscapes and relaxed country living. Infact, nearly 300 farms occupy close to one-fifth of the citysland, growing corn, soybean and wheat.

    Chesapeake is host to many culturally-diverse events and ishome to many beautiful parks, lakes and rivers. The city hasmore miles of deepwater canals than any other in the country.The Dismal Swamp Canal runs through Chesapeake as well. Itis the oldest continuously used man-made canal in the UnitedStates today and has been in service for over 230 years.

    Chesapeake is home to Naval Auxiliary Landing Field Fentress,a Navy facility under Naval Air Station Oceana in VirginiaBeach.

    HAMPTON is one of Americas oldest cities and with a proudand rich history and is one of the fastest growing cities in theregion. Hampton is the most populous settlement with thename and is the sixth most populous city in Virginia.

    Hamptons history is traced to April 30, 1607 when Captain JohnSmith and company landed at Kecoughtan.     JAMES CITY COUNTY began asthe roots of our nations heritage spreadfrom Jamestown. For thousands ofyears, migratory hunters lived along theChesapeake Bay. Then in June 1606, King

    James I granted a charter to the Virginia Company to establisha satellite English settlement in the Chesapeake region. ByDecember, 108 settlers sailed from London instructed to settleVirginia, and to find gold and a water route to the Orient. On May14, 1607, the Virginia Company explorers landed on JamestownIsland.

    The first representative assembly in the New World convened inthe Jamestown church on July 30, 1619. The General Assemblymet in response to orders from the Virginia Company toestablish one equal and uniform government over all Virginia.

    The County was formally created in 1634 as James City Shire byorder of King Charles I. James City County is considered one ofonly eight original shires of Virginia to still be in existence todayin essentially the same political form. By the early 1640s, Englishsettlers began spreading up and down the county, establishinggenerally modest farms and small plantations. The chief crop,tobacco, became the cornerstone of Virginias economy for 200years.

    In 1676, after a dispute between Gov. William Berkeley andfrontier settler leader Nathaniel Bacon, Bacon set the townablaze, destroying the church, statehouse and many otherbuildings. Fire destroyed the colonys fourth statehouse inOctober 1698. As Jamestown never had recovered from BaconsRebellion, Governor Francis Nicholson and the General Assemblydecided to move the seat of government to Middle Plantation.Richmond became Virginias capital in 1780. Jamestown began toslowly disappear above ground.

    A military post was located on the island during the AmericanRevolution, and in 1861 the island was occupied by Confederatesoldiers. Little further attention was paid to Jamestown untilpreservation was undertaken in the twentieth century.

    James City Countys most important treasure was JamestownIsland, the scene of periodic commemorative celebrationsbeginning in 1807. It was saved from both desecration andcommercialization by the Association for the Preservation ofVirginia Antiquities (APVA), which in 1893 acquired 22 1/2 acresaround the church site. Since the National Park Service came onthe scene in 1934, the two organizations have worked togetherto protect and develop the island as an important historical site.

    In 1907, the nation observed the 300th anniversary of thesettlers landing at Jamestown. A great celebration tookplace, principally at the site where the Norfolk Naval Base isnow situated. It featured elaborate exhibits and an armada ofships. A hard-surfaced road was built linking Williamsburg andJamestown for those wanting to see the actual settlement site.

    World War I brought the twentieth century to James CityCountys doorstep. The peninsula became a center for trooptraining and embarkation, munitions manufacturing, andsupply storage. By the 1920s, automobile travel had begun torevolutionize the economy. The restoration of the old capitalof Williamsburg was a vital factor in minimizing the effect ofthe 1930s depression, helping spur the local economy until thebeginning of World War II. During that war, industrial activityadded to the Countys agricultural economy. By the 1950s theeconomy was growing away from its agricultural heritage andtoward manufacturing and service-oriented businesses. TheCollege of William and Mary, long an academic and culturalpillar and leader of the areas economy, contributed further toCounty growth. Student enrollment more than doubled from1955 to 1970, leading faculty, administrators, and students toseek new housing areas beyond the city limits.

    The county is home to the Busch Gardens Europe theme park,the massive Kingsmill Resort, and the Williamsburg PotteryFactory. The Historic Jamestowne and Jamestown Settlementattractions combine with Colonial Williamsburg, and are linkedto Yorktown by the National Park Services bucolic ColonialParkway, to make worldwide tourism to the Historic Triangle amajor economic activity for the county.

    THE CITY OF NEWPORT NEWS was formally organized andbecame independent of Warwick County in 1896 by an act ofthe Virginia General Assembly. It was one of only a few cities inVirginia to be newly established without earlier incorporationas a town. Newport News is currently ranked as Virginias fifthlargest incorporated city by population and is located at thesouthwestern end of the Virginia Peninsula on the north shoreof the James River, thus playing a huge role in the maritimeindustry.

    The City of Newport News was essentially founded by CollisP. Huntington. Huntington, a Northern railroad tycoon fromConnecticut, brought two magnet industries to NewportNews: the Chesapeake and Ohio Railroad and Newport NewsShipbuilding and Dry Dock Company.

    Now known as Northrop Grumman Newport News, theshipyard, chartered in 1886, has built aircraft carriers for theNavy since the 1930s, including the first nuclear-poweredaircraft carrier USS Enterprise and is today the sole builder ofthe Navys carriers.

    The first vessel built by the yard was the tugboat Dorothy.Newport News is also home to the Armys Fort Eustis, homeof the U.S. Army Transportation Corps and to the U.S. Army

    Transportation Museum, featuring traditional and experimentaldevices like the Avro-Car which resembles a flying saucer.

    The city provides residents and visitors with plenty to do.History buffs can stroll through Civil War battlefields and1862 Peninsula Campaign sites or peruse the exhibits at TheMariners Museum, which records 3,000 years of maritimehistory.

    Children of all ages love the Virginia Living Museum whereexhibits depict Virginias natural heritage from the mountainsto the sea and introduces visitors to more than 250 livingspecies native to Virginia through discovery centers andinteractive hands-on exhibits.

    Newport News is also home to 32 city parks, including one ofthe largest municipal parks in the country.     NORFOLK is named for the large natural harbor of the same name located at the mouth of Chesapeake Bay. The city is bordered to the west by the Elizabeth River and to the north by the Chesapeake Bay. One of the oldest of the Hampton Roads cities, Norfolk is considered to be the historic, urban, financial and cultural center of the region.

    Established in 1624, the city was the colonys most prosperous town by 1775. It was an important port for exporting goods to the British Isles and beyond. But on New Years Day 1776, during the three-day siege of the Revolutionary War, most of Norfolks early structures were destroyed. In the spring of 1862, the Battle of Hampton Roads took place off the northwest shore of the citys Sewells Point Peninsula, marking the first fight between two ironclads, USS Monitor and CSS Virginia.

    1907 brought both the Virginian Railway and the Jamestown Exposition to Sewells Point. The large naval Review at the exposition, demonstrated the Peninsulas favorable location and laid the groundwork for the worlds largest Naval base.

    By 1917, as the United States built up to enter World War I, Naval Air Station Hampton Roads had been constructed. Now home to Naval Station Norfolk, the largest Navy base in the world, it is impossible to miss the imprint of the Navy at work and at play.

    Norfolk has a long history as a strategic military and transportation point. The North Atlantic Treaty Organization (NATO) has its defense headquarters here. It also has the corporate headquarters of Norfolk Southern Railway, one of North Americas principal Class I railroads and Maersk, the worlds largest shipping company.

    Norfolks easily-walked downtown waterfront boasts attractions such as the battleship USS Wisconsin and Nauticus, The National Maritime Center, an interactive science and technology. Tugboats and visiting cruise ships share the waters with sailboats and merchant ships. Other treasures include The Chrysler Museum, Norfolk Botanical Gardens and the Virginia Zoological Park.

    Norfolk serves as home to the two highest level professional franchises in the state of Virginia, the Norfolk Tides baseball team and the Norfolk Admirals hockey team. See page 40 for more information on those and other local sports teams!

    POQUOSON is an independent city located on theVirginia Peninsula, and is the oldest continuouslynamed city in Virginia. It is also one of the few to retaina name which derived from the Native Americans whoinhabited the area before colonization by the Englishbegan in the 17th century.

    The term poquoson was used to describe aboundary line between two elevated tracts of land.Such a boundary contained a stream, river, or creekwith its adjoining marsh which lay between two tractsof higher ground. Poquoson was used as a commonnoun and is found in many deeds along the easternseaboard. Through the years the term became aproper noun for the land that lies between two suchPoquosons the Old Poquoson River and the NewPoquoson River.

    The first mention of Poquoson was in CaptainChristopher Calthrope land grant issued by a court inElizabeth City on April 26, 1631. Three years later thePoquoson Parish was named as a beneficiary in thewill of Benjamin Symms for a free school to educateand teach the children of Elizabeth City and Poquoson.This New Poquoson Parish originally included the areas

    known today as Poquoson, Tabb, Grafton, Dare andSeaford.

    The southern portion of the Poquoson District in YorkCounty was incorporated in 1952 to retain controlover its schools. The city form of government wasadopted in 1975. Mrs. Carroll Lee (Peggy) Moore III isthe designer of the City logo. Her entry was judgedthe best in a contest during the National BicentennialCelebration.

    The Bureau of Economic Analysis combines the city ofPoquoson with surrounding York County for statisticalpurposes. Poquoson, which was formerly part ofYork County, became an incorporated town in 1952and an independent city in 1975. However, the tiesremain close. Over 30 years after Poquoson becamea politically independent entity, some constitutionalservices such as the courts, sheriff and jail continue tobe shared with neighboring York County.

    PORTSMOUTH, named for Portsmouth, England, is thesmallest in Hampton Roads. It was founded in 1752 by an actof the Virginia General Assembly on behalf of Col. WilliamCrawford, a wealthy merchant and ship owner, who dedicatedthe four corners of High and Court Streets for a church, amarket, a courthouse and a jail.

    Directly opposite of Norfolk, the city of Portsmouth has milesof waterfront on the Elizabeth River as part of the harbor ofHampton Roads and is another Hampton Roads communitythat boasts a lot of waterfront shorelines. Portsmouth isconnected to the rest of Hampton Roads by two underwatertunnels the Midtown and Downtown tunnels.

    Portsmouth is also home to Naval Medical Center Portsmouth.Established in the 1830s, the facility is the Navys oldestcontinuously operating medical hospital. The Norfolk NavalShipyard, the oldest shipyard in the country actually locatedin Portsmouth despite the name is a historic and active U.S.Navy facility that builds, remodels and repairs all classes ofNavy ships.     SUFFOLKOf all the Hampton Roads communities, Suffolk offers the greatest opportunity for a little extra acreage for real country living. Geographically, Suffolk is the largest independent city in land area in the entire Commonwealth. Though it is the largest of the cities in Hampton Roads, it has the smallest population.

    Suffolk was founded by English colonists in 1742 as a port town on the Nansemond River as part of the Virginia Colony. Before European contact, indigenous tribes lived in the region for thousands of years. At the time of English settlement, the Nansemond Indians lived along the river. In the early colonial years, the English cultivated tobacco as a commodity crop, but later turned to mixed farming.

    Burned by the British in 1779 and damaged by other fires throughout the next century, Suffolk survived to become a city in 1910. In 1974, it became the present City of Suffolk, consolidating with the towns of Holland and Whaleyville and the County of Nansemond. The end result was a new municipality encompassing a total of 430 square miles, making it the largest

    city in land area in Virginia and the 11th largest in the country. Peanuts grown in the surrounding areas became a major industry for Suffolk and the area is known as the heart of peanut production. Suffolk is also the birthplace of Mr. Peanut, the mascot of Planters Peanuts. In 1912, an Italian immigrant named Amedeo Obici moved from Pennsylvania to Suffolk and opened Planters Nut and Chocolate Company. Each year in the fall, the City of Suffolk hosts Suffolk Festivals Incorporateds annual Peanut Fest, hosting close to 200,000 visitors over four days.

    The charisma of Suffolk is clear and its population is expected to more than double in the next 20 years. But the city is careful about development and preserving the landscape while creating communities for new residents. The revitalized downtown features historical attractions as well as shopping and dining. Newer developments offer residents the option of suburban-style living.

    Country living provides outdoors-types with plenty to do as well. From free-fall skydiving with U.S. Parachute Association to fishing along the citys 8,000 acres of waterways the city is certain to keep you adrenaline flowing. The Great Dismal Swamp National Wildlife Refuge offers a nice getaway to bicycle, hike, bird watch or just to enjoy a family picnic.

    VIRGINIA BEACH The most populous city in Virginia, it began as a small resort area in Princess Anne County after the 1888 arrival of rail service and electricity and the opening of the original Princess Anne Hotel at the oceanfront near the tiny community of Seatack.

    The growing resort of Virginia Beach became anincorporated town in 1906 and an incorporated city in 1952. Although the resort was initially dependent uponrailroad and electric trolley service, the completionof Virginia Beach Boulevard in 1922, which extendedfrom Norfolk to the oceanfront, opened the way for automobiles, buses, trucks and passenger rail service, the latter of which was eventually discontinued.

    Virginia Beach is a resort city with miles of beaches and hundreds of hotels, motels and restaurants along its oceanfront. Every year the city hosts the East Coast Surfing Championship as well as the North American Sand Soccer Championship. The city features 79 miles of scenic waterway, 60 miles of bike paths, 38 miles of shoreline and 28 miles of public beach. In fact, the city is listed in the Guinness Book of Records as having the longest pleasure beach in the world, which is located near the longest bridge-tunnel complex in the world the Chesapeake Bay Bridge-Tunnel.

    The beaches contribute substantially to the tourism that produces a large share of Virginia Beachs economy and welcomes approximately 2.75 million visitors annually.

    Although Virginia Beach is best known for its tourism, the military and agribusiness sectors contribute considerably to the citys economy. Virginia Beach is home to three military installations Naval Air Station Oceana, NAS Oceana Dam Neck Annex and Joint Expeditionary Base Little Creek-Fort Story. The sound of freedom fills the sky every day as Navy jets swoop through the air on training missions.

    The Contemporary Art Museum of Virginia features internationally renowned artists, as well as regional and local favorites. Animal lovers should not miss the

    WILLIAMSBURG, originally Middle Plantation, a 1632 fortifiedsettlement located on high ground on the Peninsula betweenthe James and York rivers, it was renamed Williamsburgafter the capital of the Virginia Colony was moved there fromJamestown in 1698. The new city was renamed in honor ofEnglands reigning monarch, King William III.

    Williamsburg became one of Americas first planned cities. Laidout in 1699 under the supervision of Gov. Francis Nicholson, itwas to be a new and well-ordered city suitable for the capitalof the largest and most populous of the British colonies inAmerica. A succession of beautiful capitol buildings becamehome to the oldest legislative assembly in the New World. Thetown received a royal charter as a city in 1722. The young citygrew quickly into the center of political, religious, economic andsocial life in Virginia leading to the American Revolution.

    Williamsburg also became a center of learning. Famouspolitical leaders emerged from the College of William and Mary,(which had been founded in 1693), such as Presidents ThomasJefferson, James Monroe and John Tyler. The first hospitalestablished in America for the care and treatment of mentalillness was founded in Williamsburg in 1773. General GeorgeWashington assembled the Continental Army in Williamsburgin 1781 for the siege of nearby Yorktown and the winning ofAmerican independence.

    The Capital was again moved in 1780, this time up the JamesRiver to Richmond, where it remains today. Williamsburgreverted to a quiet college town and rural county seat. Inretrospect, Williamsburgs loss of capital city status was itssalvation as many 18th century buildings survived into theearly 20th century.

    The Restoration of Williamsburg began in 1926, after theRector of Bruton Parish Church, the Reverend Doctor W. A.R. Goodwin, brought the citys importance to the attention ofJohn D. Rockefeller Jr., who then funded and led the massivereconstruction of the 18th century city we see today. Nationalattention soon focused on the restoration effort. During alandmark visit in 1934, Franklin D. Roosevelt proclaimed itsmain thoroughfare, the Duke of Gloucester Street, the mosthistoric avenue in America.

    With Williamsburg known internationally as the premier centerfor the preservation and interpretation of American colonialhistory, the Historic Triangle of Virginia, which also includesJamestown and Yorktown, is among the most popular touristdestinations in the world. The three are linked by the NationalPark Services rural Colonial Parkway, a 23-mile-long NationalScenic Byway which is carefully shielded from views ofcommercial development.

    Williamsburg also offers non-automobile driving alternativesfor visitors and citizens. The intermodal WilliamsburgTransportation Center is located in a restored Chesapeake andOhio Railway station near the Historic Area, downtown, andthe College.

    the Civil War, serving as a major port to supply both northern and southern towns, depending on who held Yorktown at the time.

    After World War II, one of the nations largest and most diversified military-industrial complexes emerged in the Norfolk-Virginia Beach-Newport News MSA, spawning numerous small precision machine, metal fabrication and technical research companies. The influence of the early waterman remains active; seafood processing, ship building, and ship repair prosper.

    York County is home to several large and important military facilities of the United States. Located along the York River, Naval Weapons Station Yorktown was originally established during World War I by order of President Woodrow Wilson, and now includes the formerly separate Cheatham Annex Supply Complex. Camp Peary was established during World War II as a Seabee Training Base and is currently referred to as the Armed Forces Experimental Training Activity.

    In 1975, the county lost 15.5 square miles of land as the incorporated town of Poquoson, which had been within York County, became an independent city, although ties between the county and the new city remained close. More than 30 years later, they continue to share courts, sheriffs office, a jail and some constitutional services.

    Although the county is the third smallest in Virginia, it is one of the most densely populated in the state. It is also one of the fastest growing areas in Virginia the countys population grew more than 10.3 percent between 2000 and 2005, increasing from 56,297 to an estimated 62,100.

    PublicSchoolEnrollmentIn order to enroll a child in a Virginia public school, state lawrequires the parent/guardian to provide certain information to thechilds school division.

    BIRTH CERTIFICATE An official certified copy, not a photocopy,of the childs birth record must be provided. At the time ofenrollment, the child must be five years of age or reach his/herfifth birthday on or before Sept. 30 of the school year. Informationon obtaining a certified copy of a birth certification is available atthe Virginia Department of Health website. If a certified copy ofthe childs birth record cannot be obtained, the person enrollingthe child must submit a sworn statement setting forth the childsage and explaining the inability to present a certified copy of thebirth record.

    STREET ADDRESS Documentation of the street address or routenumber of the childs residence must be provided. If no street

    address or route number exists forthe childs residence, a post office boxnumber must be provided. If a child has noregular, fixed residence but resides withinthe school division in a temporary shelter,institution or place not ordinarily used as a residence, the schooldivision may determine that a street address, route number orpost office box number cannot be provided. The school divisionmay accept an alternative form of address that it considersappropriate.

    SCHOOL ENTRANCE HEALTH FORM A School Entrance HealthForm, MCH 213F, completed by a licensed physician, licensednurse practitioner, or licensed physicians assistant regarding thechild, must be presented at the time of enrollment. The three-partform includes Part I-Health Information Form, Part II-Certificationof Immunization and Part III-Comprehensive Physical ExaminationReport. The report must indicate that the child has receiveda comprehensive physical examination performed withinthe 12-months prior to the date the child first enters a publickindergarten or elementary school (K-5). The division superintendent or a personacting on his behalf may waive this requirement if he finds thatan individual is not eligible to obtain a SSN in accordance withguidelines established by the Board of Education or if the parent isunwilling to provide such number.

    EXPULSION STATEMENT When a student is registered, theparent must provide a sworn statement about whether the childhas been expelled. The statement must indicate whether the childhas been expelled from attending a private school or anotherpublic school in Virginia or a school in another state for an offenseinvolving weapons, alcohol or drugs or for willful infliction of injuryto another person.

    INDIVIDUAL EDUCATION PLAN Copies of an IndividualEducation Plan (IEP) must be submitted for children who requireSpecial Education or have developmental disabilities.
