• Burns Lake is located in the heart of northern British Columbia, about 222 km west of Prince George
• Aging population in Burns Lake (2012-16%, 2020-20.6%) • Community Diversity
– Aboriginal 27.7% – Visible Minority 4.4%
• Chronic Diseases – Low Chronic Complex Conditions- 1528 (23.2%) – Med Chronic Complex Conditions-547 (8.3%) – High Chronic Complex Conditions-349 (5.3%) – Frail Population living in the community 17 (0.3%) – Type 2 Diabetes is the most common type of diabetes (90%)
The purpose of the project is to create a sustainable healthcare environment in Burns Lake that will support the attraction, recruitment and retention of healthcare providers.
The local health area serves ~ 10,000 people

A Primary Care Home is where people establish a long-term relationship with a team, and through this team receive health care and are supported in managing their own health.
Building a new model of care • Practice Ready Assessments candidates available in the spring. Ongoing recruitment efforts for hospital clinic.
• Established and cohesive IPT in place
• EMR in Hospital Clinic with full functionality Early stage adoption in Private Clinic
• Developing links to community-based services
• NP integral to the care team
• Telemedicine in place
• CME: Making Burns Lake a “Learning Hub”
