    TAP)! You have just made a wise decision to protect yourselffrom the high cost of future repair bills or having to replaceyour product in the future.

    If you live in Florida, the Plan is an agreement between American Bankers Insurance Company ofFlorida (OBLIGOR) and you, the purchaser.

    If you live in California, the Plan is an agreement between Sureway, Inc. (OBLIGOR), 11222 QuailRoost Drive, Miami, Florida 33157, and you, the purchaser.

    If you live in Alaska, Arkansas, Colorado, Kansas, Maine, Maryland, Massachusetts, Missouri, NewJersey, New Mexico, North Dakota, or Pennsylvania, the Plan is an agreement between CompUSAStores L.P. (OBLIGOR)and you, the purchaser.

    If you live in the District of Columbia or any of the other state within the United States of Americanot listed above, the Plan is an agreement between Voyager Service Programs, Inc. (OBLIGOR),

    Throughout this document these terms shall have the following meanings:We, us, and our shall refer to the OBLIGOR (American Bankers Insurance Company ofFlorida (ABIC), Sureway Corporation, CompUSA Stores L.P., or Voyager Service Programs, Inc.).You and Your shall refer to you the purchaser.

    The Plan shall refer to this document along with Your sales receipt for the Covered Product.

    The obligations assumed by OBLIGOR under the terms and conditions of the Plan are guaranteedand fully insured by American Bankers Insurance Company of Florida (ABIC). Please keep this infor-mation easily accessible in a safe place along with the sales receipt you received when you pur-chased your product. It will serve as a valuable reference and will help you determine what is cov-ered by the Plan.

    Eligible Products, Coverage, Terms & Technology

    Assurance PlansThe Plan is among the most superior and dependable protection plans available in the industry. Itoffers coverage above and beyond that typically offered by the product manufacturers.

    New Product EligibilityThe Plan protects new products that are manufactured for use in the United States, which, at thetime of purchase, include a manufacturers original written warranty valid in the United States. In

    order for a new product to be eligible for a Plan, the manufacturers warranty must provide at leastninety (90) days of parts and labor coverage for computer systems, computer peripherals, and homeoffice equipment.

    Out of Warranty Product EligibilityUsed computer products may only be covered under the 1-Year Out of Warranty Plan for usedcomputer products.

    Parts Non-AvailabilityIn the event that replacement parts needed for repair should become unavailable during the cover-age period of the Plan, OBLIGOR shall be excused from performance hereunder and you shallreceive a refund of the purchase price paid by you for the Plan less claims paid, if any, whereallowed by law. (Refer to the Cancellation and State Specific Requirements section in this Plan forapplicability by state.) In no event shall the retailer and/or OBLIGOR be liable for any damages

    as a result of the unavailability of replacement parts.

    CoverageYour retailer offers a variety of Plans to meet the needs of its customers. The specific Plan, whichyou purchased, is incorporated within the sales receipt. OBLIGOR will cover labor and parts nec-essary to restore your product to normal operating condition if it becomes inoperable during theterm of your coverage and subject to the terms, conditions, limitations, and exclusions of the specif-ic plan you purchased as listed below.

    The Plan for your desktop computer (CPU) is incorporated within the sales receipt and covers thefollowing system components as included in the original configuration by the manufacturer: moth-erboard and all components resident on the motherboard, internal power supply, up to two floppydrives including Zip, Jaz and other similar drives, 1 hard disk drive, all internal cards, 1 CD-ROMdrive, 1 DVD drive, 1 CD-RW drive, 1 tape drive, keyboard, and mouse. A monitor and printer maybe included in your Plan if purchased at the same time as your desktop computer (CPU). Coverageexcludes peripherals, purchased at time of sale, that are not part of the original configuration by themanufacturer, such as joysticks, scanners, external/internal drives and other similar equipment.

    The Comprehensive Plan, the Depot Repair Plan, the Carry-In Repair Plan, the Trip and Labor Plan,and the Replacement Plan shall commence upon the date of product purchase. Benefits in yourPlan, which are not offered by the manufacturer in its warranty, will be provided from the date ofproduct purchase, during the period of the manufacturers warranty and beyond. Particular benefitsin the Plan, which are also offered by the manufacturer in its warranty, will be provided by the Planupon the expiration of the shortest portion of the manufacturers parts and labor warranty. The prod-uct manufacturer has primary responsibility for replacement of the covered product during the man-ufacturers warranty period for replacements covered by the manufacturer.

    If the Plan Administrator determines that Your product must be replaced, the Administrator willreplace Your covered product with a new or refurbished product of like kind and quality. The priceof the replacement product shall not exceed the retail purchase price of the original covered prod-uct. If Administrator elects to replace your covered product and a replacement product, as describedabove, is not available, Administrator will pay you a cash settlement. The amount of the cash settle-ment shall not exceed the current retail cost of a replacement product of like kind and quality; and,such retail cost shall not exceed the retail purchase price you paid for the original covered product.OBLIGOR will assist you in understanding your limited manufacturers warranty and Plan benefitsfrom the day you purchase the Plan. In no event shall the Administrators obligation to administer

    claims extend beyond the term of coverage commencing on the date of product purchase.

    Applicable To All Tap Plans:Fulfillment In the event your desktop computer (CPU), monitor, printer, computer peripheral, orother covered product under your Plan is replaced, all ofOBLIGORs obligations under your Planwill be completely fulfilled for the replaced product, and OBLIGOR shall have no further obliga-tions for that product for the remainder of the term of your Plan, if any. (This provision might notapply in your state. Refer to the Cancellation and State Specific Requirements section in this Planfor applicability by state.) The Plan will not cover any defects that are subject to a manufacturersplan of reimbursement (i.e., a manufacturer recall).

    Component Replacement Guarantee If any individual component within your covered desktopcomputer (CPU), monitor or printer must be repaired three (3) times during the term of your Plan,this Plan will replace your covered desktop computer (CPU), monitor or printer with one of equalor similar features, specifications, and functionality.

    Ontrack Software, Dial-A-Tech.com Support, Delivery & Installation, In-store Setup, Free LaborUpgrade, Priority Call Status and Online Training are benefits provided by CompUSA Stores L.P.and are not part of the Plan.

    Technology Assurance Plans

    Comprehensive Plan (Available in 2 Year and 3 Year Terms From Date of Product Purchase)A comprehensive on-site/depot plan which provides protection for a Desktop Computer System,Laptop, Monitors (including a Computer System with built-in Monitor), and Printers. It is designed for

    products that carry a manufacturers warranty of at least 90 days parts and labor and at most 3 years

    parts and labor.The Comprehensive Plan will furnish labor and replacement parts required to keep your coveredproduct in normal operating condition as specified in the terms of this Plan. If your productrequires service during the term of the Comprehensive Plan, because of a product failure duringnormal usage, OBLIGOR has the option to repair or replace the defective product with a likeproduct of equal or similar features, specifications, and functionality. If the manufacturers warrantyon your product requires you to carry in or ship your product to an authorized repair depot, the

    Plan will provide you with overnight pick-up and delivery of the product to the authorized warran-

    ty servicer. Replacement parts may be new or reconditioned of like kind and quality. Damage ordefects to your covered product caused by a power surge will be covered up to $1,000.00 over thelife of the Comprehensive Plan.

    During the manufacturers warranty period on the product, the Comprehensive Plan will provide thesame level or location of service as the product manufacturers warranty. Please review the manu-facturers warranty in order to determine the type of service available for your product. Under theComprehensive Plan, you have the choice of the on-site service, carry-in service or depot servicedescribed below:

    On-Site Service During the term of the manufacturers warranty, service calls may not be scheduleduntil authorized by the manufacturer. An adult (of legal age) must be present at the location where on-site service will be performed. If you live beyond a fifty (50) mile radius of an authorized service cen-ter, you may be required to ship the covered product to the designated servicer; however, shippingcharges will be covered.

    While most products can be repaired on-site, some products, due to their sensitive and technicalnature, require the controlled environment of a factory authorized service center; therefore, On-Siterepair is not always possible.On-site service is only available for Desktop Computers (CPUs).

    Depot Service We will pay the shipping charges both ways, from you to the service center and backfrom the service center to you. Also, if you choose, you may carry in the product to one of theAdministrators authorized service facilities.

    Carry-In Service Simply call the Administrator to obtain an authorization number and locate a con-venient authorized service facility. Then, take your product to the facility to be repaired.

    Depot Repair Plan(Available in 2 Year and 3 year Terms From Date of Product Purchase)

    A comprehensive depot only plan providing protection designed for products that carry a manufac-turers warranty of at least 90 days on Parts and Labor.

    The Depot Repair Plan will pay for labor and replacement parts required to keep your covered

    product in normal operating condition as specified in the terms of this Plan. If your coveredproduct requires service because of a product failure during normal usage, OBLIGOR has theoption to repair or replace the defective product with a like product of equal or similar features,specifications, and functionality. Replacement parts may be new or reconditioned of like kindand quality. Damage or defects to your covered product caused by a power surge will be cov-ered up to $1000.00 for the life of the Plan. The Plan provides for shipping charges both ways,from you to the service center and back from the service center to you. Also, if you choose, youmay carry in the product to one of the Administrators authorized service facilities. On-Site cov-erage is not provided under this Plan.

    Carry-In Repair Plan(Available in 2 Year and 3 Year Terms From Date of Product Purchase)

    A comprehensive carry-in only plan which provides protection for printers (under $500 retail pur-chase price), analog/digital cameras, camcorders, and flat screen monitors. It is designed for prod-ucts that carry a manufacturers warranty of at least 90 days on Parts and Labor.The Carry-In Repair Plan will pay for labor and replacement parts required to keep your coveredproduct in normal operating condition as specified in the terms of this Plan. If your covered productrequires service because of a product failure during normal usage, OBLIGOR has the option torepair or replace the defective product with a like product of equal or similar features, specifications,and functionality. Replacement parts may be new or reconditioned of like kind and quality. Damageor defects to your covered product caused by a power surge will be covered up to $500.00 for thelife of the Carry-In Repair Plan. Your product must be carried in to an authorized service center asassigned by the Administrator to obtain service under this Plan. On-site coverage and shippingcharges will not be provided.

    Trip and Labor Plan(Available in 2 Year and 3 Year Terms From Date of Product Purchase)

    A comprehensive on-site/depot plan which provides protection for a Desktop Computer System,Laptop, and Monitor (including a Computer System with built-in Monitor). It is designed to provideuniform terms of coverage for products that carry a manufacturer warranty of at least 2 years or 3years on Parts but usually a lesser term coverage on Labor or On-Site coverage.

    The Trip and Labor Plan will provide labor coverage should your product require a repair during theterm of the Plan. The Plan requires a minimum manufacturers warranty of 2 years or 3 years parts/1

    year labor and will only apply toward monitors if the manufacturers warranty on the monitorincludes 3 years parts/1 year labor. The Trip and Labor Plan does not apply to Printers. If the manu-facturers warranty on your product requires you to carry in or ship your product to an authorizedrepair depot, the Trip and Labor Plan will provide you with overnight pick-up and delivery of theproduct to the authorized warranty servicer. Under this Plan, the payment for repair parts is alwaysthe sole responsibility of the manufacturer and/or you. The Trip and Labor Plan will not pay forlabor costs during the period that the manufacturer covers such charges under its warranty.

    Laptop Screen Upgrade(Available in 2 Year and 3 Year Terms From Date of Product Purchase)The Laptop Screen Upgrade MUST be purchased in conjunction with either a Comprehensive Plan,Carry-In Repair Plan, Depot Repair Plan or a Trip and Labor. The Laptop Screen Upgrade will payfor labor and replacement parts necessary to replace your laptop computer screen should itbecome inoperable. This Laptop Screen Upgrade covers your laptop screen only if purchased inconjunction with a corresponding Comprehensive Plan, Depot Repair Plan or Trip and LaborUpgrade. The Laptop Screen Upgrade will provide overnight pickup and delivery, or if you like,you can carry in the Laptop to a service center assigned by the Administrator which is authorizedto have the laptop computer screen replaced. The Laptop Screen Upgrade does not include on-site service and will not cover more than two (2) screen replacements over the life of the term ofyour Plan of purchase.

    Replacement Plans:(Available in 1 Year and 2 Year Terms From Date of Product Purchase)

    Carry-In Replacement PlanA replacement carry-in only plan designed for products that carry amanufacturers warranty of at least 90 days on Parts and Labor.

    PDA Replacement PlanA replacement carry-in only plan designed for Personal Digital Assistants (PDAs)that carry a manufacturers warranty of at least 90 days on Parts and Labor.

    In the event your covered product fails to operate and cannot be repaired (including mechanical or elec-trical defect in your product screen), the Replacement Plan will provide a one (1) time replacement of

    your original product with a product of equal or similar features, specifications, and functionality. Yourproduct must be carried in to any CompUSA store location or any other authorized service center asassigned by the Administrator to obtain replacement under this Plan. You must provide the store loca-tion or service center with proof of purchase documentation at the time of replacement request.On-site coverage will not be provided. If your product is still covered by the manufacturers warranty, youmay be directed to call the manufacturer directly. To obtain a refund on your accessory kit within thefirst 30 days, you may return your product to the store for a full refund. Your product is not refundableafter 30 days. For reimbursement of your Accessory Kit Replacement Plan, refer to the section entitledCancellations & State Specific Requirements.

    Out of Warranty Plan(Available only in 1 Year Term From Date of Plan Purchase)You are eligible to purchase the Plan if your used product is fully functional at the time the Plan is pur-chased. During the first thirty (30) days of the Plan, OBLIGOR may require a diagnostic inspection atYOUR expense to determine whether the product(s) is in proper operating condition. OBLIGORreserves the right to select a servicer to perform a diagnostic product inspection. If upon inspection, theproduct(s) is found not to be in proper operating condition, it must be returned to proper operating con-dition at YOUR expense to be considered eligible for coverage under the Plan. The used computer prod-ucts may have little or no original manufacturers warranty coverage remaining. The Plans term of cov-erage will extend a maximum of one (1) year from the date of purchase of the Plan; however, under nocircumstances will it cover any defects or claims occurring in the first thirty (30) days after purchase ofthe Plan. During the term of coverage, the Plan will pay for labor and replacement parts required to keepyour covered product in normal operating condition as specified in the terms of the Plan. If service isneeded because of product failure during normal usage, OBLIGOR has the option to repair or replacethe defective product with a like product of equal or similar features, specifications, and functionality. Areplacement part may be new or reconditioned of like kind and quality. Damage or defects to yourcovered product caused by a power surge will be covered up to $1,000.00 over the life of the Plan.

    International Plan(Available in 2 Year and 3 Year Terms)The International Plan was developed to replace the original manufacturers warranty for those cus-tomers who will take their purchases outside the United States or its territories and cannot return the

    product(s) to the United States for repair.When this International Plan is purchased and a customer needs repair outside of the United States or its

    territories, OBLIGOR will cover the repair claims which would normally be covered by the manufac-turer during the manufacturers warranty period. This Plan can NOT be sold as an alternative to followingthe manufacturers warranty in the United States.

    The International Plan is handled in the following way: Call 011-972-312-3581 to obtain a proper repair authorization number prior to work being

    done. Carry product in to an authorized service center for an estimate. The repair estimate should be

    faxed to the National Dispatch Center at 011-972-528-7691. Upon receipt of the estimate,CompUSA will make arrangements to pay the service center directly.

    For customer reimbursement, submit payment to the Servicer Center. Submit a detailed service repair invoice that identifies the product and the CompUSA repair authorization numberand includes a thorough description of the repair made. This documentation must be sent toCompUSA, 18451 North Dallas Parkway, Dallas, Texas 75287, USA, ATTN: National DispatchBilling. CompUSA will reimburse the customer within 30 (thirty) days of receipt of all necessary paperwork, provided that a covered repair was performed.

    Extended Replacement Plan(Available only in 1 Year [or 2 Year] Term From Date of Expiration of Manufacturers Warranty)The coverage for Extension Plans shall commence immediately upon the expiration of the short-est portion of the manufacturers original written warranty. The product manufacturer has primaryresponsibility for replacement of the covered product during the manufacturers warranty period.All Extension Replacement Plans shall terminate completely at the end of the specified term.

    Renewal Plan(Available only in 1 Year Term From Date of Expiration of Current Plan)Renewal of the Plan is at the option of ABIC. To inquire about renewing your coverage, please callthe Administrator at (877) 520-TECH or (877) 520-8324 on or before the expiration date of the Plan.Renewal prices will reflect the age of the product, current service costs, and product repair experience.

    Easy Access to Technical AssistanceIf you have purchased the Technology Assurance Plan for your product and you need assistance,

    call the Administrators toll-free number for technical assistance and service, (877) 520-TECH or(877) 520-8324 and the Administrator will connect you with a technical service representative. Duringor after the manufacturers warranty, the technical service representatives will diagnose most hardwareor pre-loaded software challenges over the telephone and/or assist you in obtaining repair service foryour covered product.

    Original Manufacturers WarrantiesCertain original manufacturers may require their own customer service representative to perform anover-the-telephone technical diagnosis and/or arrange for service at a manufacturers authorizedfacility during the original manufacturers warranty period. Therefore, additional time may be neededto assist you in obtaining repair service during the original manufacturers warranty period.

    As with any service function to be performed, it is the availability and procurement of replacementparts that will dictate the total time required to complete a covered repair.

    In no event shall OBLIGOR be held liable for consequential damages including but not limited to anydelays in rendering service, the availability of repair or replacement parts, loss of data, or loss of use dur-ing the period that the product is at a repair center or otherwise awaiting parts either under the originalmanufacturers warranty or during the term of the Plan.

    If Your Product Needs Repair ServiceIf your product needs repair for mechanical or electrical defects, you are required to call(877) 520-TECH or (877) 520-8324 to obtain authorization prior to having repairs made. Forfaster service, please have your dated proof of product purchase (sales receipt) and this document

    available when you place the call. It is Your sole responsibility to provide proof of purchase. Ifyour equipment is still covered by the manufacturers warranty, you may be directed to call themanufacturer prior to being referred to a service center.

    Do not return the product to the retailer unless so instructed by one of theAdministrators technical service representatives.

    Once You Receive Authorization When you receive authorization for repairs, your technical

    service representative will either instruct a local servicer to contact you, or provide you with

    the name and telephone number of the service center for you to arrange service. Should your product experience a mechanical or electrical defect in an area where authorized serv

    ice is not available, your repair facility can obtain approval for repairs from the Administrator by calling (877) 520-TECH or (877) 520-8324. All repairs MUST be approved by the Administrator beforethey are performed by a service facility.

    Exclusions From CoverageThe following is applicable to all Plans: OBLIGOR is not obligated to repair or replace cosmetic orstructural items; damage, warping or rusting of any kind to the housing, case or frame of the prod-uct or any non-operating part, including plastic, or decorative parts; or parts normally designated tobe replaced periodically by you or consumed during the life of the product (i.e., batteries, ribbons,

    toner cartridges, drums, etc.). In no event shall OBLIGOR be liable for consequential damagesincluding but not limited to any delay in rendering service under the Plan, for loss of data, or forloss of use during the period that the product is at a repair center or otherwise awaiting parts. Youare responsible for backing up all computer software prior to commencement of any repairs.OBLIGOR is not responsible for restoring software to your product. If your product experiences adefect or damage that is excluded from coverage under this section or in the event that no covereddefect or damage is found, then you are responsible for all repair costs, parts, and the cost of on-site

    service, if any.

    The following is applicable to all Plans EXCEPT PDA Replacement Plans and Laptop ScreenUpgrade: The Plan applies only to the operation or use of the covered product under conditions forwhich it was designed, and does not cover loss or damage resulting from external causes such asdropping the product, collision with an object, burglary, theft, vandalism, environmental conditions,fire, flooding, corrosion, sand, dirt, windstorm, hail, earthquake, or

    damage from exposure to weather conditions, misuse, abuse, neglect or accidental damage or dam-age resulting from improper use of any electrical power source. Damage incurred during transporta-tion is not covered. In order for the Plan to be valid, you, the purchaser, must have or shall haveperformed by a qualified party any and all preventive maintenance recommended by the manufac-turer to keep the covered product in normal operating condition. Loss or damage resulting from thefailure to provide manufacturers recommended maintenance is specifically not covered by the Plan.The following items, situations and circumstances are not covered by the Plan: the costs of all routine

    maintenance to your covered product; separately purchased cables, connectors, or other acces-

    sories; unauthorized repairs or modifications; improper installation of components or peripherals;losses on any component that has never been covered by an original manufacturers warranty; prod-uct repairs which should be covered by a manufacturers warranty regardless of whether the manu-facturer honors such warranty; damage due to normal wear and tear; burned-in phosphor inCathode Ray Tubes or any other type of display; any damage caused by a computer virus; any dam-age to recording media including any software programs, data, or configuration/setup informationresident on any mass storage devices such as hard drives, CD-ROM drives, DVD drives, floppydiskettes, tape drives or tape backup systems, as a result of the malfunctioning or damage of anoperating part, or as a result of any repairs or replacement under the Plan. Products with removedor altered serial numbers are not covered.

    Year 2000 Not CoveredThe Plan does not provide coverage for any breakdown, loss, claim, or service obligation whicharises from or is in any manner related to the failure of any computer, computer chip, computerizedcomponent, computer software, or other machine or product to recognize data, perform operationsor calculations, operate properly or perform as expected because of problems recognizing, process-ing, calculating or manipulating any date related data either before, during or after January 1, 2000(The "Year 2000" Problem), including but not limited to any breakdown, loss, claim, or service obli-gation involving miscalculations, interference with or damage to existing data, loss of data, inabilityto operate properly or any consequential or ensuing loss from The Year 2000 Problem, regardless ofwhether such failure occurs in equipment owned or used by you, or by others.

    Technical Assistance for Pre-Loaded SoftwareThe Administrator will provide technical assistance to you, over the telephone, to determine what difficul-

    ties, if any, may exist with application software that was pre-loaded on your new desktop computer(CPU) by the original manufacturer.The Administrator will offer basic troubleshooting and the steps necessary to help you over the telephonewith reloading software, loading simple drivers in the Windows environment, helping to navigatebetween applications, and verify that you have made every reasonable attempt to follow the basic set-upinstructions outlined in your owners manual. To the extent that the diagnosis confirms a hardware failure,the Administrator will assist you in arranging repair service of your covered product. The level of service

    expertise does not support customized or proprietary software, those applications that were installed after

    your original computer product purchase, or those software errors that confirm improperly functioning ordefective software. These service requests must be directed to the software manufacturers and are notcovered by the Plan. Additionally, the Plan does not provide customer education services (i.e., on how toutilize or perform tasks using any type of software program, pre-loaded or otherwise). It is your responsi-bility to backup all software, data, and set-up/configuration information prior to commencement of anycovered repairs. The Plan does not cover loss or corruption of data, damage due to computer viruses,and/or the restoration of software and operating systems to your covered product.

    Disclaimer of Certain LiabilitiesUnder no circumstances shall the retailer, the service facility, OBLIGOR, or ABIC be liable for indi-

    rect, consequential, or incidental damages (including damages for lost profits, business interruption, lossof data, and the like), even if any party has been advised of the possibility of such damages. The Planwill not cover any defects that are subject to a manufacturers warranty or program of reimbursement.The Plan is not a warranty or insurance policy; it is a Service Contract and is not intended to create orlimit any implied warranties concerning your product which may or may not exist under applicablestate or federal law.

    If you cancel within thirty (30) days of purchase of the Plan you will receive a refund in the amountof one hundred percent (100%) of the Plan purchase price. If you cancel within thirty (30) days ofpurchase of the Plan and a repair has been made, you will receive a refund in the amount of onehundred percent (100%) of the purchase price of the Plan, less the cost of any repairs made. ThePlan is not cancelable after thirty (30) days from the date of purchase, except as otherwise required

    by law. For those states that do not permit non-cancellation of your Plan, the law of that state shallapply to residents requesting cancellation.

    The following State Specific Requirements apply if your Plan was purchased in one of the followingstates and supersede any other provision herein to the contrary:

    Alabama only: You may return this Plan within twenty (20) days of the date this Plan was provided

    to you, or within ten (10) days if the Plan was delivered to you at the time of sale. If you made noclaim, the Plan is void and the full purchase price will be refunded to you. A ten percent (10%)penalty per month will be added to a refund that is not made within thirty (30) days of your returnof the Plan. These provisions apply only to the original purchaser of the Plan. In the event we can-cel the Plan, we will mail a written notice to you at your last known address at least five (5) daysprior to cancellation which shall state the effective date of cancellation and the reason for cancellation.However, prior notice is not required if the reason for cancellation is nonpayment of the provider

    fee or a material misrepresentation by you relating to the covered property or its use.

    Arizona only: Our replacement of your covered product in its entirety shall not eliminateOBLIGORs obligation for future repairs or replacement as otherwise provided under your Plan.You may cancel this Plan at any time and receive a pro-rata refund, less administrative expenses asso-ciated with the cancellation. Administrators cancellation fee may not exceed twenty-five dollars($25). No claim incurred or paid shall be deducted from the amount of the refund.

    California only: If you purchase your contract in California and then you return the product or theproduct is sold, lost, stolen or destroyed, you may cancel the contract (Plan) according to the fol-lowing terms: If you inform the Administrator of your request for cancellation in writing withinthirty (30) days of receipt of the Plan, upon such notice, you will receive a refund in the amountequal to one hundred percent (100%) of the Plan purchase price. If a repair has been made, uponsuch notice of cancellation, you will receive a refund in the amount equal to one hundred percent

    (100%) of the Plan purchase price, less the cost of repairs made. If you inform the Administrator of

    Connecticut only: The term of your Plan is automatically extended by the length of time in which thecovered product is in our custody for repair under the Plan. You may cancel this Plan if you returnthe product or if the product is sold, lost, stolen, or destroyed. In the event of a dispute with theAdministrator, you may contact the State of Connecticut, Insurance Department, P.O. Box 816,Hartford, CT 06142-0816, ATTN: Consumer Affairs. The written complaint must contain a descriptionof the dispute, the purchase or lease price of the product, the cost of repair of the product, and a

    copy of the warranty contract.

    Florida only: In the event you cancel this Plan, you shall receive a refund equal to ninety percent(90%) of the unearned pro-rata purchase price, less any claims paid or the cost of any repairs made. Inthe event the Administrator cancels this Plan; you shall receive a refund equal to one hundred percent(100%) of the unearned pro-rata unearned purchase price.

    Georgia only: If you purchase your contract in Georgia, you are entitled to cancel this contract atany time. Cancellation will comply with Section 33-24-44 of the Georgia Code. If you wish tocancel, you must notify the Administrator in writing or surrender the Service Contract toAdministrator, whereupon Administrator will refund the unearned pro-rata purchase price. TheAdministrator is also entitled to cancel this contract at any time based upon fraud, misrepresenta-tion, or failure to pay for the Service Contract.

    Hawaii only: You may return this Plan within thirty (30) days of the date this Plan was provided toyou, or within twenty (20) days if the Plan was delivered to you at the time of sale. If you made noclaim, the Plan is void and the full purchase price will be refunded to you. The Administrator will paya penalty of ten (10) percent on a refund that is not paid or credited within forty-five (45) days afterreturn of the Plan to the Administrator. These provisions apply only to the original purchaser of thePlan. In the event Administrator cancels the Plan, Administrator will mail a written notice to you atyour last known address at least five (5) days prior to cancellation which shall state the effective date

    of cancellation and the reason for cancellation. However, prior notice is not required if the reason for

Obligations of the Provider under this Plan are insured by: American Bankers InsuranceCompany of Florida, 11222 Quail Roost Drive, Miami, FL 33157.

    Illinois only: You may cancel this Plan at any time. If you cancel this Plan within the first thirty (30)days of purchase and if no service has been provided to you, you shall receive a full refund of thepurchase price less a cancellation fee equal to the lesser of ten percent (10%) of the purchase priceor fifty dollars ($50.00). If you cancel this Plan at any other time or if you cancel after service hasbeen provided to you, you shall receive a refund equal to the pro-rata purchase price less the valueof any service received and less a cancellation fee equal to the lesser of ten percent (10%) of the pur-chase price or fifty dollars ($50.00).

    Kentucky only: This Plan is an agreement between Voyager Service Programs, Inc. (OBLIGOR),110 W. Seventh Street, Fort Worth, TX 76102 and you, the purchaser. You consent to acknowledgethat the obligations ofOBLIGOR under this Plan may be assigned to an entity ofOBLIGORschoice provided written notice is sent to you prior to the assignments effective date.

    Nevada only: If you have not received any service under this Plan, you may return this Plan within

    twenty (20) days of the date this Plan was provided to you, or within ten (10) days if the Plan wasdelivered to you at the time of sale. In the event you return the Plan within the time periods specified,the Plan is void and the full purchase price will be refunded to you. A ten percent (10%) penalty permonth will be added to a refund that is not made within forty-five (45) days of your return of the Plan.These provisions apply only to the original purchaser of the Plan. You may cancel this Plan at any timeand receive a refund equal to the pro-rata purchase price. The Administrator may not cancel this Planonce it has been in effect for seventy (70) days, except for the following conditions: failure to pay

    the Plan purchase price; the conviction of you of a crime which results in an increase in the servicerequired under the Plan; fraud or material misrepresentation by you in purchasing the Plan or obtain-ing service; the discovery of an act or omission, or a violation of any condition of the Plan by youwhich substantially and materially increases the service required under the Plan; or a material changein the nature or extent of the service required under the Plan which occurs after the purchase of thePlan and substantially and materially increases the service required beyond that contemplated at thetime of purchase. If the Administrator cancels, you will receive a refund equal to the unearned pro-rata

    purchase price. In the event we cancel the Plan, we will mail a written notice to you at your last known

    address at least fifteen (15) days prior to cancellation which shall state the effective date of cancellation.In the event your covered product or any component of your covered product under your Plan isreplaced, OBLIGOR reserves the right to transfer the remaining coverage to the replaced prod-uct or component of your product or refund the remaining unearned portion of the purchaseprice. Refunds will be in accordance to the refund method stated above for Nevada.

    New York only: You may return this Plan within twenty (20) days of the date this Plan was providedto you, or within ten (10) days if the Plan was delivered to you at the time of sale. If you made noclaim, the Plan is void and the full refund price will be refunded to you. A ten percent (10%) penal-ty per month will be added to a refund that is not made within thirty (30) days of your return of thePlan. These provisions apply only to the original purchaser of the Plan. In the event we cancel thisPlan, we will mail a written notice to you at your last known address at least fifteen (15) days prior

    to cancellation with the reason for cancellation. But we are not required to mail you written noticeif the reason for cancellation is nonpayment of the provider fee, a material misrepresentation, or asubstantial breach of duties by you relating to the covered property or its use.North Carolina only: The purchase of a Plan is not required in order to obtain financing for the product.The Administrator may not cancel this Plan except for nonpayment by you. If you cancel your Plan, youwill receive a pro-rata refund less the cost of any claims paid and less a cancellation fee of ten percent(10%) of the amount of the refund.

    Oklahoma only: The Plan is not issued by the manufacturer or whole-sale company marketing the product. This Plan will not be honoredby such manufacturer or wholesale company. In the event you cancelthis Plan, you shall receive a refund equal to ninety percent (90%) of

    the unearned pro-rata purchase price, less any claims paid or the costof any repairs made. In the event the Administrator cancels this Plan;you shall receive a refund equal to one hundred percent (100%) ofthe unearned pro-rata unearned purchase price.

    South Carolina only: This Agreement does not provide coverage for pre-existing conditions. You

    may return this Agreement within twenty (20) days of the date this Agreement was provided to you,or within ten (10) days if the Agreement was delivered to you at the time of sale. If you made no

In the event the Administrator cancels this Agreement, Administrator willmail a written notice to you at your last known address at least fifteen (15) days prior to cancellation

    with the reason for cancellation. Administrator is not required to mail you written notice if the reasonfor cancellation is nonpayment of the provider fee, a material misrepresentation, or a substantialbreach of duties by you relating to the covered property or its use. If a claim for service has not beencompleted within sixty (60) days after proof of loss has been filed with the Administrator, the claimcan be submitted to American Bankers Insurance Company of Florida, who insures theAdministrators obligations under this Agreement, at the following address: 11222 Quail Roost Drive,Miami, FL 33157. In the event you have a question or complaint, you may contact the South

    Carolina Department of Insurance, P. O. Box 100105, Columbia, South Carolina, 29202-3105,Telephone (803) 737-6134.

    Texas only: You may return this Plan within twenty (20) days of the date this Plan was provided to you,or within ten (10) days if the Plan was delivered to you at the time of sale. If you made no claim, thePlan is void and the full purchase price will be refunded to you. A ten percent (10%) penalty per monthwill be added to a refund that is not made within forty-five (45) days of your return of the Plan. These

    provisions apply only to the original purchaser of the Plan. In the event we cancel the Plan, we willmail a written notice to you at your last known address at least five (5) days prior to cancellation whichshall state the effective date of cancellation and the reason for cancellation.     Utah only: If you purchase your contract in Utah, coverage for the replacement or repair of the product islimited to $3,000.00 per occurrence. Coverage afforded under the Plan is not guaranteed by the Propertyand Casualty Guaranty Association.

    Vermont only: You may return this Plan within twenty (20) days of receipt and, if no claim for serv-ice has been made, receive a full refund of the purchase price.

In the eventyou need to file a claim directly with the insurance company, you may submit your claim toAmerican Bankers Insurance Company of Florida at 11222 Quail Roost Drive, Miami, Florida 33157.Under provision titled, "Important Information", the following statement is deleted in its entirety: You

    consent to acknowledge that the obligations ofOBLIGORunder this Plan may be assigned to anentity ofOBLIGORs choice. However, the benefits under this Plan shall continue to be insuredthrough ABIC of Florida.

    Wisconsin only: THIS WARRANTY IS ONLY SUBJECT TO LIMITED REGULATION BY THE OFFICE OFTHE COMMISSIONER OF INSURANCE. This service contract may be canceled by the purchaser withinfifteen (15) days of the date of purchase. Administrator shall return one hundred percent (100%) of the

    purchase price, less an administrative fee of ten percent (10%) of the Service Plan price up to twenty-fivedollars ($25.00). Unauthorized repairs may not be covered.
