Addictive Diseases Services

• Outpatient services (core benefit package): evaluation, diagnosis, comprehensive assessment of needs, counseling, individual and family education

• Crisis services: telephone or face-to-face intervention with the individual and family to address immediate crisis and link to services; available around the clock and in any setting

• Withdrawal management (detoxification): helps adults and teens at risk of complications withdraw safely from the physical effects of alcohol and drug use

• Residential programs: intensive treatment and structure to help people live a drug-free life style; for adults or adolescents with severe addictive disease

• Special “women’s treatment and recovery support” programs: treatment for women on welfare who are unable to find jobs due to alcohol or other drug problems; includes intensive outpatient treatment, continuing care and residential treatment with a therapeutic childcare component

How to Access Addictive Diseases Services Georgia Crisis & Access Line (GCAL) (800) 715-4225 Georgia offers a statewide toll-free call center for individuals to access services. The call center operates 24/7 and has the capacity to screen and assess callers for intensity of service response.

Language Assistance: Telephone interpreting services are provided to callers with limited English proficiency. Emergencies: Individuals determined to be in immediate danger are “warm transferred” to the local 911 service in

the area where the individual is located. Call center staff do not leave the call until they have confirmation that 911 responders are on site with the caller.

Crisis Assistance: Individuals in need of crisis management will receive 24/7 mobile response to assess the situation, de-escalate the crisis, consult and refer with post crisis follow-up to assure linkage with recommended services.

Routine Service Access: For individuals with less intense needs, the call center staffs are able to offer a choice of providers, and to schedule appointments for services. Individuals are able to specify the distance they are willing to travel, and the call center identifies service providers within that proximity to the individual’s zip code location.

Individuals may also choose to contact local providers directly for services. The “no wrong door” philosophy is an important value in the Georgia system. In addition to GCAL’s call center, its website contains a resource database of service providers and contact information throughout the state. Using your zip code, you can identify a provider near you.

Adult Services DBHDD contracts with providers in all six regions to provide outpatient and residential substance abuse treatment to men and women who are struggling with the disease of addiction. To locate a community behavioral health service near you, call the Georgia Crisis Access Line at (800) 715-4225, or go to www.mygcal.com. If you have questions or need further assistance with accessing services, please contact your Regional Office.

Comprehensive Community Providers (CCPs) A CCP is a DBHDD public provider that operates within a recovery-oriented system of care with the capacity to deliver comprehensive and vital community mental health and substance use disorder (SUD) services to identified Georgians. Collectively, the CCPs are known as the Tier I Provider Network. The Tier I providers have the unique capacity and infrastructure to provide a seamless continuum of care for the target population identified by DBHDD. They also have the community stature, visibility, accountability, and credibility to be seen as the local and reliable safety net for the delivery of supports and services.

CORE BENEFIT PACKAGE AND OTHER SERVICES

A. Table 1 on the following page displays the core benefit package that CCPs must be able to provide for individuals who need such services. Some services are not available for all ages, or require special approval. The current Provider Manual for Community Behavioral Health Providers, 01-112 contains the requirements for each service.

Crisis Stabilization Units

Crisis stabilization units are a medically-monitored, short-term residential service operated by community service boards for the purpose of providing psychiatric stabilization and withdrawal management (detoxification services). The crisis stabilization program must be designated by DBHDD as both an emergency receiving facility and an evaluating facility. Specific services include but are not limited to:

1) Psychiatric medical assessment; 2) Crisis assessment, support and intervention; 3) Medically-monitored residential substance withdrawal management (detoxification) ASAM Level

3.7-WM; 4) Medication administration, management and monitoring; 5) Brief individual, group and/or family counseling; and 6) Linkage to other services as needed. Services must be provided in a facility

Withdrawal Management Services (Detoxification) Withdrawal management includes a short-term stay in a licensed and accredited community-based hospital for the treatment or habilitation of a psychiatric and/or substance use disorder. Services are short in duration and provide treatment for an acute psychiatric or behavioral episode. This service may also include medically-managed inpatient withdrawal management (detoxification) at ASAM Level 4-WM.

Ambulatory substance abuse withdrawal management (detoxification): Process of Medical monitoring of the physical withdrawal from alcohol or other drugs in an outpatient setting for those individuals with an appropriate level of readiness for behavioral change and level of community or social support. This level is appropriate for individual experiencing physiological dysfunction during withdrawal, but life or significant bodily functions are not threatened. Residential substance withdrawal management (detoxification): An organized and voluntary service that may be delivered by appropriately trained staff who provide 24-hour per day, 7 day per week supervision, observation and support for individuals during withdrawal management (detoxification); residential withdrawal management is characterized by its emphasis on medical monitoring or on peer and social support, and should reflect a range of residential withdrawal management service intensities from ASAM Level 3.2-WM to 3.7-WM.

Residential Services Independent Residential Services: A community-based program, closely supervised to provide a structured, safe and drug-free environment for individuals who require a low level of residential treatment; the IRS allows recovering individual to live independently and work while receiving ongoing treatment support to maintain sobriety, stable housing and increase self-sufficiency. Semi-Independent Residential Services: Provides onsite low intensity programming that meets ASAM Level 3.1 for individuals with a substance use disorder, within a residential setting to assist them to successfully maintain sobriety while transitioning into recovery. Intensive Residential Services: Provides a 24/7 clinically managed medium/high intensity services that meet ASAM Level 3.5 to individuals in a residential setting with a substance use disorder; assist them to successfully maintain sobriety while transitioning into recovery. Through skill building programming, individuals can then transition into stable housing in the community while increasing self-sufficiency.

Adult Recovery Support Services Adult Recovery Support Center provides hope, healing and wellness to individuals in a safe and comfortable environment. The Center is designed to assist individuals with substance abuse problems establish, maintain and enhance recovery by utilizing peer support approach.

I. Target Population Individuals 18 years or older and their families affected by the disease of addiction. There is no diagnose needed to receive services.

II. Service Deliverables: The following services are available at each center: • Employment services and job training • Housing assistance and services • Educational/Vocational • Transportation, directly or indirectly provided to support employment, vocational,

educational, or recovery support activities, etc. • Relapse prevention • Family Support • Case Management, linkage to other identified services • Peer to peer services, mentoring and coaching • Self-help and support groups • Life skills

Adult Transitional Housing Services

The transitional housing services provides a safe, stable, drug free residence for a minimum of 30 days to individuals discharged from a Crisis Stabilization Unit (CSU) who are without a safe and sober home environment to return to. These transitional housing services coincide with AD Intensive Outpatient Transitional Services. Transitional housing is part of a continuation in service; thus, a positive completion from a CSU is necessary for admission into this program.

Medication Assisted Treatment Medication Assisted Treatment (MAT) programs are an organized, usually ambulatory, addiction treatment service for opiate-addicted individuals. For Medicaid individuals, the actual administration of the opioid maintenance medication is conducted under the medication administration service code, as stipulated by state law and regulation and the federal regulations at FDA 21 CFR Part 291. Treatment with methadone is designed to address the individual’s need to achieve changes in his or her level of functioning, including elimination of illicit opiate and other alcohol or drug use.

Adolescent Substance Abuse Clubhouse Programs

Clubhouse Recovery Support: DBHDD funds nine unique adolescent substance abuse “Clubhouse” sites. These programs provide a comprehensive substance abuse model designed to engage adolescents and their families in their own recovery. The clubhouse is a supportive environment where individuals are members. Staff and members work together to perform the jobs of the clubhouse and participate in clinical sessions, social outings, educational supports and specific clubhouse activities.

Adolescent Intensive Residential Treatment Programs

Adolescent intensive residential treatment services is a twenty-four hour supervised residential treatment program for adolescents age 13 to 17 years old, who are in need of a structured residence due to substance abuse. These programs are located in the geographic metro part of the state and the southern part in order to afford statewide access; treatment is the appropriate level of care as defined by the ASAM Level 3.5

Adolescent Core Services Core service providers must be prepared to respond within two hours when either an existing individual in treatment needs intervention, or when a referral for crisis services is made to the agency through the Georgia Crisis and Access Line, or other referral sources. CORE CUSTOMER CLASSIFICATION AND ELIGIBILITY DETERMINATION There are four variables for consideration to determine whether a youth qualifies as a “core customer” for child and adolescent mental health and addictive disease services. 1. Age: A youth must be under the age of 18 years old. Youth ages 18-21 years (children still in high school, in custody by the Department of Juvenile Justice or Division of Family and Children Services, or when it is otherwise developmentally or clinically indicated) may receive services to assist with transitioning to adult services. 2. Diagnostic Evaluation: The DBHDD system utilizes the Diagnostic and Statistical Manual of Mental Disorders (DSM) classification system to identify, evaluate and classify a youth’s type, severity, frequency, duration and recurrence of symptoms. 3. Functional/Risk Assessment: Information gathered to evaluate a child or adolescent’s ability to function and cope on a day-to-day basis comprises the functional and risk assessment. Such information includes child and family resource utilization and the child’s role performance, social and behavioral skills, cognitive skills, communication skills, personal strengths and adaptive skills, needs and risks as related to an emotional disturbance, substance use disorder or co-occurring disorder. The functional/risk assessment must yield information that supports a behavioral health diagnosis (or diagnostic impression) on Axis I in accordance with the DSM.

Women’s Treatment and Recovery Support (WTRS) Services

(Formerly Ready for Work-RFW) The Women’s Treatment and Recovery Support (WTRS) programs are designed to eliminate substance abuse as a barrier to employment for pregnant and parenting women. The target population is pregnant and parenting women with children under the age of 13, with substance use disorders. WTRS programs also provide services for women who are at high risk for relapse, are sufficiently medically stable to participate in intensive outpatient and residential treatment, and who may have one or more of the following risk factors:

• Highest priority is given to women who meet the needy families definition • Pregnant women will be given priority status • Involvement with the criminal justice system • History of relapse, or secondary medical or psychiatric disorder that can be safely managed in a

substance abuse treatment environment • Women with an open child protected service case

DUI Intervention Program

All DUI offenders are required to attend DUI School, have a clinical evaluation and complete any treatment recommended by the evaluator. DBHDD’s Office of DUI Intervention implements Georgia DUI laws pertinent to clinical evaluation and treatment. DBHDD writes rules and implements procedures to interpret and enforce laws passed by the Georgia General Assembly. Procedures include setting criteria for qualified clinical evaluators and treatment providers. More information is available at www.mop.uga.edu.
