    Tips for preventing similar accidents

    1.

Ensure that everyone knows that they are not to climb onto a machine in any location other than the operator's seat.

    2

.Before leaving the operator's seat, place the loading device at the lowest position. Then take measures to prevent the forklift from movingunexpectedly, such as turning off the engine and firmly setting the brake to keep the machine in place.

    3.

Never let a worker stand between the masts. Never use a mast as a ladder.

    4.

Secure unstable loads and ensure that they will not collapse or fall over before operating the vehicle.

    Tips for preventing similar accidents

    1.

Always approach inclines, whether ascending or descending, from straight on. Never cross inclines diagonally.

    2.

To change direction, return to a flat area first.

    3.

When you have poor visibility in front, or when going down a ramp, lower the load to the lowest position possible, and go down the ramp inreverse.

    17/36

    Tips for forestalling similar accidents1

.Provide a sidewalk exclusively for pedestrians along the isles and seethat they walk on the sidewalk without fail.

    2.

If a worker is obliged to step in the area where a lift truck is operated,he/she will caution the operator and both parties will confirm mutualsafety.

    3.

A lift truck operator will check safety in the traveling direction inadvance.

  • 8/9/2019 FLT Accidents

    18/36

Tips for forestalling similar accidents1. Select and install an attachment in the lift truck that is the best suited to cargos to be transported. At the same

time fasten the cargo to prevent it from sliding down.

    2. Do not lift up the fork higher than required, when it is loaded.

    3. Determine a safe traveling course in advance and have an operator travel a lift truck along it without fail. If itcannot be avoided for a lift truck to ride over an opening, make a thorough review over the safety beforehand.

    4. A signalman will take a post where he/she can be easily seen from the lift truck and guide it, keeping a sufficientdistance for safety.

    23/36

Tips for forestalling the accident1. Use a hinged fork or a log fork that is suited to the handling of logs.

2.Never give a push to a log for the purpose of adjusting its posture. By the same token, an assistant worker (the truck driverin this case) will take a safe position in anticipation of perils resulting from sudden movement of logs.

3.Heighten the truck gates over the normal height. In addition set up bolsters on the truck body to prevent a log heap fromcollapsing.

  • 8/9/2019 FLT Accidents

    24/36

Tips for forestalling the accidentA lift truck is all likely to overturn for the structural reason, if an operator makes a sharp turn or asudden stop. Therefore, it should be avoided to tow it with a truck even for a short distance, butload it on a truck and fasten it so that it wi ll not move around while in transportation.

For your reference:A towing pin at the rear of lift trucks isprovided only for bailing them out

  • 8/9/2019 FLT Accidents

    25/36

    Tips for forestalling the accident1

.Make out a working schedule as well as working proceduresbeforehand so that lift truck jobs and other jobs wil l not be overlapped, and make them known to every worker concerned.

    2.

If such overlapping of the jobs cannot be avoided, post a signalmanon the scene

Tips for forestalling similar accidents1. Through the basic training and education for safety, drill a simple principle into the operators and workers that they must not step

in any other part of the l ift truck than the operator's compartment.*Never put a hand, foot or any other part of the body in the mast of lift trucks. It may be caught in a moving part of the mast,

causing a serious bodily injury.*Do not use the mast of lift trucks as a substitute for a ladder. He/she may slip off the mast and fall off.

    2. Observe the instructions set forth in the Article 151-11 of Ordinance on Industrial Safety and Health. They are:* Lower work equipment like a fork or bucket to the lowest position possible.* Take precautions required of loading vehicles such as switching off the engine and applying a parking brake without fail toensure their safety parking.

    3. For jobs at higher elevations, be sure to use a special purpose lift truck (otherwise called an order picking truck), or other vehiclesfor high lift work
