IdeoRCP : tool for pluridisciplinary meetings, used

Médecin Référent tool : facilitates GP and pediatricians
« MR » (i.e. having signed a « referential doctor »-convention with a patient
suffering from long-term conditions), to supervise and share health
data by means of 2 documents registered in the DSP :
- patient summary (deposited by the MR)
- summary of health benefits (deposited yearly by the National
Health Fund)
