Step 1 – Sign the agreement

• The Service Provider Agreement and all applicable exhibits must be completed and signed.

• Service Provider Agreement main contact (page 2) • Agreement Signature (page 42)

• Exhibit C-1 contains the Rights Issuer Certificate information• Exhibit D to include Participating Affiliate to the agreement • Exhibit E CMLA Advisory Board Amendment • Exhibit F to include subcontractors to the agreement • Exhibit G to add authorized employees for order issuance

Service Provider Main Contact

• The main contact is the Service Provider employee responsible for communicating with CMLA on behalf of the Service Provider.

• The main contact has authority to authorize the Service Provider orders for Rights Issuer certificate request

• The main contact (or some other designated employee) may authorize the authorized employees in the Exhibit G (see Exhibit G) only if this person is authorized to legally bind the corporation.

Exhibit C-1

• The information in the Exhibit C-1 will be used to create the Rights Issuer Certificate Request. One Exhibit C-1 is submitted per Service Provider.

• CMLA will use the information in the Exhibit C-1 against the Rights Issuer Certificate Request issued by the Service Provider.

• The format and syntax must be carefully followed e.g. ISO 3166 for the countryName field

•E.g. FI for Finland, US for United States …

• A Service Provider may submit more than one Exhibit C-1 only if each Exhibit C-1 submitted is uniquely numbered by the Service Provider and the commonName is unigue for each Exhibit C-1 submitted.

• The Exhibit D is used to include other companies that are Participating Affiliates of the company signing the Service Provider Agreement

• One completed Exhibit D is required per Participating Affiliate

• Signatures go here• Exhibit D is signed by the same person signing the CMLA Service Provider Agreement (authorized to bind the company).

Step 2 – Send the agreement• The Service Provider sends (via Forwarding Agent, e.g. DHL):

• Signs 2 (two) copies of the Service Provider Agreement

• Send via courier e.g. DHL, the 2 copies of the agreement and all the appendixes to CMLA.

• Indicates via email, the US Agent representing the Service Provider

• (optional) Indicates to CMLA via email other information such as: Purchase Order number, Shipping Address, Billing Address …

• Note: • Exhibit C-1 and Exhibit G must be filled before being able to make a production

order of a Rights Issuer Certificate

Step 3 – Receive the Invoice

• CMLA will send the .pdf invoice via email to the Main Contract Contact as it appear in the Service Provider Agreement

• The invoice contains:

• License fee • Access to the Development on-line system • Handling fee (as specified in the CMLA Service Provider Agreement)

Step 4 – Make the Payment

• Service Provider pays CMLA the invoice amount.

• Banking details: • Banking details will be communicated to the adopter by CMLA Services

via direct contact after completion of the agreement.

• Notice: when wiring funds, please pay your bank wire fees (and that of the "intermediary" bank - usually about $20). CMLA does not enlist the services of an intermediary bank so it will not pay for use of such services. Including this amount in a wire transfer would likely pay for any intermediary bank charge (you may wish to contact your company's bank to find out exact amount).

Step 5 – Receive the Welcome Kit

• At the conclusion of the set-up process the Service Provider will receive the CMLA Welcome Kit including the following:

• One hardcopy of the Service Provider Agreement countersigned by CMLA

• One digital copy of the CMLA Technical Specification with CMLA IP• One digital copy of the Ordering Forms Template (both for CMLA

production and development system)• One digital copy of the Service Provider Ordering Process • Other relevant information
