Were assigned an individual to facilitate linkage to CHANGES TEAM DECISIONS MADE Sheridan Health Services will make a referral to Rothstein Core Center in Chicago and receive a confirmed appointment date at least 30 days prior to discharge and convey appointment to the offender. Sheridan Correctional Center will reissue appointment dates to the client at time of release along with the directory of wrap around services available to the client. Sheridan will also ensure the Release of Information is completed to allow sharing of information between Sheridan, Core Center, TASC, and Illinois Department of Corrections Adult Parole Services. Core Center will schedule the client the first Wednesday after the offenders release. The Core Center will provide outreach services to the client to verify the scheduled appointment. The Core Center will report to Sheridan Correctional Center on outcomes of appointments. TASC will contact the client during the 30 day prerelease Parole Staffing to make client aware of his appointment and stress the importance of keeping the appointment. TASC will follow up with the client upon his release to insure the client keeps his scheduled appointment. TASC will assist the client in getting to his appointment if he is unable to get there. TASC will notify Parole of the date of the Parole Staffing and get Parole the Release s. Parole will attend the 30 day Prerelease Parole Staffing to meet with the client.
