We are excited to launch our new-look The Australian Chiropractor (TAC), in celebration of this new era of the Association. More than a revised look, we have been hard at work to evolve this publication into a more sophisticated and member-centric one. We hope you enjoy the new features and that each edition provides something of relevance and interest for each and every member.

This publication has been close to my heart ever since I first saw the CAA's job advertisement that described overseeing a monthly publication. When I had my job interview a little over three years ago, I told those interviewing me that I would make the TAC ”my baby”. Commencing with the new-look TAC launched in July 2015, I have taken great joy and precision in crafting each edition over the last three years.

It is a fitting time to launch a new-look TAC as we have expanded our team of contributors, including Megan Walker who runs Market Savvy and has an extensive background in marketing health. We also welcome our new Graphic Designer, Matthew Gray, who is responsible for this fresh look to the publication. Matthew brings new perspective to our Association, having

previously worked for Bauer Media and most recently the Qudos Bank Arena in Sydney.

One significant change to the TAC is how we publish our classifieds. Classifieds are now primarily housed on our website chiropractors.asn.au/classifieds and those advertising have the additional option to place their classified in the TAC. This means that we have more space for content and that classified advertisements are able to be updated more regularly as opposed to being primarily housed in a monthly publication.

This is your publication and more than ever, we would love to hear from you, our members. You can contribute by submitting us an article or report, writing a ‘Letter to the Editor’, or even simply sending us a photo or link. You can email me directly at [email protected] to have your contribution considered. While we welcome all submissions, please note that we are under no obligation to print any material submitted for publication.

Lastly, we would love to hear your feedback on this new-look.

ell, here we are. Four years in the making and the ACA has now officially commenced

business. The inaugural ACA Board met for the first time in Melbourne on 2-3 June.

In attendance were all ACA Board directors: Drs John de Voy, Andrew Lawrence, Alison Bennett, Warren Genders, Bruce Whittingham and Mr Martin Baird. To assist with the transition, we also had the retiring CAA Board directors Drs Ari Diskin, Mark McEwan and Harley Rushton.

One important item of business was the election of the President and Vice President. In accordance with sections 18 and 19 of the ACA constitution all future Board directors are elected by the membership, but President and Vice President are elected by the Board.

I was honoured and humbled to be elected as the first President of the new ACA and delighted to have Dr John de Voy as Vice President.

A key focus of our first meeting was to distil the direction for the ACA going forward. The last four years have been somewhat of an aberration for our profession, with unprecedented negativity from both within and outside the profession.

The CAA took the position of setting things right internally: to elevate our levels of professionalism to the point of making us bullet proof. This was a necessary and painful time for many.

It became very clear as we planned for the future, that Andrew Lawrence and the CAA made the difficult decisions that have put us in a stronger position today. AHPRA statutory complaints are now higher in dental, medical and physiotherapy than they are for chiropractic.

We now have the policies, relationships and track record in place, from which we can launch a more positive and outward looking approach to forge the profession forward into the future. Time to draw a line in the sand.

OUR PURPOSEUltimately, the purpose of the ACA is to ensure that more Australians experience the benefits of chiropractic care. It is our job to make your practice life easier.

We are a diverse profession. As a member organisation it is our role to represent you, our members, and navigate a clear direction despite different philosophical views. As chiropractors, we recognise the vital role of the spine and nervous system play not only with back pain, neck pain and headaches, but potentially with other aspects of health and wellbeing.

The ACA bases its direction on professionalism in thought, action and outcome. Part of this professionalism is realising the need to work from an evidence based, patient centred framework.

The message to the greater community needs to be strong, considered and defendable. The message from within the four walls of your practice can be more personalised and expansive in order to suit the best needs of each individual patient.

NEW LOOK AT LOGO AND BRANDINGThere is no better time than now to

take a fresh look at our logo and brand. The options are for a modification, evolution or revolution. In addition to engaging a design agency, we also plan to put this out to crowd sourcing and to the membership, so you can make your opinion known.

The big reveal will happen at the Annual Conference in October. Until this time our branding is officially under new construction.

NEW FEESBy now you would have received your membership renewals for the 2018/19 membership year. Excitedly, we now have equity of service and fees across the nation.

Of note is the extension of the Limited Practice category from 12 to 16 hours. This should make the decision for part-time chiropractors to maintain their ACA membership even easier.

Allowing for tax deductions, a 30% saving on Guild Insurance and a host of other benefits, why wouldn’t you be a member of ACA?

Membership is an important investment into your practice; we look forward to you continuing this with us into a successful future.

he inaugural Board of the ACA met recently to set in place the governance structure to

enable the organisation to commence its operations. A significant part of this was procedural, given the new Constitution and included matters such as membership subscriptions and draft budget for 2018/19 to be in keeping with the proposition put to members.

The ACA budget and subsequent fees were formulated in accordance with the usual business practice using GST exclusive amounts. The fees approved by the Board were based on GST exclusive pricing as is the usual business practice and used in all the restructure documents; GST must be charged in accordance with GST legislation where applicable. Ultimately, it is the end consumer who pays the GST. The entitlement for members to claim a GST credit on their BAS is determined by their individual financial and business structures and arrangements. Therefore, fees are set with the GST exclusive price. Members should seek independent advice from their accountant or financial advisor regarding their entitlement to claim a GST credit; the ACA membership fee is tax deductable.

Additional matters of note included the discussion and adoption of a strategic plan which provides direction for the ACA, the establishment of Regional Committees and the continuance and/or re-establishment of Standing Committees of the ACA. With regards to Regional Committees, these are established in accordance with Section 24.2 of the Constitution.

The Regional Committees act in an advisory capacity to ensure that local priorities are identified, resourced and responded to through the Regional Manager. The Regional Manager reports to the CEO, whilst regional staff report to the Regional Manager. Regional Committees are

being comprised of members of the Association and do not have authority over staff. Regional Committees are integral to ensure local engagement, consultation and action; they act under delegated authority from the ACA Board.

The ACA Board has also set up eight Standing Committees in accordance with Section 24 of the Constitution, to forward the position of the ACA through considered, broad-based engagement of members and appropriate non-member expertise. These Committees include: Policy, Practice Advisory, Public Engagement, Conduct & Ethics, Tertiary Education & Research, Member Engagement, Women in Chiropractic, and Continuing Professional Development. The ACA Board takes advice from a wide range of sources, including committees and external sources to develop and review policies and guidelines.

The Board encourages you to be part of a committee to diversify the views received. A Committee comprises between 4–8 members recruited from the membership and may include staff members of ACA or appropriate invited non-members. Depending on the Committee, members ideally

come from diverse backgrounds and expertise, such as private practice in multi-clinic, multi-disciplinary, solo practitioner and group practice. They also come from sectors such as public health, tertiary education, adult education and research.

Being engaged with a committee gives you an opportunity to be involved and contribute; Committees meet annually in person and as required, using technology to fulfil their Terms of Reference. We are currently seeking expressions of interest for many of our Committees, and invite you to put yourself forward. ADVANCED LEARNING PATHWAYS: THE AUSTRALASIAN INSTITUTE OF CHIROPRACTIC EDUCATION (AICE)Another significant decision taken at the first Board meeting was to progress the development of AICE by supporting the business case paper within the 2018/19 budget to bring it to reality. For the past 12 months, Adjunct Professor Henry Pollard has been working with the CAA National Board to present the business case from a governance, financial and educational perspective to build on the original proposal first launched in 2006. The project was to determine the feasibility and sustainability of profession-led, advanced learning pathways in the field of chiropractic. Due to a conflict in name and identity of the ACC, AICE was accepted and registered with ASIC.

The AICE uses the governance construct of the Royal College of Chiropractors and the academic construct of the Australian College of Physiotherapy. The Institute model provides the opportunity not only as a way to provide pathways and research

for the profession, but may contribute to a defragmentation of profession related interests through a focus on special populations (for example - clinical neurology, paediatrics, sport) in the absence of specialisation under National Law in Australia. The profession doesn’t currently have a simple method for the demonstration of advanced skills and recognition in standardised, post-professional training and AICE is looking to address this. The advanced learning pathways and competencies are envisaged to include:1. professional behaviours of a special

interest chiropractor;2. communication skills of a special

interest chiropractor;3. knowledge, science and advanced

skills in information retrieval and analysis in the special population managed by the chiropractor the application of evidence-based practice;

4. skills in relevant chiropractic, allied health and medical assessment;

The intent is to provide the profession, and the public, with further confidence in what chiropractors provide within the Australian healthcare system through this profession-led approach to advanced learning and is based on existing successful models. More information will be made available and the formal launch of AICE is envisaged at the Annual Conference in Hobart.

knew Ben and many others in the renewals video. It was very satisfying to see them reach their goals of becoming a chiropractor, getting involved in the areas of their interest and providing the high-quality of care required by our patients. It is my opinion that we need to support the future generations of chiropractors as those fabulous chiropractors did for many of us. We have advanced our place in healthcare in so many ways due to the sacrifice of many. However, we still have so far to go.

We often rest on our laurels and, individually, in our own practices are very successful but there is more to it than that. Just as we mature as an individual, most of us move from a self-centred approach and philosophy to one of consideration and actions for

others first. Altruism is a key characteristic of a professional and we need to discuss, nurture and practice more of this characteristic. This is just one example of our profession stepping out of their practices to help patients and promote our profession.

One of the best rewards as an academic and administrator, though there are many, is to see people find their passion and complete their dreams far

his spinal health week was the best one yet. The conditions

highlighted are issues that are pertinent within our practice. It was so great to use the media collateral to help educate everyone. We loved this campaign and have made all the postures a permanent part of our practice along with the pull-up banner which has been placed in our front window for all to see. We are located on a busy main road and we are excited that so many people will be reading the statement “chiro can help”. This tag line is perfect. It covers so many aspects in life and is so specific, yet so general all at the same time. I loved how you chose such basic health conditions to target in order for all generations to relate. Thanks so much for this year's campaign. It will be a massive challenge for you to do better next year!

We represented Queensland members to an audience of 15,000 at the Pregnancy Babies and Children's Expo 2018 held at the Brisbane Convention and Exhibition Centre on 15-17 June.

With a myriad of brands and organisations taking part, it was an important opportunity to get out in the community and showcase our profession and reiterate the message, that ‘Chiro Can Help’ support spinal health.

This event has become a regular part of public engagment activities for the Northern Region, not only do we receive great feedback from the public about chiropractic to support our endeavour, but we are able to highlight how ‘Chiro Can Help’

all the family. In addition to our stand this

year, our promotion incorporated Spinal Health Week posters on display, a take away brochure on spinal health tips for both pregnancy and post-birth, along with handing out a flyer and fridge magnet which promote spinal health for all the family and will serve as a reminder for people to ‘Locate a Chiropractor’.

Each year, we have an amazing group of members volunteer to supervise the stand in shifts across the three day event. We thank each of these volunteers for their professionalism and for assisting to promote a positive chiropractic message to the community in Queensland.

PREGNANCY, BABIES AND CHILDREN’S EXPO – BRISBANE

There were ten enthusiastic chiropractic science students who volunteered at the recent MS Queensland Brissie to Bay Bike Ride, held Sunday 17 June in Brisbane.

The Brissie to Bay Bike ride provided students with a fabulous opportunity to support their community and assist those participating in the event who were raising funds for an important cause.

Chiropractic Science lecturer Dawn Dane said a total of 120 cyclists were treated on the day under the supervision of the Chiropractic Science faculty.

“Around 5,000 cyclists rode a range of distances from 10-100 kms on a cold and windy day and were very happy to see our enthusiastic students for some musculoskeletal attention afterwards,” Ms Dane said.

“Our students offered a mix of soft tissue massage, stretching, taping and gentle mobilisation techniques.”

“It was a great opportunity for our students to build their hands-on skills as well as learning about the management of sore and tired athletes immediately following sporting events”, she added.

Recently we held a celebration to commemorate the End of Era, in terms of the history of Victorian chiropractors associations in their many forms, dating back to 1943 and the beginning of an important new chapter with a newly formed single body.

There was acknowledgement of the many achievements, including registration of chiropractors, the creation of the first government funded chiropractic program, third party insurance cover, etc.

We also acknowledged the contribution of Association staff through the years but more importantly the incredible contribution of those many chiropractors who have selflessly and passionately given back

to their profession to foster its continued development and growth. Almost ironically, the common message from those volunteers present was that they received back far more than they gave.

A special acknowledgement was reserved for Dr Andrew Lawrence, who has been a wonderful servant of the profession over many years. Moreover we thank him for his invaluable stewardship of the CAA during the recent years, even when the profession faced its greatest external challenges.

It was a very enjoyable evening for all, with many reconnections between old friends and the making of new ones.

Dr Mario Stefano, Chair of our Public Engagement Committee, was featured on 9 News Adelaide last week discussing back pain.

"70-90% of the population [suffer] at some point in their lives. It doesn't really tell the true story because there are lots of people right now that don't have back pain but they're suffering with spinal problems that prevent them from doing things that they like doing" said

Dr Stefano, speaking of the wide-reaching effects of lower back pain on Australians.

This year we again assisted RMIT in delivering the Practice Management course (PMC) to its 5th year chiropractic students.

Every chiropractor, whether principal, associate or locum, knows that they operate in the small business environment which involves dealing with a wide range of business issues on top of maintaining their clinical expertise.

The PMC covers a range of business areas that the new graduate is likely to confront in practice. Such areas include basic business principles,

communication, insurance, accounting and finance, industrial relations issues, marketing and the raft of compliance issues that confront a registered chiropractor. It is designed to expose the final year students to these subjects in order to prepare them and ease their transition from student to practitioner.

We would especially like to thank those members who donated their time to present to the students; Drs Anthony Coxon, Bianca Dobson, Nicola Steward and Marcus Yeo.

We are delighted that Guild Insurance is continuing its great support of the Health Sector Leadership Program.

The program is designed for health professionals who are ready to start their leadership journey. It brings together recent graduates from across seven different health professions and with the experts from Leadership Victoria will over the next five months take them through a series of modules covering issues such as ethical leadership, communications, influence and resilience.

This year we were delighted to offer places to Drs Angie Acquilina, Eleanor Caffra, Sarah Jane Perri and Jessica Sexton.

It is important to note that building on the strength of this program, there are now alumni who will work to continue communication between tomorrow’s leaders.

Furthermore, we have just had the first meeting of the Presidents of the various associations involved in the program to discuss matters of mutual interest.

Our vision is to improve the musculoskeletal health of the community, to undertake research that will advance knowledge in musculoskeletal health, improve clinical practice, inform healthcare policy, and improve the health outcomes of the community.

In order to achieve this vision, we need funding to support researchers to conduct high

quality research. With financial support, we can invest in young researchers to undertake critical work to underpin the quality and veracity of chiropractic practice for years to come”, said Professor French.

While Australia leads the world in many areas of healthcare research, investment in research underpinning chiropractic practice is falling behind. In order to survive the increasingly competitive academic research environment, our profession must increase our research productivity.

DR DENNIS RICHARDS - OAMOn the Queen's Birthday, Dr Dennis Richards (NSW) was awarded Medal of the Order of Australia in the General Division (OAM), for his service to the chiropractic profession.

Dr Richards has held a number of leadership roles, most notably as the President of CAA (2005-2009) and also the WFC (2012-2014).

He received the CAA Chiropractor of the Year Award in 2001. Dennis is currently completing a PhD through Southern Cross University.

DR PETER COWIE - OAMDr Peter Cowie (NSW) was awarded Medal of the Order of Australia in the General Division (OAM), for his service to the chiropractic profession.

While currently serving as Treasurer and Councillor/Director at the Council on Chiropractic Education Australasia, Dr Cowie has also served in a number of leadership positions. he restructuring of the CAA into the ACA brings about several changes, including a changing of

the guard. With the election of Dr Anthony Coxon as President, we wish to appreciate Dr Andrew Lawrence for his term as both Deputy-President and President over the past three years.

Dr Lawrence has been a pillar of the chiropractic profession in Australia, serving numerous times on the Board and as President in 1987-1989 (of the original ACA) and again in 2001-2005. Andrew joined the Board most recently in 2012, on the basis that he wouldn’t be President but things didn’t work out that way.

Andrew decided to put his hand up for President again in 2015, as he felt that he had the skills, experience and

stabilising influence the Association needed at the time. Dr Lawrence was first elected Deputy-President in October 2015 during a vacancy in the position of President, before being elected and taking on the Presidency in October 2016.

“Andrew has great intelligence both intellectually and emotionally; he gives matters deep consideration and can guide decision making even when it is a tough call,” said ACA CEO Dr Matthew Fisher.

“Maybe MacArthur got it right - 'A true leader has the confidence to stand alone, the courage to make tough decisions, and the compassion to listen to the needs of others. He does not set out to be a leader, but becomes one by the equality of his actions and the integrity of his intent.' I have

observed great integrity” he said.Since becoming President for

the third time, Dr Lawrance has steered the organisation through the restructure process and some difficult situations for the profession and remains a member of the Executive Committee of the ACA Board.

“As an incoming President I get the inside view on how the ACA/CAA company has been managed under the stewardship of Dr Andrew Lawrence. It’s comforting to know that under his watch our Association is in a strong position today,” said new President Dr Anthony Coxon.

“Andrew’s legacy will be that as Deputy-President and President, has directly oversighted from a loss of ~$471k to surplus. He was strong enough in his convictions to make unpopular decisions that raised the bar of professionalism at a time when we were under intense negative pressure.

“Over the next years I believe you will see the ACA go from strength to strength. This could only have be made possible with a strong foundation, that has been built under Andrew’s leadership”, he said.

Believing he still has much to contribute to the profession, Andrew sees himself as a resource and advisor for younger chiropractors for years to come. The ACA Board and staff wish to thank Dr Lawrence for his service to the Association and the chiropractic profession in Australia.

nsettled infant behaviour and colic are terms used to describe a range of behaviours in infants aged up to 12 months that include prolonged episodes of crying, difficulties with

sleeping and/ or feeding. A baby may cry under normal circumstances including when they are hungry, wet or tired. While there remains no consensus on classification, excessive crying from colic often occurs at the same time of day, often late afternoon or evening, with the most widely used definition of colic reported as unexplained episodes of paroxysmal crying for more than three hours for three days per week for at least three weeks.1 While fewer than 5% of distressed infants are reported to have identifiable medical explanations, consideration should be given to medical assessment in order to exclude conditions that may require further investigation and treatment.2 It is also important for clinicians to be aware that colic normally peaks at six to eight weeks with symptoms spontaneously decreasing by three to four months.3

In this article, we examine the evidence base for manual therapy for unsettled infant behaviour or colic in light of a systematic review published in BMJ Open in February: Manual therapy for unsettled, distressed and excessively crying infants: a systematic review and meta-analyses.4

To provide context for the recent systematic review, we will first re-examine the evidence base for manual therapy to treat unsettled infant behaviour or colic as described in the CAA Research Summary and Strategic Research Opportunities 2016 (RSSRO).

This topic has proved to be a controversial one for chiropractors, not least because of advice published by the Chiropractic Board of Australia in March 2016 when it stated:

“Patients must be adequately informed when making healthcare choices. Advertisers must ensure that any statements and claims made in relation to chiropractic care

do so5 (available from our website). The RSSRO was also featured in the June 2018 edition of The Australian Chiropractor.

One purpose of the RSSRO is to empower chiropractors to understand the current evidence in relation to public claims of effectiveness, and how to communicate the quality of this evidence to patients. The method used to rate quality of evidence in the RSSRO and Carnes et al., follow from Bronfort et al. (2010)6 and Clar et al. (2014).7

Appendix 1 of the RSSRO (p. 44) outlines the level and quality of evidence with recommended actions that each level of evidence will support (see table 1):

INFANTILE COLIC: SPINAL MANIPULATIVE THERAPY AND MASSAGEThe RSSRO states the following in relation to colic:

WHAT IS KNOWNThere have been two high-quality systematic reviews to assess the effectiveness of SMT for colic.8,9 Both systematic reviews found limited evidence to support manual therapy as a more effective intervention than sham therapy for the treatment of colic.

Dobson et al. (2012) in their review of the evidence concluded: “Quality of the evidence is mixed: the studies included were generally small and methodologically prone to bias, which makes it impossible to arrive at a definitive conclusion about the effectiveness of manipulative therapies for infantile colic. Taken together, the evidence seems to suggest that there may be benefits in terms of reduction in crying hours from low-quality evidence for a reduction in daily hours of crying of over one hour and for a greater proportion of patients reporting resolution of their infants colic symptoms. The majority of the included trials appear to result in significant reductions in reported crying hours per day and in a greater proportion of parents reporting clinically significant reduction in daily crying.

If one excludes the poorer-quality evidence, these benefits do not reach statistical significance: most studies had a high risk of performance bias introduced, owing to the fact that the

are not false, misleading or deceptive or create an unreasonable expectation of beneficial treatment.

The Board is concerned about a number of practitioners who are making claims in advertising that there is a relationship between manual therapy (such as manipulation) for spinal problems and achieving general wellness or treating various organic diseases and infections. Furthermore, they are concerned about advertising claims suggesting that spinal problems may have a direct role in various organic diseases and infections. It is important to note there is insufficient scientific evidence to support these claims.

Of particular concern is the number of treatment claims in advertising relating to infants and children. Claims suggesting that manual therapy for spinal problems can assist with general wellness and/or benefit a variety of paediatric syndromes and organic conditions are not supported by satisfactory evidence. This includes claims relating to developmental and behavioural disorders, ADHD, autistic spectrum disorders, asthma, infantile colic, bedwetting, ear infections and digestive problems."

• it does not support public claims regarding effectiveness;

• to recommend an effective alternative to patients if available; and

• to advise patients that this is a treatment option in the absence of an effective treatment.

WHAT DID THE RECENT SYSTEMATIC REVIEW AND META-ANALYSES FIND?More recently, Carnes et al. (2018)4

conducted a systematic review and meta-analyses to assess the effectiveness of manual therapy for healthy but unsettled, distressed and excessively crying infants. They also provided information to help clinicians and parents provide care decisions.

Nineteen studies were included, and consisted of seven randomised controlled trials, seven case series, three cohort studies, one service evaluation study and one qualitative study.

In conclusion the authors state:“We found moderate favourable evidence for the reduction in crying time in infants receiving manual therapy care (around 1 hour per day), but this may change with further research evidence. We still do not know if this result is meaningful to parents or if the reduction is due to the manual therapy component of care or other aspects of care. For other outcomes, the strength of evidence was low and inconclusive.”

It is important to note the strengths and limitations of this study as described by the authors, in particular:• “meaningful outcomes for parents

with distressed, unsettled and excessively crying infants were investigated to help inform their decisions about seeking manual

therapy care for their infants;• compiling evidence for distressed,

unsettled and excessively crying infants based on multiple ‘clinical diagnoses’ using varied definitions is difficult;

• the mechanism of action of complex interventions was not explained by the pragmatic research investigations used in this review; and

• low to moderate quality studies limited the certainty of conclusions, suggesting they are liable to change with further research.”

CONCLUSIONIn summary, Carnes et al. (2018) offers equivocal support for the management of unsettled babies using manual therapy. This result is unlikely to provide sufficient support or justification for the Chiropractic Board of Australia to change its stance as regards advertising. In other words, advertising claims that suggest manual therapy can assist with unsettled infant behaviour or colic are not supported by robust evidence, so cannot be made.

Practitioners should note that where better evidence for care for managing colic and/or excessive crying exists outside of chiropractic then patients should be advised of this. If not, a trial of manual care can be discussed after an explanation of the current evidence on effectiveness and safety is provided.

Since publication of the recent systematic review there has been discussion about whether this new evidence provides evidence for safety relating to treatment by manual therapy. Although the review reported adverse events, the study design (type of study) was not sufficient to test safety of any particular intervention. The authors extracted data on adverse events, as reported by primary studies, to conclude that manual therapy is a relatively low risk intervention. However, they argue the need for better study designs that focus on adverse events in children before robust conclusions about treatment safety can be made.

assessors (parents) were not blind to who had received the intervention and when combining only those trials with a low risk of such performance bias, the results did not reach statistical significance. We cannot quantify any risk of adverse effects when using manipulative therapies for the treatment of infantile colic.”

In addition, a Miller et al. (2012)10 conducted a single-blind randomised trial to test the efficacy of manipulative therapy for the management of infantile colic. They reported no adverse events, and a positive effect after controlling for performance bias (parent assessors). However, study limitations were small sample size and (early) discharge of patients part way through the study due to early improvement (i.e. any relapse was not recorded). The authors concluded that rigorous randomised trials to track adverse events with parental blinding were required to advance the field.

These examples highlight methodological limitations within the current literature on this topic. When combined with the lack of high-quality basic science research needed to underpin biological mechanisms associated with this type of treatment, caution must be exercised when recommending this type of care for infants with this presentation.

EVIDENCE RATINGIn summary, the RSSRO reported ‘favourable inconclusive evidence’ for cranial osteopathic manual treatment, massage and spinal manipulation for the treatment of infantile colic.

pinal Health Week is our flagship public education campaign and in 2018 used the theme ‘Chiro Can

Help’ to help Australians discover how chiropractors can help improve their quality of life. Held 21-27 May, this year’s campaign highlighted some common symptoms that prompt people to visit a chiropractor, such as neck and back pain. By using common characteristics of modern life, such as phone usage and binge watching, we aimed to educate the public on the benefits of chiropractic care in a creative and relevant way.

“I feel the theme was relevant and applicable for the average person and led to good discussion with patients as well as creating interest,” said Dr James Farmer from Maryborough, Queensland.

Many of our members took part wholeheartedly and we were delighted to see the high level of participation. We saw our members engaging with the campaign by putting up posters, wearing Spinal Health Week t-shirts, raising awareness using local media and posting thought-provoking content on social media.

It was great to see the resources provided by CAA National put to good use for the campaign. “The videos and social media images were great and easy to share and get the message across, said Dr Eleanor Cafra from Tullamarine, Victoria.

We were very pleased with this year’s media coverage, with almost 300 positive articles in online news and newspapers, up from 91 last year. This

positive media response to SHW has been a testament to the collective hard work put in by so many members across Australia. Many thanks and congratulations to everyone who contributed.

This year we released a series of videos which recieved over 75,000 views across our social media channels, up from 44,000 last year. Approximately 250,000 Australians saw one of our social media posts and just under 5,000 visited our campaign website.

The online presence of Spinal Health Week continues to grow and this year we had almost 900 social media posts using our campaign hashtag #ChiroCanHelp, up from 580 last year. “The social media templates and images were helpful and made it easy to schedule posts for the week”, said Dr Aisha Strand from Mortdale, NSW.

THE SOCIAL MEDIA TEMPLATES AND IMAGES WERE HELPFUL AND MADE IT EASY TO SCHEDULE POSTS FOR THE WEEK.

Dr Lindy Kuburic from Balance For Life Chiropractic in Castle Hill, NSW won a $200 gift card as well as a full set of SHW promotional material. Their clinic encouraged patients to take a photo with ‘chiropractic’ props which were then placed in a #ChiroCanHelp frame and posted onto their Facebook and Instagram accounts. Congratulations to the Balance for Life Chiropractic team and thank you to all the members who participated.

As part of our commitment to providing high-quality public education campaigns, we sought members’ feedback on this year’s campaign. 84% of members were satisfied with this year’s theme. The videos were particularly well received by members, with the only negative feedback being that they weren’t on television. “I loved the video and the comedy feel it had to common issues that people can relate to”, said Dr Karina Drake from Terrey Hills, NSW.

We would like to thank all members for sharing our posts and also contributing to their own social media accounts and websites throughout the week.

To celebrate SHW we ran several competitions. We received 236 colourful entries for our childrens colouring competition. This year our lucky winners received a pair of Vivobarefoot shoes from the kids Primus range, valued up to $129.95.

We received over 100 entries for our online competition, asking participants to tell us in 50 words or less how ‘Chiro Can Help’ improve their quality of life. Our three lucky winners are: Simon Aird, SA (iPad); Margaret-Anne Lauren, WA (Wireless Headphones); and Mollie Smith, NSW (Nutri Bullett). Congratulations to Simon, Margaret-Anne and Mollie, and thank you to all those who participated.

Members have indicated their preference for more online marketing resources, reflecting the shift of marketing campaigns from print to online. We are looking at how to expand our online resources for future campaigns and help members utilise the cost-effective nature of online marketing.

Once again, we would like to thank members for making Spinal Health Week 2018 a success. We look forward to developing another successful campaign next year.

he Australian Health Practitioner Regulation Agency (AHPRA) is a Federal Government

agency that regulates approximately 630,000 Australian health practitioners in partnership with 15 National Boards.

The organisation’s mission is to develop a system that has public safety at its heart. This means that more than 630,000 registered health practitioners are expected to follow professional standards of practice set by their health profession board and this includes ethical advertising that adheres with the National Law.

“The Australian public is entitled to receive good quality information about their healthcare services. Health practitioners, as trusted professionals, have regulatory obligations when advertising a regulated health service.” [Source: AHPRA]

THE RULESUnder the National Law, a regulated health service or a business providing a regulated health service must not advertise in a way that:• is false, misleading or deceptive;• uses gifts, discounts or

Wording about the potential to reduce the severity of symptoms is often safer, such as “I cannot cure arthritis but I may be able to reduce the severity of the symptoms”.

THE WORDS “CAN HELP / IMPROVE/ TREAT” OR “EFFECTIVELY TREATS” When there is acceptable evidence that a health service can help certain conditions, it may be reasonable to state something like “x treatment or x approach can help / improve these conditions”. When there is limited or inconclusive evidence that treatment can help certain conditions, it is unacceptable to claim or suggest that it can help / improve or treat those conditions. In these cases, it can still be misleading to state that treatment or a particular approach may / might help or improve certain conditions unless the advertisement is clear about the limited or inconclusive evidence.

THE WORD “SAFE” When a treatment is generally considered safe based on acceptable evidence, it may be reasonable to

use wording like “x treatment is generally considered to be safe but occasionally may be associated with possible adverse reactions in individual cases”. It is potentially misleading to state that treatment or a particular approach is safe without also acknowledging that all forms of treatment have the potential for adverse reactions.

THE WORD “EFFECTIVE” When there is acceptable evidence that a health service can help certain conditions, it is acceptable to state something like “X treatment or approach has been shown to be effective for the treatment of these conditions”. When there is limited or inconclusive evidence that treatment has been shown to be effective in the management of certain conditions, it may be reasonable to state something like “there is mixed and / or inconclusive evidence about whether X treatment or approach may be effective in the management of certain conditions”.

NAIDOC Week

he National Aborigines and Islanders Day Observance Committee (NAIDOC) is a committee

made up of both Indigenous and Islander groups and NAIDOC week takes its name from this committee. This year, NAIDOC Week is held 8-15 July and celebrates the 65,000 years of unbroken rich cultural history that makes Australia unique.

This week has its origins before the 1920s when Aboriginal rights groups boycotted Australia Day in protest against the status and treatment of Indigenous Australians, called the Day of Mourning. In 1955 this day was shifted to the first Sunday in July and it became not only a protest day, but also a celebration of aboriginal culture. In 1974 the NAIDOC committee comprising entirely of indigenous

members, decided the event should cover a week.

This year’s celebrations are themed “because of her, we can” and celebrates Aboriginal and Torres Strait Islander women, their achievements, passion and the strength they have and give to past, present and future generations. Such women include Evonne Goolagong Cawley who works with young indigenous tennis players, Linda Burney a member of parliament and also pioneering women such as Truganini and Mum Shirl.

here is more than meets the eye when it comes to healthy ageing, and Professor Peter Reaburn

is no exception. As Professor and Head of Exercise and Sports Science at Bond University’s Bond Institute of Health and Sport, he is an exemplar of a lifetime’s commitment to health and fitness. His athletic achievements parallel his professional ones: he is an academic, author, researcher, competitive swimmer, Surf Lifesaver and National Ironman Triathlete, to name a few.

With an abundance of knowledge and experience to share, Professor Reaburn will be doing just that, as he steps on stage as a keynote speaker at CAA’s Annual Conference in Hobart this October. His session will focus on masters athletes and the conclusive findings that highlight these individuals as paragons of successful ageing. He will discuss the age-related changes in sports performance, why such changes occur, as well as the measures that are necessary in preventing the deterioration in athletic performance and recovery.

Older masters athletes do recover less quickly than their younger counterparts and there is now conclusive scientific evidence behind why this is. Quite simply, “older athletes take longer to assimilate proteins”, Professor Reaburn adds. So, what can older masters athletes do in their biggest competition against nature, and how can they age successfully and keep performing at their best? At a physical level, he suggests they increase their protein, carbohydrate and nutritional intake, do active recovery, stretching and utilise compression activewear.

As he awaits this year’s conference, Professor Reaburn has his objectives in clear sight. He aims to help practitioners gain a better understanding of the older masters athletes; they “…are as dedicated and as committed as younger athletes and, therefore, are to be taken seriously”, he said. He also aims to teach practitioners to take a comprehensive approach when working with such individuals, in turn understanding the fundamental reasons for their engagement and

motivation. He anticipates that attending practitioners will come to have a better understanding of this area; “…they’ll take away four or five strategies that they can use in their practice to better screen and work with older athletes”, he said.

Professor Reaburn agrees that a positive mindset, dedication and healthy lifestyle are some of the fundamental complementary factors that facilitate ‘successful aging’. He does, however, suggest the need to look beyond the periscope and learn from other countries and their advancements. He advocates for change not only at Federal Government level, but also at a societal level. That is, we need to work towards a cohesive approach that addresses the individual, public and governmental components of healthy aging. He cites the Netherlands and Canada as prime examples of this; here “…the government has created infrastructure,” he said of the aforementioned countries.

“If you put in footpaths and put in cycleways, people will use them; put in walking trails and mountain bike trails, people will use them, and that’s what we really need in Australia”, he added.

Professor Reaburn agrees with the need to move towards a proactive approach to health and ageing, moving away from the reactive paradigm that has previously governed the treatment, rights and access of ageing Australians.

“We are being reliant on doctors to fix our health, whereas all the evidence points to exercise as medicine”, he added.

With the conference approaching, we look forward to acquiring more knowledge on this subject and working together to abolish the stigma that is too often attached to ageing. It is individuals like Professor Reaburn that pave the way for a bright future for ageing Australia. ccording to some, stress can be good. Specifically, small doses of stress are said to

keep us alert, on our toes and able to overcome the challenges put before us. As we are all aware, however, too much stress can be dangerous and this is particularly pertinent in the workplace. While it is fair to assume that nearly all of us will encounter stress at some point in our working lives, this is not to say stress is uncontrollable. This article will provide some practical tips for employers and employees to ensure stress is controlled, managed and limited in your workplace.

STRESS? WHY THE WORRY? If left to fester, stress can lead to more serious mental and physical health issues. Sadly, research conducted by the Black Dog Institute found that one in six Australian workers are currently suffering from a mental illness. Of greater concern, however, is that many more are said to be suffering from issues such as excessive worry, insomnia and fatigue.

As employers, there is an obligation under the Work, Health and Safety Act 2011 to provide a safe and healthy workplace for all workers. Therefore, limiting and controlling stress can go a long way to reducing the likelihood

TIPS FOR EMPLOYERS

BE REASONABLE IN YOUR EXPECTATIONS While every employee is expected to perform to a particular standard, one which positively contributes to the overall functionality of your practice, placing unreasonable or unrealistic expectations on them can be one of the key triggers of workplace stress. Demanding excessive overtime, placing an unrealistic deadline on a particular task or repeatedly expecting an employee to perform at a level 'beyond their pay grade' can contribute significantly to workplace stress.

BE CLEAR, CONSISTENT AND UNAMBIGUOUSTwo of the major contributors to stress in the workplace are task ambiguity and information overload. Task ambiguity is when a worker is not clear on the job at hand; whether this be from confusing instructions,

This will help them to feel more valued and allow them to stay on top of their work. Similarly, ensure communication channels are open and transparent.

ACORN Update

he ACORN project has continued to make excellent progress since initial recruitment in

late 2015. The commitment of the 2,005 chiropractors who completed the baseline practitioner survey and the 1,680 chiropractors who joined the ACORN practice-based research network (PBRN) database has provided a solid platform for a wide range of publications, sub-study initiatives and extensive research capacity building in Australian chiropractic profession and beyond, to date.

ACORN is the only chiropractic PBRN worldwide to have acquired US Department of Health and Human Services, Agency for Healthcare Research and Quality (AHRQ) Registration and approval and has attracted the largest coverage of any PBRN for any profession in world to date (Adams et al 2017).

To date, the ACORN PBRN has directly produced 17 publications. With ten papers published or in press/submitted, in the journals BMJ Open, Australian Journal of Rural Health, BMC Complementary and Alternative Medicine, Chiropractic and Manual Therapies, Chiropractic Journal of

Australia, Journal of Manipulative and Physiological Therapeutics, Australasian Journal of Ageing).

The ACORN PBRN remains active and strong with latest sub-study recruitment achieving over 30% response rates (please note: the average response rates from non-coordinated recruitment not via a PBRN is in the region of 5-15%). A number of recent sub-study recruitments have successfully utilised the ACORN database and the database continues to attract extensive engagement from not only across Australia but also internationally with another four sub-study proposals in train at time of writing. ARCCIM continues to offer ACORN recruitment at no cost to applicants and we foresee the resource being available, and utilised, for the mid- to long-term period (well beyond the initial three years of CAA National funding).

hiropractors often provide care for children and adolescents with musculoskeletal (MSK)

conditions such as spinal pain and sports injuries. In clinical practice, young patients and their parents seek to understand the cause of these painful conditions. The clinician’s response has a meaningful contextual impact on the nature and quality of chiropractic care. Growth and maturation, the defining biological events of adolescence, are frequently clinically linked to MSK disorders in young people. This snapshot evaluates evidence underpinning the assertion for a relationship between growth, maturation, and MSK conditions in adolescents.

ARTICLE BACKGROUND The prevalence of MSK conditions increases steeply during adolescence to adult rates.1 Persistent problems in this period of life (adolescence) strongly predicts pain and disability later in life.2 Understanding the role of biological development in the onset of MSK conditions in adolescents is important to guide clinical and preventative efforts. Rapid physical growth (i.e., ‘the adolescent growth spurt’) and biological maturity (e.g., stage of skeletal or pubertal development) have been proposed as risk factors for MSK pain and injury.3

Michael Swain is a chiropractor, health researcher and full time lecturer in the Department of Chiropractic at Macquarie University. His clinical interests are in sports injuries and pain management. He completed a Master of Philosophy in 2011 and is currently completing a Doctor of Philosophy (PhD) in the School of Public Health at the University of Sydney. His research focuses on the epidemiology of sports injuries and adolescent musculoskeletal pain.

Despite much clinical inference, it is currently unclear the role physical growth and pubertal change plays, if any, in adolescent pain and injury.

While adolescence has been defined as the second decade of life (10-19 years of age), chronological age remains a poor clinical measure to encompass the construct of physical growth and maturation. Adolescents of the same age can vary considerably in physical stature, and the timing and rate of pubertal events ranges greatly between adolescents. Pain in adolescence, like in adulthood, is clinically contextualised through a bio-psycho-social lens, with several potential risk factors adding to the clinical impression. Given there is no systematic review on the topic, we aimed to specifically determine whether physical growth and/or stage of development are linked to MSK conditions in adolescents.

METHODSThis study is a systematic review of the literature. Three electronic databases (PubMed, EMBASE and CINAHL) were searched up to 6 September 2017. We identified epidemiological studies that evaluated the association between biological maturation or growth, and MSK disorders in adolescents (chronological age 10-19 years). The quality of each study was appraised by assessing sources of study bias.

Associations between maturation and/or growth, and MSK conditions were extracted. Meta-analysis was not possible due to clinical and methodological heterogeneity, so narrative synthesis was conducted.

RESULTSA total of 56 studies exist on this topic and were included in our narrative synthesis. All included articles were quality appraised and deemed to be at high risk of bias.

Of the 56 included studies, 25 studies evaluated associations with outcomes of pain, 22 with injury and nine with fracture. Pain types identified in studies include back pain (13 studies), neck pain (three studies), extremity pain (five studies), head/face pain (nine studies), chest pain (one study), and any or multiple pain (eight studies). This snapshot will summarise only studies that evaluate MSK pain.

At present, 101 associations comprise the total body of knowledge on the relationship between adolescent development and back, neck, extremity, or any report of MSK pain. Exposure measures, the factor that may be associated with an outcome of interest for adolescent development were categorised as either biological maturity (i.e. physical and physiological state of human development) or growth (i.e. the process of increasing in size).

Regarding biological maturity, 35 associations evaluated biological status (a physical characteristic state) and 23 associations evaluated biological timing (a physical characteristic at a given time). For growth, 26 associations evaluated growth rate (increase in physical stature in a given time-period) and 17 associations evaluated growth spurt (a prespecified rapid growth rate). Across the 101 associations, there were inconsistent patterns of association for both longitudinal (a research design that involves repeated observations of the same variables) and cross-sectional (a research design that measures a variable at a specific point in time) estimates. Towards this, some associations reported a positive relationship between growth/maturation and MSK pain, some reported no associations, and some reported negative association between growth/maturation and MSK pain in adolescents.

DISCUSSIONThis study found little research evidence to support the supposition that rapid growth and/or stage of development are linked to MSK conditions in adolescents. Although there are 56 articles on the topic, the knowledge base has several gaps for common MSK conditions. The data that are available provide limited evidence due to high risk of bias, including inherent limitations in studying risk in cross-sectional studies.

In primary care, care seeking due to pain increases dramatically from childhood to adolescents. More than 10% of adolescents seek medical care for musculoskeletal pain.4 Lower extremity conditions, specifically knee pain is the most common reason for adolescents to seek care. Primary care providers must postulate a timely and accurate diagnosis to aid in the effective management of pain conditions.5 Albeit, there are few researchers and limited research into child and adolescent pain to empower clinicians’ to diagnose, or identify a biological cause that explains pain in adolescence, much less select an appropriate treatment intervention.

Several risk factors have been proposed for pain in adolescents.

These include demographic, clinical, biological, family, psychological, ergonomic, and lifestyle factors. A systematic review by Huguet et al., suggests there is high-quality evidence for low socioeconomic status (SES), and moderate-quality evidence for negative emotional symptoms and regular smoking as risk factors for MSK pain in adolescents.6 Shared psychosocial factors (including catastrophising, negative affectivity, and poor sleep hygiene) have been proposed as mechanisms underlying the associations between SES and emotional symptoms, whereas structural damage to body systems is thought to be the consequence of exposure to cigarette smoke and nicotine. Challenging our clinical beliefs was the finding that there is moderate-quality evidence to also suggest that high body mass index, taller height, and having joint hypermobility are not risk factors for onset of MSK pain.6 Ironically, other factors that we, as clinicians, have historically thought to be important predictors of LBP, such as “poor” spinal posture, scoliosis, carrying school bags, joint hypermobility, and poor back muscle endurance, are not strong predictors for MSK pain in adolescents.7

Our study advances the adolescence field in several ways. Firstly, it is the first study to draw doubt on a meaningful link between growth, maturation and MSK pain, which has formed a longstanding basis for clinical management of MSK conditions. For example, our review queries the position statement of the American Medical Society for Sports Medicine that states that overuse injuries may be more likely during the adolescent growth spurt.8 This position leads clinicians to monitor adolescent growth rate and limit training workloads during rapid growth periods. Secondly, and unlike previous work, we delineated the construct of pubertal development into specific construct domains of biological maturity status (e.g. Tanner staging), biological timing (e.g. age at peak height velocity), growth rate (i.e. change in physical stature within a specific time-period) and growth spurt (e.g. 5 cm of height growth in

a 6-month period). This, along with clearly separating MSK outcomes (i.e. specifying factors and effects) allowed us to appropriately evaluate studies that propose causality. Finally, by evaluating each study for risk of bias and applying the Bradford Hill criteria for causal inference in our evaluation of aetiological associations we have highlighted methodological issues that currently impede progress in understanding adolescent MSK conditions.

Our review also highlights several knowledge gaps in the adolescent MSK field, which may be of clinical interest to chiropractors who work with youth sports. For example, periods of rapid growth have traditionally been linked to the onset of traction apophysitis, such as Osgood-Schlatter Disease. However, we found only one article that explored this clinical viewpoint.9 In this study, skeletal age and disorders of the knee extensor mechanism were studied in 40 young male soccer players, and no association was found. Adding to the notion of adolescent vulnerability is the biologically plausible relationship between rapid growth and fracture, given a reduction in cortical bone mineral density occurs as growth increases during early-mid puberty. We found only two conflicting studies had attempted to determine whether maturation (not rapid growth) is associated with fracture in adolescents. The inconsistency between these two studies, and lack of exposure construct specifically typifies the issues that create uncertainty in the field.

CONCLUSIONWhile the relationship between maturation, growth and MSK conditions remains plausible, the available evidence is not supportive. This review challenges our clinical beliefs and highlights the need for a flexible and targeted multidimensional approach to assessment and management of adolescent MSK pain. Given the potentially adverse contextual impact, clinicians should avoid supposing a causal relationship between maturation, growth and MSK conditions as studies on the topic report inconsistent findings and are at high risk of bias.

or over 10 years, Kim Tompkin has been pivotal in keeping the Association running smoothly. Now

as Company Secretary, Kim works closely with the CEO and Board in the legal requirements and governance processes in running a company.

Kim joined the CAA in 2008 as the CPD Coordinator, a role she held for approximately a year. She had previously worked for a metal manufacturing company, working across marketing, administration and executive services. These skills proved to be of value for the Association and she became better utilised as an Executive Assistant to the CEO at the time.

Over the last 10 years Kim’s role has expanded significantly as the Association has grown and evolved. As Company Secretary, Kim is vital to the operations of the Board. For Board meetings Kim arranges the meetings, prepares papers, writes the minutes and follows up action items. Kim also coordinates the AGM, including the Awards and the election process - from notice of nominations to counting the votes.

Our various Committees are supported by Kim, including the work relating to them such as the coordination of PhD and MRes scholarships. Kim has also been involved with other projects that come up, such as the restructure or the relocation of the Head Office to Parramatta.

“It’s never been boring working for the Association as it is continually evolving. I’m particularly excited for the changes coming with the restructure” added Kim.

Kim lives on the fringes of Sydney with her husband Mark, and recently celebrated 30 years of marriage. They have three daughters and now have four grandchildren – all boys!

When not working or travelling the country for Board meetings, Kim enjoys camping and keeping active – running, boxing and even kayaking. Kim has even completed the MS Sydney Wollongong Charity Bike Ride, cycling for 68kms to raise money for this important cause.
