Network Configuration

The Northern Trauma Network was established in April 2008 and became fully operational in April 2012 with two Major Trauma Centres supported by nine Trauma Units across the

North East and North Cumbria covering a population of 3.08 million1.

Rehabilitation Services Tertiary ‘specialised’ rehabilitation services (Level 1):

Regional Level 1 facilities are based at Walkergate Park Centre for Neurorehabilitation and Neuropsychiatry, Newcastle upon Tyne. Regional Level 1 facilities for Spinal Cord Injury are based at The Golden Jubilee Regional Spinal Cord injuries Centre at The James Cook University Hospital, Middlesbrough.High cost / low volume services, which provide for patients with highly complex rehabilitation needs that are beyond the scope of their local and district specialist services.

Local (district) specialist rehabilitation services (Level 2):

Level 2 facilities are currently available at Sunderland, South Tees and Carlisle. Typically planned over a district-level population of 250-500K, and are led or supported by a consultant trained and accredited in Rehabilitation Medicine (RM), working both in hospital and the community setting. The specialist multidisciplinary rehabilitation team provides advice and support for local general rehabilitation teams.

Local non-specialist rehabilitation teams (Level 3):

Each locality provide general multi-professional rehabilitation and therapy support for a range of conditions within the context of acute services (including stroke units), intermediate care or community services. The rehabilitation pathway is not complete until the patient has re-joined society having reached optimum functional potential e.g. employment and leisure. Level 3 services include community based rehabilitation

teams which are significantly under resourced. Therefore there needs to be a much stronger focus on AHP led rehabilitation and reablement when re-designing services.

Northern Trauma Network does not necessarily endorse or recommend any service listed and no liability for accuracy of information can be accepted. This Directory is not exhaustive, and is correct (based on returns from services) as of February 2015. Additional information will continue to be collected and added to future editions. Services are commissioned either by the NHS or private funding arrangements, and referrers should contact the service to clarify their individual funding mechanism.

The Trauma network office will contact the services every 6 months to prompt them to complete the template, however if it is not returned the service will not feature in the

An anticipated and ongoing deterioration in an individual’s long term condition. When other, appropriate services, are unavailable or not able to respond – The ISIS team is unable to respond to support gaps or pressures within other services. End of life care. People experiencing a current mental health crisis should be referred to the Mental Health crisis team. Short term needs where substance misuse is the primary cause of need. Service overview

Momentum provides a structured pathway of care, community integration and rehabilitation as well as vocational rehabilitation support to individuals throughout the North East of England. Support packages are tailored to individual requirements following structured psychological assessment of needs. Care Services; Individualised goal-directed rehabilitation increasing independence in social, domestic and community activities. Psychological intervention to manage cognitive, emotional and behavioural effects is also available.

Community Integration; Providing access to an array of social interest groups, 1:1 support including psychological and promotion of independence within the local community, sessions available to support with self-management of conditions, specifically around confidence building and reduction of anxiety and adjustment to injury and or diagnosis of condition.

Vocational Rehabiliation; A centre based rehabilitation service tailored to meet individual needs in order to support service users in improving or acquiring social skills, cognitive retraining and strategies both within the home and workplace as well as support with vocational strategies and behaviours appropriate for the work place. Extensive group and 1:1 psychological input is provided to support individuals with their adjustment to life following injury or diagnosis of condition. Work placements form a vital part of the programme with individuals working 1:1 with assigned job coaches to establish appropriate areas of vocational interest, skills and retained knowledge.

Service overview

The out-patient Physiotherapy service is primarily based within Whitby hospital and also provides Physiotherapy clinic sessions at Egton, Danby and Staithes GP practices. The service provides Physiotherapy assessment and treatment of adults with Musculo-skeletal pain or problems. These problems include: soft tissue injuries, joint pain, and rehabilitation after trauma or surgery. Patients receive an assessment and then an individually tailored treatment regime. The service is a separate service from the MSK Orthopaedic triage service which provides a diagnostic service referring for investigations, surgical opinion or directly for out-patient Physiotherapy. The service provides individual and group treatment sessions and includes a well-equipped gym facility.
