Executive Summary
Four Days District Level Training of Trainers (DToT) on Multi Sector Nutrition
Plan(MSNP) aimed to bring all concerned stakeholders in a single platform and discuss about
effective implementation of MSNP.
The objectives of the programme were to increase knowledge and enhance facilitation
skills on multi sector nutrition plan (MSNP) to the representatives of the district level
government line agencies and stakeholders and equip them with evidence based planning skills
for preparation of annual work plan and budget for smooth implementation of multi sector
nutrition plan's activities in their sector that eventually contribute to reduce prevalence of
malnutrition.
The approach of the training was participatory and different advance teaching and
learning methods were used to facilitate training to achieve the desired goal. Based on evaluation
and feedback, the training was a success to show its output as desired.
Introduction
Four Days District Level Training of Trainers (DToT) on Multi Sector Nutrition Plan(MSNP) is
a well-coordinated set of prioritized activities designed to: conceptualize multi sector nutrition
plan, explore background with linking previous nutrition plan and policy, enhance basic
knowledge on nutrition, over nutrition , under nutrition ,sensitize on nutrition sensitive and
nutrition specific intervention and capacitate to architect nutrition at district level as desired to
mainstream into their integrated plan and development agenda. In addition, it capacitates
participants of respective organization to make plan, design activities and incorporate in their
regular program including provide supportive supervision and close monitoring to reduce
prevalence of malnutrition and prevent vulnerable groups from its consequences.
Objectives
To increase knowledge and enhance facilitation skills on multi sector nutrition plan (MSNP)
to the participated chiefs and focal persons of the district level government line agencies and
stakeholders,
To equip with evidence based planning skills to prepare annual work plan and budget for
smooth implementation of multi sector nutrition plan's activities in their sector that
eventually contribute to reduce prevalence of malnutrition,
To enable participants to organize and provide VDC level training on MNSP and educate them
to architect MSNP at cluster to district level,
To sensitize stakeholders to mainstream nutrition in their development agendas,
To enable to ensure participation of dalit, janjati and marginalized (DAG) in MSNP activities
To equip with counselling skills to participants on nutrition to educate subordinates and
beneficiaries on aforesaid subject matter through different appropriate counselling tools.

Following methods were mostly used in the training.

During DToT, participants training manual and facilitator guideline were provided to each
participants.
Participants Expectations
MSNP was perceived as an interesting subject matters for them so they were highly excited to

4. Know role and responsibility of agency for implementing MSNP.

Ground rules

Facilitation Modalities The facilitation of this training was quite practical. Facilitators, co-facilitators, guest speakers
and resource persons were heavily engaged to get whole training package ahead with
participatory approach. In the beginning, training learning environment was created with
inspiring and motivational quotes. Some intellectual and funny games and other recreational
activities were done back to back for active participation and made session more interesting.
The first introductory session was started via thread game while participants gave their short
introduction with name, post and organization of participants in turn and importance of the
game was ultimately linked with multi sector approach to implement MSNP at district level with
coordinated effort. This was expressed in a very interesting way as how the MSNP is interlinked
with each sectors and once one is impractical, others ultimately fail to reach the goal.
Topics of training were allocated to facilitators as per their expertise and interest to conduct
session smoothly with high level of participation.
Process of Training
The training was conducted semi – informally. It was divided into three parts namely Inaugural
Ceremony, Technical Session with Training core activities and Closing Ceremony. In the
beginning, pre-test and after completion post-test were done to fathom the effectiveness of
training contents, facilitation skills and methodologies used to conduct training. Overall training
conducted through adult learning approach.
1. Inaugural Ceremony
Mr. Yogendra Neupane gave a warm welcome to all the representatives of sectoral line agencies.
The key information on nutrition program, nutritional indicators with under 5 children’s
nutrition status was explored by Mr. Prabhakar Jha ,Act.DHO.
Importance of MSNP for reducing malnutrition was then highlighted by Mr. Yogendra Neupane,
Public Health Officer, Child Health Division in a participatory approach. This was followed by the
presentation of the Objectives of training session which was delivered by Act. DHO. With this,
Technical session was on the go!
2. Technical Session with Training core activities
DAY I
Preliminary session was taken ahead with some important motivational quotes to create a good
learning environment and inspire participants to draw their attention on subject matters. Ms
Pragya Rimal, MIYCF Officer of UNICEF, conducted a discussion session where she informed
about norms, rules and facilitated for nominating of officer, reporter and entertainer for 4 days.
Pre-test was then taken in the beginning to fathom the participant's level of knowledge on multi
sector nutrition plan (MSNP). Mr. Phulgendra Kumar Singh, Nutrition Officer, UNICEF revealed
the nutritional status of under five children and pregnant women with references of NDHS and
NMICS. He presented the four major indicator i.e. Stunting (Low Height for Age), Underweight
(Low Weight for Age), Wasting (Low Weight for Height) Anaemia (below level of haemoglobin
in the blood) and BMI. Additionally, presented the severity of malnutrition prevalence range and
threshold given by WHO. Based on these data, Nepal is still in the high public health concern
zone. Apart from that, calculation process of BMI was shown and illustrated. According to WHO
classification,
<18.5 is Low BMI
18.5-24.99 is Normal
25-29 is overweight and
30 or > 30 is Obesity.
According to NDHS 2011, he highlighted the nutritional status via showing data on IYCF
practices including breast feeding, exclusive breast feeding and complementary feeding and
poor feeding practice as well. He also demonstrated the situation of Central Terai with focus on
Sarlahi and exhibited that the data were similar to the nation's.
Informative session on nutrition, type of micronutrients, source of micronutrients, deficiencies,
its importance and functions to the body was detailed with concept on malnutrition, type of
malnutrition, causes of malnutrition, its immediate and long-term consequences on health
linking with morbidities and death among children. A clear concept on malnutrition due to
direct/immediate cause and indirect/underlying cause including basic causes was anatomized
and figured during causality analysis.
Glimpse of Multi Sector Nutrition Plan (MSNP)
Mr. Dhananjay Poudel , Nutrition Expert , Golden Thousand Days , MoFALD highlighted the
milestone of MSNP linking with the development of national nutrition strategy to current
situation. He explored the objectives of MSNP where he focussed on enhancement of human
capital especially among the poorer segments of the society, improvement in maternal and child
nutrition by accelerating the reduction in maternal and child under nutrition through
implementation of nutrition specific and nutrition sensitive interventions aimed at the “window
of opportunity of 1000 days from Pregnancy to 24 months” with following major outcomes.
Outcome 1: Policies, plans and multi-sectoral coordination improved at national and local
levels
1.1: Policies and plans updated/reviewed to incorporate nutrition specific indicators at national
and local government levels
1.2: Multi-sectoral coordination mechanisms functional at national and local government levels
Outcome 2: Practices that promote optimal nutrition behaviors improved
2.1: Maternal and child care service utilization pattern changed
2.2: Adolescent girls’ education , life skills and nutritional status improved
2.3: Reduced episodes of diarrheal diseases and ARI among mothers, adolescents and IYC
2.4: Feeding behaviors improved with increased availability and access to appropriate food (in
quality, quantity, frequency and safety)
Outcome 3: Strengthened capacity of central and local governments on nutrition to
provide basic services in an inclusive and equitable manner
3.1: Capacity of national and local government enhanced to provide appropriate support to
improve maternal and child nutrition
3.2: Multi-sectoral nutrition information updated and linked both at national and local
government level
Moreover, Mr. Poudel explored on life cycle development among different age group where he
illustrated that a child's mental development is accomplished 30 % during pregnancy, 25%
during 1st year of life, 25% during 2nd year of life, 10 % up to 5 years of age and rest 10 %
onward. Additionally, he detailed about different level's nutrition and food security
steering/coordination committee, their major roles and responsibilities to plan, design,
implement, supervise and monitor MSNP with special focus to DLNFSSC. Apart from that, he
described the approaches and criterion for selecting district i.e poor nutritional indicators, DAG,
net enrolment rate in school, food security status, numbers of 10-14 years aged children
involved in wage labour, access on safe water and sanitation etc.
In context of the way forward, Mr. Poudel emphasized on mapping of stakeholders and resources
at centre, district and VDC level and advocated for maintenance of functional coordination at all
levels with full cooperation and linked with scaling up districts phase wise. With the aim of
reducing challenges that may arise like poor coordination, scattered resource allocation and
weak implementation, DLNFSSC should play coordinating and leading role via afore-said
activates.
Mr. Yogendra Neupane, Public Health officer, Nutrition Section, Child Health Division revealed
that nutrition is a priority issue for reducing global burden of disease and death, improving
physical growth and cognitive development, increasing productivity and progressing toward
sustainable development goal. So addressing these issues, integrated effort via both nutrition
sensitive and specific interventions are needed to implement from respective line agencies and
partners. He highlighted the major national regular nutrition program, scaling up nutrition
program through existing health networks and leveraging with other agencies. Additionally, he
mentioned that sensitizing to VDC level health network to architect nutrition as desired in MSNP.
In a query of a participant, Mr. Phulgendra Kumar Singh, Nutrition Officer, UNICEF described
about Logical Framework Analysis where he mentioned that resources are Physical, financial,
human resources and information and result is measured through its 3rd to 5th step.

With regards of food, diet and balanced diet, he revealed information with reference to different
organization/expert. He also described in detail about food and nutrient classification based on
sources, function and body requirement and highlighted importance of diversified food for
critical window of opportunity (with in golden 1000 days of children and pregnant women).
IYCF not only prevent children from disease and morbidity but also improve nutrition as well of
children as of present evidence and trial. He also explained about dietary guideline and
managing balance diet for different age group and their daily requirement with special focus on
breast feeding and exclusive breast feeding. As he mentioned daily requirement of kilocalorie
for male viz.
2800 KCL for sedentary working people (for adult male)
3200KCL for middle level working people (for adult male)
3900KCL for heavy working people & (for adult male)
900 KCL for 6month to 12 month of age
Mr. Singh suggested that its better to consume 2-3 eggs per week, meat 2-3 times per week and
140 gm fish or 148-gram meat for an adult. Having adequate fiber is better to health to maintain
cholesterol level and reduce constipating. He highlighted to have plant source fat rather than
animal source.
DAY II
The session of the second day was started with review by Sushil Kumar Jha (HA DHO). During
review session, he highlighted the major topics and contents which were presented on
previous day via using a brief report that was given to him. Lastly, he handed the reporting
note over to the reporter of the 2nd day
Mr. Chudamani Aryal , Planning Officer , National Planning Commission, revealed the policy,
strategy and program of respective line agencies to prepare plan with embedding nutrition
activates in their annual plan and budget as architected in MSNP. He also illustrated on
planning steps, process and its importance with allocation of budget to respective line agencies
as per their role mentioned in MSNP. Regarding monitoring he described on process
monitoring, input monitoring, sustainability monitoring and precondition of monitoring and
emphasized that monitoring should be focused on relevancy, effectiveness, efficiency, impact
and sustainability linking with Rastriye Anugaamn Digdarshan 2070 and log frame. As he stated
for result based monitoring a standard check list has to be developed and standardised. He also
highlighted techniques and process of information management and its importance.
Mr. Dhananjay Poudel discussed on how to convey the multi sector nutrition plan to local level
and make them aware to architect nutrition at ground for making nutrition agenda of local
development and embedding in local governance. He also highlighted the basic information on
log frame with its component viz input, output, outcome and impact.

out from their office and network
DAY III
Session started with review of previous day‘s climax by Jodh Narayan Giri from DADO. Mr.
Dhananjay Podel , Nutrition Expert , MoFALD conferred on planning; defined as a bridge of
present and future , its importance for development to accomplish work in time in spite of
limited resources. He also remarked on it's types i.e. Periodic plan & annual plan, Long-term
plan and short term plan, based on ecology and geography, subject or area i.e. health, nutrition
and education, ownership i.e public, private and non governmental. He also showed the output
of planning with giving an example.
Local Level Planning Cycle
Mr. Poudel remarked that good planning for better result needed scientific steps which are
problem identification, prioritization, budget ceiling and guidance, analysis and projection of
resources and possible constraints. He also disclosed on concept of planning, central planning
process, decentralized planning process, 5 years planning process and process of 14 steps
planning with time, level and participants.
Result Based Management
Mr. Phulgendra Singh, Nutrition Officer, UNICEF revealed on RBM as an approach focused on
outputs and outcomes. From a results perspective, the implementation process is significant
only in terms of what it leads to – or what follows from the process of planning, managing and
implementing.
Impact is the overall and long-term effect of an intervention; it is the ultimate improvement in
people’s lives; e.g. higher standard of living, increased earnings from exports, etc.
Outcomes are the actual or intended changes in development conditions that development
interventions are seeking to support. They describe a change in development conditions
between the completion of outputs and the achievement of impact.
Outcomes are the likely or achieved short term and medium-term effects of an intervention’s
outputs. Results and effect are the related term of outcomes.
Outputs are tangible products and services that emerge from processing inputs through
programme or non-programme activities. Outputs, therefore, relate to the completion (rather
than the conduct) of activities and are the type of result over which managers have a high degree
of influence.
Outputs are the products, capital goods and services that result from a development
intervention; may also include changes resulting from the intervention which are relevant to the
achievement of outcomes.
Activities are the various steps taken to carry out a program/project e.g. to train agricultural
extension officers, fund a study, build a school, etc.
Inputs are means mobilized to carry out activities e.g. funds, staff time and other resources.
Apart from that, it was also shown in result chain as a pyramid.
The contents included in training package and topics allocated to respective facilitator was
responsible to conduct the session in participatory approach. (For detail see annex II)
Facilitation Skill, Advocacy and Communication
Concluded facilitation is a psycho-social method called facilitation/moderation; is invited to
manage the group work process in which someone manages the process, while others actively
participate in it by putting up their own ideas. People work collectively for solving a given
problem or completing a task. The process adopts the consensus building approach for decision-
making. During facilitation, its essential to deal with diversities, nature of participants and focus
on 3Ins i.e Introduction, Information or main body and Inference or conclusion. Some additional
tips were given to make presentation more informative and consisted via remembering KISS
rule i.e keep it short and simple. Apart from that, for scientific slide making the presenter
suggested to follow a good rule of thumb about one slide per minute, clear explanation of
technical term, avoid using complete sentence, audience cantered approach and ongoing
communication activities. Another 6X6 rules i.e six words per line, six lines per slide, keep
regular and use sort and strong statements.
Communication is the conversion between 2 or more persons. In the communication process
message is delivered through which attitude of the receiver is expected to change. For effective
communication inspirational quests to be used for motivating introvert participants and simple
question should be asked to motivate them for active participation. Participants information
should be gathered and extracted to highlight in appropriate topics or place and control them
with group norms and set boundary.
DAY IV
Group works were assigned for the day. With response to that, groups were formed and asked
for situation and causality analysis and most effective nutrition specific and sensitive
intervention. As assigned, participants worked in group according to their sectors and presented
causes for malnutrition and enlisted appropriate nutrition intervention.
The other task was to prepare annual program and budget in prescribed format. After
completion, chief of all line agencies presented his/her AWPB and incorporated feedback in their
plan.
Review of participants’ expectations
After completion of the technical session of the training, the participants were asked about the
expectations of the participants if were addressed or not. The most of the participants
'expectations were met during presentation session. Likewise, confusions were addressed
accordingly.

3. Closing Ceremony
The closing ceremony of the training was chaired by Local Development Officer (LDO). During
the closing sessions, he gave his remarks to express gratitude to all facilitator/representatives
of National Planning Commission, MoFALD , CHD , Chief and Focal person of district level line
agencies, staffs of UNICEF regional office, HERD team including all for their active and
meaningful participation. He repeatedly sensitized to keep MSNP in high priority of planning and
implementation to respective district line agencies. Lastly, certificates were awarded to all
participants and financial activities were done as well.
Evaluation
The evaluations were done by adopting following activities:
I. Review of the previous day
The leader of the day reviewed the previous day activities with SWOT analysis and highlighted
the strength aspects of training, aspects need to be improved. Evaluation cadre evaluated
activities of the all days and shared in the floor to make session more effective and participatory.
II. Question Answer Method
During different training session some questions were asked to participants to know the
effectiveness of training and participants were involved in many group work and with that, their
work performance was observed.
III. Per-test and Post-test
Above table shows that participants had given more correct answers during post-test
compared to that during their pertest.
IV. Feedback provided by the participants
Participants provided their feedback on the last day of the training. They shared their
experiences during the training days. They evaluated that the training was successful and was
able to deliver the required information on MSNP. They also remarked that the training was able
to enhance their capacity and augment skills for providing nutrition education and counselling
to the target groups.

outcome of the training was good and it expected for better impact on multi sector nutrition plan
implementations.
Recommendation
On the basis of training modalities, training contents, targeted group and facilitation process as
well as participant’s responses, some recommendations are made which are as follows:
The contents of MSNP were relevant though latest information and data could be included
to revitalize.
The time set for the training didn’t seem sufficient as the amount of contents to be
delivered were high.
It would be great if the participants were given with sector wise planning draft where
they could make necessary corrections according to their sectoral situations.
