    Performance Appraisal Checklist

    Have you completed blocks 1-9 of form AD-435?

    Is the appraisal period in block 7 either 10/01/2009 through 9/30/2010 (appraisal period for the ForestService) or the actual dates for seasonal employees or newly hired or terminated employees? The

    only exceptions for extending the appraisal period are (1) if there is a supervisory change within 60days of the end of the normal appraisal period; (2) if the employee will not be under the plan for atleast 90 days by the end of the normal appraisal period; (3) if an employee is on a PerformanceImprovement Plan where the observation period extends beyond the normal appraisal period.

    Have you only used the mandatoryperformance elements (3 for non-supervisory employees and 5 forsupervisory employees) on form AD-435?

    Have you entered a 2 in the rating block (15B, 15C, or 15D) for each Critical performance elementon the form AD-435?

    Have you entered a 1 in the rating block (15B, 15C, or 15D) for the Non-critical performanceelement?

    Have you correctly added columns 15B, 15C, and 15D into blocks 15E, 15F, and 15G of form AD-435?

    Have you correctly added blocks 15E, 15F, and 15G into block 15H of form AD-435? The number inblock 15H should be either a 5 for non-supervisory employees or a 9 for supervisory employees.

    Have you correctly applied the Decision Table (16B) to determine the Summary Rating in block 16Aof form AD-435?

    Has the employee checked block 17 indicating they have a copy of USDA regulations on employeeresponsibilities and conduct? This regulation may be found athttp://www.ocio.usda.gov/directives/doc/DR4070-735-001.pdf

    Have the employee, supervisor, and reviewing official signed form AD-435?

    Have you completed blocks 1-6 of forms AD-435A and AD-435B?

    Have you marked each Element Rating in block 9 of forms AD-435A and AD-435B?

    Have the employee, supervisor, and reviewing official signed in block 11 of form AD-435A? (Note:This should have been done at the beginning of the appraisal period once the performance plan wasdeveloped.)

    Have you completed at least one progress review (preferably midway through the appraisal period)and documented it in block 12 of form AD-435A?

    Have you attached any supporting documentation such as performance expectations and measuresor accomplishments done on separate pages? The complete performance appraisal includes formsAD-435, AD-435A, AD-435B and all supporting documentation.

    Note: Summary ratings may be assigned up to 30 days in advance (9/1/2010) of the end of the appraisalperiod in order to balance workload.
