Letter from CEO & President, Jim Matney,

We are extremely pleased to announce that 2013 was a banner year for Colquitt Regional Medical Center. Its success is definitely a product of the hard work and dedication by our employees, physicians, and many volunteers. I want to personally thank everyone for job well done. In addition, I want to give my much appreciation to the patrons of our hospital and healthcare system. Without your support and trust in us, we would not be able to withstand today’s healthcare challenges and provide our community the medical services needed and deserved.

In 2013, we opened a 13,000 square foot state-of-the-art rehabilitation center that was well received by patients and athletes. We launched a $30 million dollar expansion and renovation project to better serve our community and to increase access to healthcare with a new emergency department, surgical services and intensive care unit. All the while receiving honors from The Joint Commission as a Top Performer on Key Quality Measures and Georgia Hospital Association’s Honor Roll for Core Measures performances. This year has been an encouraging and exciting one. We continue to focus and maintain top scores on quality measures and patient satisfaction. Our priority has been improving healthcare in our community.

Letter from Board of Trustees Chairman, Kirk Friedlander

Good things are worth waiting for: the perfect pitch, your first car, the completion of Colquitt Regional Medical Center’s expansion and renovation project. If you have driven by our hospital campus lately, you have seen a really good thing. Construction is moving at a rapid pace and every week shows amazing progress!

Our hospital has enjoyed another prosperous year due in large part to our hard-working employees and administration. We continue to recruit highly-skilled physicians while we also begin to invest in state-of-the-art medical equipment in preparation for the completion of the ER and OR facilities.

Colquitt Regional’s Board of Trustees is pleased and excited about the success of our patient satisfaction reports and quality of care scores – both of these reports continue to be the most subjective report cards for staff performance, level of service provided, our facility and how we perform compared to our competitors; a sincere thank you to each Colquitt Regional employee for your continued commitment to our hospital. The Board of Trustees would like to also thank our patients and their families for choosing Colquitt Regional Medical Center. With so many healthcare options available in our area, we are grateful that you chose us to care for you.

Since our last annual report, we have opened the Vereen Rehabilitation Center. This has been a tremendous success offering physical therapy services, occupational therapy, aquatic therapy, speech pathology with The Learning Center, a wound care clinic and the Black and Blue weekend clinic for youth athletes. This clinic has attracted patients from many surrounding counties in addition to Moultrie and Colquitt County.

We also said a fond farewell to two outstanding Colquitt Regional legends, Mrs. Madis Spires and Dr. D.W. Adcock upon their retirements. Their commitment to Colquitt Regional Medical Center sets an amazing example for many to follow.

Just as the photographer waits for just the right moment or the race car driver waits for the exact moment to pass his competitor, our hospital’s expansion and renovation will be worth the wait – a new emergency department, additional operating rooms, a new and expanded intensive care unit, an improved outpatient facilities and an impressive patient entrance are all worth the wait.

I hope you will join us late this year to celebrate and tour the new Colquitt Regional Medical Center. Thank you to Moultrie and Colquitt County for realizing the importance and need to expand hospital services in our area.

All of the Hospital Authority members at Colquitt Regional Medical Center are “Trustee Certified”. Georgia Hospital Association has designed a Trustee Certification program to enable hospitals to utilize governance best practices, to promote the coordination of care and the best use of resources. Each year, hospital leaders must complete this education program. In 2013, Colquitt Regional’s hospital authority members, physician leaders, CEO and other members of the leadership team where recognized for earning a Trustee Certification.

Providing you with a continuity of care, from admission to discharge. That’s what you will find with our Hospitalists here at Colquitt Regional Medical Center.

Hospitalists are physicians who specialize in the care of hospitalized patients. They are here 24 hours a day and are experienced in diagnosing and managing acute care medical illnesses.

When you are in the hospital, a hospitalist is an extension of your primary care physician and specialists. They make sure you and your family members get the information and care you need during your stay.

Your hospitalist will: • Be the chief physician of your care while you are in the hospital. • Admit you to and discharge you from the hospital. • Order any lab work or tests needed and will read the results. • Prescribe treatments and medications as needed.

Quality health care is a high priority focused on at Colquitt Regional Medical Center. In 2013, Colquitt Regional Medical Center was named Top Performer on Key Quality Measures by The Joint Commission, the leading accreditor of health care organizations in America.

Colquitt Regional Medical Center is one of the 1,099 hospitals in the United States to earn this distinction. They were recognized for its achievement on the following measure sets: Heart Failure, Pneumonia and Surgical Care Improvement. To receive this honor, hospitals must: 1) achieve cumulative performance of 95 percent or above across all reported accountability measures; 2) achieve performance of 95 percent or above on each and every reported accountability measures where there are at least 30 denominator cases; and 3) have at least one core measure set that has a composite rate of 95 percent or above, within that measure set all applicable individual accountability measures have a performance rate of 95 percent or above. A 95 percent score means a hospital provided an evidence-based practice 95 times out of 100 opportunities. Each accountability measure represents an evidence-based practice – examples include giving aspirin at arrival for heart attack patients, giving antibiotics one hour before surgery, or providing a home management plan of care for children with asthma.

Vereen Rehabilitation Center was dedicated in May of 2013 after Moultrie’s Vereen family. Since the 1930’s, the Vereen family has supported the medical community of Colquitt County. When the Public Works Administration approved a funding request for the construction of a new hospital in Moultrie contingent upon the community matching the request, Mr. W.C. Vereen stepped forward and began

a grass roots campaign for the community to match his personal donation of $50,000. This community matched his gift, and Vereen Memorial Hospital was built in 1939. Today, the Vereen family still values access to quality healthcare in our county. Barbara Bunn Vereen served selflessly on the Hospital Authority Board from 1985 thru 1999

and held the position as Chairperson on the Quality Assurance Committee. Chairman W. Jerry Vereen, Chairman of the Vereen family foundation, and The Community Welfare Association have generously committed to support our Medical Foundation in its efforts to ensure quality healthcare and physician access close to home for future generations. Other trustees on the Community Welfare Association are Barbara Bunn Vereen, Harvey Bunn Vereen, and Lisa Vereen Zeanah.

In addition, the construction of the facility wouldn’t have been possible without the support received through the Waldo Deloache Charitable Trust, which is managed by the Southwest Georgia Bank.

For their generosity, the lobby of the center was named in honor of the Deloache family.

2013 was a milestone for Colquitt Regional Medical Center. Ground was broken in May to begin the largest expansion and renovation project in the history of this community hospital.

Areas involved are Parking, the Emergency Department, Surgical Suites, Intensive Care Unit, and a Centralized Patient Lobby. This project is to be completed by November 2014.

IMPROVING HEALTHCARE IN OUR COMMUNITY!

Volunteers are essential to any organization but especially to Colquitt Regional Medical Center.

We are fortunate to have 61 volunteers both men and women of all ages to form a Volunteer Auxiliary. The Volunteer Auxiliary is a non-profit organization whose mission and purpose is to provide service in a healthcare setting. In 2013, the Volunteer Auxiliary contributed over 26,000 hours to assist and serve our hospital.

•Volunteers greet you at hospital entrances and assist you with directions.

•Volunteers operate the hospital gift shop, The Pink Pearl, for patients, visitors and employees.

•Volunteers provide and deliver daily newspapers and flowers to patient rooms.

•Volunteers assist with filing, phone calls and retrieve mail.

•Volunteers provide healing through smiles and hugs daily.

•Volunteers serve a variety of areas such as Human Resources, Oncology, Rehabilitation, Accounting, and Nurses Stations just to name a few.

•Volunteers donate items and equipment to areas in need for better patient care such as baby blankets and warmers.

Learning Center...2013 showed much growth for Speech-Language Pathology and The Learning Center. Volume increased by 41% with serving students in the following areas in addition to our local students; Camilla, Bainbridge, Lake Park, Lee County, Thomasville, Tifton and even as far as Alabama.

Earlier in the year, The Learning Center presented students of Cox and Stringfellow schools a Fast ForWord Challenge, an effort to succeed, and a one-year gain in literacy and reading. If students met the challenge then they would be rewarded with an iPad mini.

The criteria for this challenge stated that each eligible student had to have good attendance, successfully participation and completion of the program. The challenge resulted in students surpassing each challenge! The average learning gain was two years. As a result, 10 students from Cox and 10 students from Stringfellow were presented with the iPad minis during Honor’s Day at each school.

Not only do our Fast ForWord products help children increase their reading level by one year within as little of 8 to 12 weeks, they have also been proven to be effective with adult patients suffering from cognitive decline, strokes, etc.

Imaging Department received re-accreditation with a perfect score from the Mammography Quality Standards Act in 2013. “With the accreditation, women who receive mammograms at Colquitt Regional Medical

Center can be confident they are receiving the highest quality, consistent and reliable resources for breast imaging,” said David Spence, Director.

Breast cancer can be found early! We believe early detection is the key to fighting breast cancer in women. The American Cancer Society recommends women with no risk factors begin screening with mammograms at age 40. So, Colquitt Regional set out to remind women in Colquitt County to make regular annual appointments by delivering pink roses to all the women in the school system.

At Colquitt Regional Medical Center, mammography patients are given an appointment for the following year and a magnet reminder card.

Patient Satisfaction was a major focus in 2013 and continues to be in 2014. We ended our fiscal year in September 2013 achieving 6 of the 8 HCAHPS domains with scores higher than our goal set for the year. We also achieved our positive responses goal on rate the hospital for

inpatient. In an effort to continue this progress, HCAHPS domain teams have been formed and higher goals have been set for 2014. Our focus is to continue to make the patient experience the best it can be at Colquitt Regional Medical Center.

Colquitt Regional Medical Foundation Celebrates 25 Years!This year marked the 25th anniversary of the Foundation, having been incorporated in 1988. The first Chairman of the Foundation Board was Mitchell Smith, followed by Dr. D.W. Adcock who served 20 years as Chairman. Jimmy Jeter was named Chairman in 2013 and has served on the board since its inception.

During the year, the Foundation received more than 5300 individual donations totaling $345,714.34. Our capital campaign efforts to support the expansion and renovation of the hospitals new Emergency Department, Surgical Suites and Intensive Care Unit brought the majority of the contributions to the foundation with gifts totaling $90,000.

The Foundation conducted several events to raise funds during the year. The first was a gala benefiting our hospice program. Secondly, we hosted a fall golf tournament. Thirdly, we held an Art on the Wall photography sponsorship and lastly was the Christmas Love Lights to benefit pediatric and obstetric needs. These events brought in over $96,000 in revenue.
