Introduction
Prostate cancer is a cancer that forms in tissues of the prostate (a gland in the male reproductive system found below the bladder and in front of the rectum), usually occurring in older men (NCI, 2014). For the year 2014:
• 233,000 new cases are estimated in the United States (NCI, 2014). • 29,480 deaths are estimated in the United States (NCI, 2014). • Fourteen percent of new cancer cases will be prostate cancer.
Since using early detection tests for prostate cancer became relatively common in the United States, the prostate cancer death rate has dropped. Despite the helpfulness of the tests, they aren’t 100% accurate (ACS, 2014).
The goal of Bank of America’s Swing for the Cure is to:
1. Raise awareness about prostate cancer 2. Raise money to support research for the development of a more accurate test 3. Encourage early testing
Not only will the funds raised from the golf tournament go towards research for a less invasive and more accurate test, but it will help spread awareness. With increased awareness the result will be more people getting tested and detecting prostate cancer while it still treatable. Bank of America’s Role

Background
What is Prostate Cancer?
Prostate cancer is the most common form of cancer among men in the United States. Most prostate cancer begins in the gland cells of the prostate and is known as adenocarcinoma. Prostate cancer is known as the silent killer because most men do not show any symptoms and it is the second leading cause of cancer deaths among men. However, if detected early, patients have nearly 100 percent chance of survival after 5 years. Prostate cancer can be a very serious disease, but most men diagnosed with prostate cancer do not die from it.
Prostate Cancer Statistics
1 in 6 men will be diagnosed with prostate cancer in their lifetime and one man in 36 will die from it. The American Cancer Society’s estimates for prostate cancer in the United States for 2014 are:
• About 233,000 new cases of prostate cancer will be diagnosed • About 29,480 men will die from prostate cancer
The expected new cases and deaths in Dallas County alone in 2014 are:
• About 1,467 new cases • About 177 deaths

Survival Rates by Stage
Prostate cancer is broken down into four stages, stage I, stage II, stage II, and stage IV. However, the National Cancer Institute (NCI) groups the stages into 3 groups in order to estimate survival statistics. These groups are local stage (stages I and II), regional stage (stage III and some stage IV), and finally distant stage (stage IV). According to the NCI’s national database on survival statistics, survival rates by groups are described in table below.

Fig. 5 The 5-‐year survival rate refers to the percentage of patients who live at least 5 years after their cancer is diagnosed. Many of these people live much longer than 5 years.
Prostate Cancer Risk Factors
Even though exact causes of prostate cancer are not clear, there are a number of risk factors that have been identified by the medical community. The most common risk factors are; age, race, family history, and diet.
• Age – the risk of developing prostate cancer increases with age, with most cases occurring in men over the age of 50. About 64% of cases occur in men over the age of 65.
• Family History/Genes – Having an immediate family member with prostate cancer more than doubles a man’s risk of developing prostate cancer.
• Race – African American men face a higher incidence of prostate cancer than men in any other races and are more likely to die from it than any other ethnic group.
• Diet and lifestyle

Prostate Cancer Vs Breast Cancer
According to the National Cancer Institute, men are 35% more likely to be diagnosed with prostate cancer than a woman is to be diagnosed with breast cancer. However, breast cancer has higher mortality rates than prostate cancer and receives the most funding than any other type of cancer from the NIC. Notably, prostate cancer, the most common cancer, receives the least funding. The following table shows some of the NCI spending for FY2011, 2012, and FY2013 for 4 types of cancers in the United States.

Overall Plan
Proposal
With the support of the Dallas Chamber of Commerce and Bank of America, we would like launch “Swing for the Cure,” a charity golf tournament devoted to raising money and awareness for prostate cancer in the Dallas/Ft. Worth area. We will host this event at Twin Creeks Golf Course in Allen, TX. We chose this location because the costs are reasonable and the quality of the golf is exceptional.
Event Format
The tournament will be a 144 player event (36 teams of 4). It will be a “Best Ball” format, which means that everyone will hit a shot and then choose where to hit next from the locations of each of their team members’ best shot. There will be a hole-‐in-‐one contest, closest to the pin contest, putting contest, and longest drive contest. Dinner will be held after the golf tournament, but before the announcement of the auction winners.
Payment
Payment at check-‐in is required in order to participate in these contests. The payment that is made at check-‐in covers raffle tickets, unpaid entry fees, and golf contests depending on which event they choose to participate in. There, participants can also buy mulligans (do-‐over-‐shots), drink coupons, and golf balls/tees, etc. The participants can pay for the contest fees on the hole of each contest. The entry fee is 150 dollars per person. It will be 100 dollars to participate in the hole-‐in-‐one contest and 100 dollars to participate in the other three contests.
As stated in the paragraph above, there will be raffle tickets for purchase and there will also be a silent auction held throughout the golf tournament. There will be a minimum of 25 raffle gifts and 25 auction gifts. The auction bidding will range anywhere from 75 dollars to 500 dollars. All of the gifts for the auction and raffle, along with the prizes for the winners of the contest and tournament, will be donated by the corporate partners of Bank of America and the actual parent company. The corporate partners will be able to buy and sponsor holes on the course.
This event is expected to bring in 22,560 dollars in proceeds. All of the proceeds will be donated to the North Texas Prostate Cancer Coalition.
North Texas Prostate Cancer Coalition (NTxPCC)
Mission Statement: To inform the Texas community by providing coordinated educational support about prostate cancer for all men and their families, and to be a collective voice in elevating prostate cancer to major health priority status in the State of Texas through increased awareness and advocacy.
NTxPCC works with many support groups as well as prostate awareness groups here in the North Texas are to provide information on prostate cancer. These groups offer support and information for men who have questions about prostate cancer or just need support during the battle with prostate cancer.

Us TOO
Us TOO is a local prostate cancer support group that operates in the hospitals that NTxPCC would be donating a portion of the proceeds to. This support group works to help patients with knowing more about educating, testing, and treating prostate cancer.
Free Prostate Cancer Testing
Not only does NTxPCC help with support and questions, but they also help provide free prostate cancer testing. Every second Thursday of each month free testing is available from 9 am – 11 am at Methodist Medical Center for men ages 55 and older.
DNA Blood Test
Due to NTxPCC’s involvement with the community, we chose to donate our money to them for research towards a DNA blood test. The problem with prostate cancer isn’t that it’s untreatable. The problem is that for many people the cancer is detected too late. The DNA blood test would encourage more people to get tested because it would (Dallas, 2014):
• Provide a better assessment of a man’s disease • Reveal when treatment becomes ineffective and start supporting tumor growth • Allow true personalization of treatment • Less expensive • Less invasive
Goal
Given this event’s potential for success, we hope to sponsor multiple tournaments like this in the future. We hope that this event can be something that truly changes peoples’ perception of prostate cancer. Our hope is that these tournaments help encourage men to get tested more often and at an earlier stage in life. We want to offer peace of mind to all of those men that either don’t get tested or don’t know about the different types of testing.
Earlier testing will save more lives. As stated in the information section above, 98.9% of all men survive more than 5 years after their initial diagnosis with prostate cancer. The 1.1% of men that don’t survive are the people that are too scared to get tested or don’t know about testing. They may feel embarrassed about their current situation. Men might feel scared about getting tested. Others may feel as if they are invincible and don’t need to get tested. Our goal is to reduce that 1.1% or even eliminate it. We want to show the North Texas community that prostate cancer can be easily treated and prevented with the correct actions.

There will be a lot of needed preparation leading up to the event in order for it to be a success. Below is a list of some tentative dates that will explain how we plan out this tournament:

Appendix B: Waiver and Release
In consideration of being allowed to participate in “Swing for the Cure” to be held at Twin Creeks Golf Course on the date of March 14, 2014, and any and all related events and activities (collectively, the “EVENT”), the undersigned hereby acknowledges and agrees that.
1. I acknowledge and fully understand that my participation in the EVENT may involve the risk of injury, including the possibility of serious injury or death, not only from my own actions, but also from the action or inaction of others, the condition of the premises, or the failure of the equipment used. I knowingly agree to assume all such risk, known and unknown, and assume full responsibility for my participation.
2. I agree to conduct myself during the EVENT with all due regard for my own safety and the safety of all participants, observers and bystanders. I agree to comply with the directions of all EVENT organizers, and I agree to immediately notify EVENT organizers if I observe any unsafe condition or behavior during the EVENT.
3. For myself, and on behalf of all of my heirs, assigns, executors, personal representatives and next of kin, I hereby waive and forever release the officers, directors, agents, volunteers, sponsors, donees and local organizers of the EVENT from and against any and all liabilities, claims, expenses or causes of action of any kind or nature whatsoever, arising out of or relating to my participation in the Event.
I have read this waiver and release agreement, fully understand its terms, and understand that I have given up substantial rights by signing it, and I sign it freely and voluntarily without any inducements.
Furthermore I also recognize that BOFA and the EVENT organizers have the right and responsibility to record the event through video and still photography of any and all participants. I hereby grant permission that such photography may be used for event publicity, public information, or any other promotional use that falls within PDGA or Event host activity.

Appendix C: Frequently Asked Questions
Q: Why is prostate cancer awareness such an important cause if the survival rates of those diagnosed are so high, when detected early?
A: Men are very reluctant to be tested for prostate cancer, for a myriad of reasons. With the high survival rates of those diagnosed with prostate cancer, the single most important step of treatment is simply the detection of a tumor itself. Many people may not realize the importance of early detection. Others may feel that the discomfort of standard prostate exams outweighs the importance of early diagnosis, or may be unable to afford to visit a physician. By raising awareness, we can dispel these myths, as well as emphasize the dangers of prostate cancer.
Q: Why are the proceeds from Swing for the Cure going to NTxPCC? What makes them the best organization to receive funds?
A: The NTxPCC has been chosen because they are a local organization and because of their track record in advances in prostate cancer detection and treatment. NTxPCC is a coagulation of North Texas’s leading institutions and experts on prostate pathologies. Quite simply, they are the experts in this field. At Bank of America, we believe that it would be best to give the proceeds to the experts. The proceeds will be distributed by NTxPPC to continue offering free prostate exams and research on the innovative DNA blood testing method of tumor detection.
Q: Is the DNA blood testing method a worthwhile investment when current prostate exams are proven to be a reliable method of detection?
A: The importance of blood detection is to offer a less invasive alternative to current clinical methods of prostate examination. Since many men, even those at risk, may not be comfortable with a prostate exam, a simple blood draw will be an appealing substitute. Blood detection is also an alternative to current tumor imaging techniques (X-‐ray, MRI) that will allow physicians to monitor the success of tumor treatment.
Q: What is the contingency plan in case of bad weather, such as rain, on the day of the tournament?
A: The plan in case of rain will be to move the tournament to the next day or week. We have already worked with our corporate partners, catering services, and golf course on this plan of action. There will be no additional cost on our part if this occurs.
