Poop deck platings found thinning down. 2. P,C,S Generator and Main Engine covered with lube oil at various locations. An indication that both machinery had experienced leaking of lube oil while running. 3. Self closing weight of F.O. tank short sounding pipe at lower platform was missing. 4. Insulation of F.O. pipe from service tank to main engine was partly damaged. 5. Port engine room ventilator mushroom found thinned and holed. 6. All pipe supports and U-bolt clamps on main deck were badly thinned/wasted. 7. Several steps of external ladders at poop decks found badly thinned. 8. Air pipe on main deck was leaking and some sections were badly corrode

Scope

This is to confirm that, Undersigned Surveyor attended onboard the above vessel and had carried out the

following surveys:

Postponement of RCH,sighting survey

Postponement of Bottom Survey, in water survey

Postponement of Load Line Cert., renewal ,sigthing survey

Findings

Following items were found not in order:

Main Class/Tanker for Oil:

1. Poop deck platings found thinning down. 2. P,C,S Generator and Main Engine covered with lube oil at various locations. An indication that both

machinery had experienced leaking of lube oil while running.

3. Self closing weight of F.O. tank short sounding pipe at lower platform was missing.

9. Hydraulic pipes at emergency generator room bulkhead and at main deck found badly thinned. Some

sections had been repaired and still in progress.

10. Pillar and brackets for securing the aft crane’s boom/block were found badly thinned.

11. Tranverse walkway at fwd main deck was completely deteriorated. Handrails, floor grating, ladder

steps,brackets etc. were badly corroded and thinned.

12. Inner plating of P&S chain locker found holed/thinned. As the above deficiencies have not been rectified, a condition of class given (see form CSE.001)

Safet Eqpt. Certificate

1. While testing of main and emergency fire pump, fire line at main deck found leaking due to hole/wasted.

2. Stbd. fire hose at main deck that was using for testing found leaking due to hole.

As the above deficiencies have not been rectified, a condition of class given (see form CSE.001)

IOPP Cert.

1. Side plate of stbd manifolds drip tray found thinned/holed.

2. Aft drip tray for stern loading/unloading arrangement found thinned/wasted.

As the above deficiencies have not been rectified, a condition of class given (see form CSE.001)

Load Line Cert.

1. P&S air vent heads at funnel bulkhead were badly damaged due to corrosion.

2. P&S air vent heads at cargo pump room bulkhead found thinned/wasted.

3. Air vent pipe at port forecastle deck was temporary repaired by cement.

4. Vent flap at emergency generator room was missing. Flap coaming was badly wasted.

5. Bulwark face plates and bracket flanges at forecastle and fwd P&S bulwarks found wasted/thinned.

As the above deficiencies have not been rectified, a condition of class given (see form CSE.001)

Continuos machinery survey:

1. CW pipe connector of stbd. generator found leaking.

2. CW pump of port generator found leaking. 3. Generator L.O. Cooler SW cooling pipe found leaking.

As the above deficiencies have not been rectified, a condition of class given (see form CSE.001)

Bottom in water survey:

- Bottom in water survey satisfactory carried by diver.

- Fishing rope entangled at tailshaft iwo rope guard had been removed.

Deletion of CC 138

Fixed installation CO2 bottles were hydraulic tested in 1994/04/07. All bottles found marked with test

date and pressure. Test certificate sighted onboard. Accordingly condition of class is now deleted. (see

form CSE.001)

Posponement of CC 100, CC 114, CC 123 and CC 139.

Above CCs are now postponed until 1999-07-15 upon satisfactory results of verification.

Conclusion

Upon satisfactory completion of examination, relevant renewal/complete surveys were extended. Interim

Certificate of Class and Short term certificates issued and valid until 1999-07-15. Safety Equipment

Certificate was endorsed.
