Dave Glowacz The City of Chicago's 2010 budget shows funding in the Dept. of Family
and Support Services (DFSS) for two deputy commissioners (DCs), and
the 2011 budget shows funding for three DCs. However, according to
the Dept. of Human Resources, DFSS appointed these people to DC
positions on the dates shown, Merritt, Kenya K: 6/2/10’ Myers, Jenne D:
6/2/10, Rafac, Monica M: 6/17/10, Santacaterina, Amy M: 6/2/10,
Welch, Jennifer: 9/10/10. Additionally, the organization chart that
appears on the DFSS Web site shows 12 DC positions. It appears at
www.cityofchicago.org/content/dam/city/depts/fss/supp_info/dfssorgchar
t.pdf. I request any documents that identify where in the department's
2010 and 2011 budgets all of the DC positions described above appear,
what fund pays for each, and in what budget section (e.g., 3015 -
Community Services) each is funded.

January 1, 2006 to the present day. The data contained in the
database/log that I am seeking includes, but is not limited to, the url of

attempt to access to the website, any user ID, logon ID or actual name
of the user who attempted to access the website, the time and date the
user attempted to access the blocked website and any reason or
classification which would indicate why the website was blocked.

database/log of websites which City of Chicago Family Support
Services department employees have been blocked from accessing on
department computers or through department/city networks. To be
clear I am looking for a log that records each and every time the user of
a department computer/IT device has been blocked from accessing a
web page by web-filtering or internet content-control software, or any
similar method that limits department employees' access to
inappropriate/unsuitable/non-work related websites while using city
technology. Specifically, I would like the information contained in every
field of every database/log entry for the last five years, dating from
January 1, 2006, to the present day. The data contained in the
database/log that I am seeking includes, but is not limited to, the url of
the blocked website, the IP address of the computer/device used to
attempt to access to the website, any user ID, logon ID or actual name
of the user who attempted to access the website, the time and date the

between the City of Chicago and the State of Illinois and its public
aid/human services departments in order to protect the rights of the over
9,000 low-income Chicago residents who are in danger of losing cash
benefits under Article V of the Public Aid Code due to the zero funding
of the Transitional Assistance and Children and Family Assistance
programs under Article V of the Public Aid Code in the Illinois fiscal year
2012 budget recently adopted by the Illinois General Assembly. We

Roger Klose Dear Family and Support Services Senior Center programs, as I
understand it, the City of Chicago strictly prohibits using the work place
to participate in any political activities. My proposal is for a volunteer led
group of seniors at a senior center to study current events. This begs
the question - is the Senior Center a work place? I know there are many
points of view regarding most local, state and national events. The
group would try to explore and present the different sides of every issue
up for discussion and encourage participants to discuss their ideas
about what they might responsibly do about their stand on the issue. So
the question is, are senior citizens discouraged from participating in a
group like this or is this a citizen education group promoting participation
in our democratic process? Perhaps unknown to me there are already
such groups currently up and running? Please give me some guidance
on this subject.

violence). I have an education, some further traiining and some housing
however, I require transportation assistance to search for employment
and the only programs I have found are only specifically designed for
individuals who have substance abuse problems, a lack of education,
domestic violence issues, criminal backgrounds, etc.. I am not part of
those groups and I still need transportation assistance to find a job. Are
there any programs for just regular people out of work whose
unemployment benefits were cut because of an unemployment rate
quota. Please see attached resume. I have an education and some
training in different fields however the programs I have found do not
focus on regular individuals, only people with substance abuse
problems, a lack of education, domestic violence issues,etc.. WIA
provided some training for me however now I require transportation
assistance to get to the job until I receive a first paycheck. I was told
WIA only provides help with the training part. (CDL grant). Do you have
an email address to one of the case managers at the below location
whom I could ask these questions to via email? And if they are able to

residences named Ruth and Steven Strimling. Multiple neighbors in the
area have made several calls to the city in with concerns regards to the
animals, the neglect, the mess and the smell. My neighbors and I have
experienced a rat problem because of the state of the yard is kept with
garbage and feces all over. The residents have livestock, four chickens
and 2 goats living in the yard that are being neglected (already 3
chickens have died in their care.) I have seen multiple citations posted
on the front door since June, yet, the situation continues to get worse. I
want to know why this is. The yard and home are a hazard as well as a
private nuisance. I would need said citations for private nuisance
proceedings to help make this property safe again for the neighborhood.

Chicago that offer to put families of gun victims together with
professional psychologist, grief therapists or social workers on a pro
bono basis.
Greg Will Upon reviewing the documents, I have a few questions about
documents that appear to be missing from the response. I am asking
about these in case there may be such documents that you would be
able to provide.1) In the Fiscal Year 2013 Head Start and Early Head
Start Grant Application under Section 4, Training and Technical
Assistance Plan, instead of a plan there is a note saying “The Training
and Technical Assistance Plan will be submitted by September 14,
2012.” It doesn’t appear that I received the plan. I am asking, if the plan
has in fact been completed and submitted, that a copy be provided to
me.2) In the letters of certification and policy committee membership
roster, several delegate agencies appear to be missing. These are the
ones I noted in a check against the grant application:- Ada S. McKinley
Community Services- Chicago State University- Firman Community

Delilah johnson I would like the City of Chicago to forward me records concerning the
amount of welfare benefits that its various public agencies have
released to undocumented immigrants for the last fiscal period. If
possible, please include those granted exemptions for "migrant" or
"refugee" status. I am concerned about this matter because there have
been many complaints from American citizens about the lengthy and
extensive process they have to go through to receive benefits
(background checks, bank account records, property records, etc) while
it seems that those who are coming from a foreign country don't have to

Gautreaux Child Development Center.) During the
repainting/redecoration of the south wall of the Head start classroom
area, sometime between 1998-2002, a City of Chicago 1979 Percent-
for-Art commission, "Reaching Children/Touching People," a 38- foot-
long by 8-foot-high acrylic paint mural by prominent African- American
artist William Walker, was evidently destroyed. The City had
commissioned a conservator, Cynthia Kuniej-Berry, to restore the mural
in 1989. (About a decade later--the exact date is unknown--the mural
was no longer extant.)

HHS review of the city’s Head Start grant, I’d like to request a few of the
documents mentioned in the review (or the DFSS response) related to
the city’s purchase of more than $300,000 worth of EpiPens. These
include: Executive Approval Narrative dated Nov. 4, 2013, justifying the
purchase of the EpiPens. The July 2014 findings from the triennial Head
Start review (page 10 of 20) indicate that the Manager of Finance
provided the Head Start reviewers with a copy of said narrative.
Background/Distribution of EpiPens document provided by the grantee
(City) during the review. This document apparently explains the plan to
distribute the EpiPens.
Any documents that show how many children enrolled in Head
Start/Early Head Start programming have allergies, and how many of
those have “weight issues” that require higher dosage of EpiPens. (The
DFSS “monitoring response” dated Nov. 11, 2014, indicates on Page 7
that DFSS had gathered research on the allergies and enrollment data.)
Any communications between DFSS and CPS regarding the CPS
determination not to accept some 350 EpiPens, which led to a refund of
$115,297 to the federal government. Any other documents explaining

John Andersen Hi, I opened the 2011 City of Chicago Domestic Violence Help Line
highlight report and noticed the chart at the end, which listed median
victim information. I pulled up the full report 2011 City of Chicago
Domestic Violence Help Line full report as suggested by the highlight
report. I couldn't find a breakdown of either referrals to police and
shelter requests broken down by gender. It would be interesting to
learn what the resolution was for the shelter requests as well. Were
men in particular able to find shelter services?
I would think it would be difficult to assess whether any groups were
falling through the cracks when it comes to services without that
information. I'm sort of surprised it's not already available. I also
understand the help line is run by the Chicago Metropolitan Battered
Womens Network. Words matter. When someone uses a gendered
term in their organizations name, implicit bias is a concern.

writing this letter in response to your latest correspondence, which I
have read through. I am contacting you to file an appeal to the records
request I had submitted with your offices dealing with the requirements
of the Jeanne Ann Clery Disclosure of Campus Security Policy and
Campus Crime Statistics Act . Please be advised that I have concerns
about your response. Specifically, I am concerned about your failure to
promptly disclose records detailing (1) the undergraduate, graduate and
law schools you have previously attended and visited; (2) the time, the
place and the manner in which you were first informed about the rape
and murder of Jeanne Ann Clery. As a representative of the media and
a member of the general public, I am worried about the manner in which
I have come to be informed about the requirements of the Jeanne Ann
Clery Disclosure of Campus Security Policy and Campus Crime
Statistics Act. In hindsight, I am also worried about the manner in which
I have come to be informed about the brutal rape and murder of Jeanne
Ann Clery on April 1986. As a representative of the media and a
member of the general public, I cannot recall any incident of rape and
murder, which has had a more profound and devastating impact on
American society and post-secondary education than that of Jeanne
Ann Clery in the past 40 years. As a representative of the media and a
member of the general public, I feel comfortable asserting that not much
is known about Jeanne Ann Clery except what is told about her through
others. Specifically, not much is known about what she thought of
issues dealing with chauvinism, discrimination, misogyny, racism, and
sexism. Please be advised that I have concerns about several reports,
which mention the requirements of the Jeanne Ann Clery Disclosure of

different dates 7/11/20 I filed a non emergency police report after my
husband verbally threatened my life. I filed for divorce shortly after this
