HX• 19 years young male from Karnataka state.• known cardiac patient since the age 15 yrs
not taking his medications.
• Has dyspnea on mild to moderate exertion because of which not attending the school.
• Palpitation and occasional chest pain.
• Currently came with ↑ dyspnea ,hemoptysis and fever of 2 weeks.
P/E NO DYSMORPHIC FEATURES V/S BP=100/60mmHG PR= 1o4’ T= 102
SPO2=98%CREPITATION AT LEFT MIDDLE POSTERIOR LUNGALL PERIPHERAL ARTERIES PALPABLE,ACTIVE
PRECORDIUMPARASTENAL HEAVE,RV APEX ,P2 ACCENTUTEDG III SYSTOLIC MURMUR AT RIGHT 2ND INERECOSTAL,GIII
PSM AT LLSB.NO CLUBING ,NO CYANOSIS.
DDX???
TC slightly incresaed ABG=PH =7.45 PO2 =77.3 PCO2=31.1 SAT=96%
