This booklet is a list of resources to assist adult stroke survivors and their caregivers to find community agencies that provide direct service or the link to other programs that support re-integration into the community. As it is impossible to list all
community services in the booklet, healthcare providers across the continuum of care can provide you with additional resource
information to meet your needs.
The material contained in this booklet is for information purposes only. It should not be used as a substitute for the

HOME AND COMMUNITY CARE-LOCAL HEALTH INTEGRATION NETWORK (LHIN)
Ontario’s 14 Local Health Integration Networks (LHINs) plan, fund, integrate and coordinate health care services for people living in their local communities. LHINs also deliver and coordinate home- and community-based health care and support services.
Your LHIN’s caring and knowledgeable Home and Community Care staff can help:• Connect you with the care and support you need at home,
at school or in the community following injury, illness or the complications of age or disability
• Arrange for and coordinate the provision of nursing, rehabilitation and supportive care to people of all ages
• Facilitate access to long-term care homes, assisted living, convalescent care and adult day programs
• Link you to health supports, services and resources in the community.
To access LHIN services and programs, you may self-refer or be referred by a family member, caregiver and/or health care practitioner. Patient consent is required prior to receiving any treatment or intervention. In order to receive services, you must have a valid Ontario health card (OHIP) and meet the LHIN’s eligibility criteria. There is no cost for services.

ADULT DAY PROGRAMS
Day programs offer social and recreational programs in an engaging environment for clients with disabilities or cognitive
impairment living in the community. Assistance and supervision with personal care, as well as caregiver respite, support and
education is also provided however services may vary between programs. Referrals to some of the adult day programs are
made through the Local Health Integration Network Home and Community Care Office. Please note that wait lists may exist for this service. Most of these services may require a fee to attend. Transportation to and from the program may be an additional
fee.
