qualified electrical contractor prior to the operation commencing (Risk Rating4)

    3. All gas supply pipes and fittings to be disconnected and made safe by a

registered gas engineer prior to the operation commencing (Risk Rating 4)

    4. Strip out front window and cladding. Work at height of up to 4 meters will be

required during the removal operation and this will be carried out from a

suitable Sky Lift. This will be situated on firm level ground in a safe area

within the segregated work area. All personnel using this equipment have been

formally trained in its safe use and the Sky Lift will be inspected prior to its

use on a daily basis with these inspections recorded. The lift will also have

undergone the statutory inspections required with the latest inspection report

available on site at all times. Where necessary fall arrest equipment will beprovided and worn by PP Contract Services personnel who have been trained

in its safe use (Risk Rating 4)

    5. Strip out timber / plaster board office. The timber framed office will be

operating area of the Sky Lift. No ladders will be used during this operation.

Where possible the heater fixing rods will be removed by hand but where this

is not possible they will be removed using a steel hack saw. During this

operation it may be necessary to use a petrol stihl saw. If this is the case the

petrol oil fuel will be stored in an area away from the main operation and

during operation of this equipment the following PPE will be worn at all times

face mask, goggles, gloves, boots and flameproof trousers. If necessary a fire

watcher will be designated to ensure that the operation does not cause a fire in

the work area and surrounding areas. This operation will only be carried out

when the weight of the heater has been taken by the slings and the vehicle

mounted crane used in the removal operation. The gas fired heater has beensold and will be transported directly to the purchasers site removing the need

for safe storage. Personnel used in this operation will be familiar with the

equipment being used and trained in its safe operation. (Risk rating 3)

    7. Once removed the industrial gas heater will leave a hole in the roof

approximately 12 inches in diameter in the roof which will need to be made

good. The roofing material at the site is believed to be asbestos cement and

special care much be taken to ensure that the roofing material is not moved

excessively or broken. Work at height of up to 7 meters will be required

during the removal operation and this will be carried out from a suitable Sky

Lift. This will be situated on firm level ground in a safe area within the

segregated work area. All personnel using this equipment have been formallytrained in its safe use and the Sky Lift will be inspected prior to its use on a

daily basis with these inspections recorded. The lift will also have undergone

site at all times. Where necessary fall arrest equipment will be provided andworn by PP Contract Services personnel who have been trained in its safe use

    (Risk Rating 4)

    8. The area where the cladding has been removed will be made good to allow the

installation of a new roller shutter door. During this operation it may be

necessary to use a petrol stihl saw. If this is the case the petrol oil fuel will be

stored in an area away from the main operation and during operation of this

equipment the following PPE will be worn at all times face mask, goggles,

gloves, boots and flameproof trousers. If necessary a fire watcher will be

designated to ensure that the operation does not cause a fire in the work area

and surrounding areas. All work involved in mounting the fixing points for theroller shutter door will be carried out from the Sky Lift and again no ladders

will be used in this operation. The shutter door will be lifted into place again

using vehicle mounted lifting equipment and slings. Personnel used in this

operation will be familiar with the equipment being used and trained in its safe

    operation. (Risk rating 3)

    Work to be undertaken

Remove 6 post sockets from the area. Also remove all foreign cables or potential

obstructions. Shutter off area and fill with approximately 148 tonnes of in fill andapply mesh and concrete with approximately 21.4 cubic meters of concrete. Power

    float if necessary.

    Method Statement

    1. Make the area safe with the provision and erection of portable fencing on both

sides of the wall. Also suitable and sufficient warning notices will be

displayed in the working area. (Risk Rating 1)

    2. All electrical installation wiring and control boxes are to be removed by a

qualified electrical contractor prior to the operation commencing (Risk Rating

    4)

    3. Ensure safe traffic routes are designated and clearly identified to ensuresuitable segregation between vehicles delivering the concrete and in-fill and

human visitors to the site (Risk Rating 1)

  • 7/28/2019 Draft+Risk+Assessment+Method+Statements

    10/11

    4. A 7 tonne tracked machine with attached concrete pecker will be used to allowthe removal of the 6 post sockets. When this equipment is being used it will be

noisy in the surrounding areas so hearing protection must be worn by anyone

in close proximity to the operation. There is also the potential of high velocity

particles being ejected from the work area any one in close proximity should

wear eye protection and all non essential operatives should be moved from the

area.. The equipment can only be operated by trained and authorised members

of PP Contract Services personnel and must be securely isolated at all times

when not in use. (Risk Rating 3)

    5. Once the area has been suitably prepared concrete shuttering will be installed

to allow the void to be filled with approximately 148 tonnes of in-fill material.

This material will be deposited as close to the work area as possible and thentransferred to the void using the 7 tonne tracked vehicle. Any delivery vehicle

on site must be strictly controlled and must not be allowed to reverse on site

or deposit all or part of a load unless under the direct supervision of a member

of PP Contract Services personnel who will act as a banks man. Reinforcing

mesh will be put in at various levels throughout the void to ensure a sound

construction f the finished floor area. This will be lifted either manually or by

the 7 tonne tracked vehicle and suitable slings. When the in-fill reaches an

adequate depth it will be rolled using a 1.25 H/D roller. If this is the case the

petrol oil fuel will be stored in an area away from the main operation. The use

of the roller or striker will ensure that there are no unseen weaknesses or

voiClients in the finished concreted area. A striker plate may be used instead

of the roller which again has the potential to operate at a high noise level all

operatives will wear suitable personal protective equipment. (Risk Rating 3)

    6. Once the void has been filed and adequately prepared approximately 21.4

cubic meters of liquid concrete will be used to completely fill the void. When

this has been done a hydraulic poker will be used to vibrate any air pockets

within the concrete to the surface ensuring a good quality construction and

finish to the new floor area. The area will be fenced off to prevent access at all

times during the curing period. (Risk Rating 1)

    7. Following a minimum 10 day curing period the shuttering will be removed

using the 7 tonne tracked vehicle and manual handling with any defectsidentified in the new flooring area dressed and made good. . (Risk Rating 1)

As the operation will take longer than one working day the site must not be left

unattended unless it has been adequately secured against intruders and all equipment

and machinery used in PP Contract Services operations has been isolated to prevent

use by unauthorised visitors to the site.
