    VERIFY THE PATIENT NAME

    1) DO NOT address patient by their first name especially

    when meeting patients for the first time, because some

    patients take offense at being addressed by their first

    names, especially if they are much older than the

    pharmacist.

    2) Common courtesy dictates that patients be addressed byappropriate title (e.g., Sir, Maam ,Mr., Mrs., Ms., Rev.,Dr.). However, use the correct titles.

    3) Do not assume that all adult women are married or, ifmarried, wish to be addressed as Mrs. Conversely, donot assume that all adult women, married or single,

    want to be addressed as Ms.

    4) The best way to avoid confusion is to ask each patienthow he or she wants to be addressed.Example: Saying Hello. My name is Jane. Do you

    wish to be called Ms. or Mrs. Santos or would youprefer to be called Susan?
