After 89 years of age the prescriptions tended to decline in almost all the first 10
therapeutic classes.The prescriptions of statins and beta-blocker agents began to
decrease after 74 years.
Inappropriate prescribing encompasses:
• The use of medications that introduce a significant risk of an
adverse drug event (ADE) when there exists evidence for an
equally or more effective but lower-risk alternative therapy
for treating the same medical condition
• Over-use of medications at a higher frequency or for longer
duration than clinically indicated
• Under-use of medically indicated medications based on ageist
or irrational reasons
• Use of multiple medications that have documented drug-drug
interactions or drug-disease interactions

Esistono molte forme di interazioni farmacologiche e, in particolare
nei pazienti anziani, è possibile riscontrare interazioni farmaco-
farmaco, che sono anche le più comuni, ma anche interazioni
farmaco- malattia, farmaco-cibo, farmaco-alcool, farmaco-prodotti
erboristici e farmaco-stato nutrizionale

In a European study of 1601 elderly outpatients living in six European
countries, 46% of patients had at least 1 potential clinically significant
drug-drug interaction, and 10 %of these interactions were regarded as
high severity (Bjorkman et al. 2002).
Lindblad et al. Assessed drug-disease interaction s in a group of frail
elderly patients in hospital. They reported that 15-40% of patients had a
potential drug-disease interaction . The use of several prescription drug
and a higher comorbidity index were significantly associated with having
one or more potential drug-disease interactions
On the other hand…
1. There is a large variability in how drug interactions are defined, their
clinical importance and the sources used to detect them
2. Many potential drug interactions never lead to an actual clinical effect
A study in France showed that half of patients admitted to hospital had
at least one potential drug-drug interaction, but that this interaction led
to an adverse drug event in a quarter of patients (Doucet et al. 1996).

The elderly patients received 50.5% of the overall
prescriptions, corresponding to a mean of more than 4 active
substances per person

More than 700000 patients are treated in the emergency departments annually
for ADEs, accounting for 2.5% of all emergency department visits for
unintentional injuries

L’aderenza ai regimi polifarmacoterapici
L'aderenza alla terapia è generalmente più alta in
pazienti affetti da patologia acuta, mentre tende ad
essere bassa nei pazienti affetti da patologie
croniche, riducendosi drammaticamente nei primi
sei mesi dalla prescrizione farmcologica.
I trials clinici riportano tassi di aderenza alla terapia
compresi tra il 43 ed il 78% tra pazienti che
ricevono un trattamento per condizioni croniche
Tra tutti i ricoveri ospedalieri riconducibili a
trattamenti farmacologici, il 33-69% è determinato
da una scarsa aderenza alla terapia.

The extent to which drug adherence may affect survival remains unclear, in part because
mortality differences may be attributable to "healthy adherer" behavioral attributes more so
than to pharmacological benefits.
OBJECTIVE: To explore the relationship between drug adherence and mortality in survivors
of acute myocardial infarction (AMI).
DESIGN, SETTING, AND PARTICIPANTS: Population-based, observational, longitudinal study
of 31 455 elderly AMI survivors between 1999 and 2003 in Ontario. All patients filled a
prescription for statins, beta-blockers, or calcium channel blockers, with the latter drug
considered a control given the absence of clinical trial-proven survival benefits.
MAIN OUTCOME MEASURES: Patient adherence was subdivided a priori into 3 categories--
high (proportion of days covered, > or =80%), intermediate (proportion of days covered,
40%-79%), and low (proportion of days covered, <40%)--and compared with long-term
mortality (median of 2.4 years of follow-up) using multivariable survival models (and
propensity analyses) adjusted for sociodemographic factors, illness severity, comorbidities,
and concomitant use of evidence-based therapies.

RESULTS: Among statin users, compared with their high-adherence counterparts, the risk of
mortality was greatest for low adherers (deaths in 261/1071 (24%) vs 2310/14,345 (16%);
adjusted hazard ratio, 1.25; 95% confidence interval, 1.09-1.42; P = .001) and was
intermediary for intermediate adherers (deaths in 472/2407 (20%); adjusted hazard ratio,
1.12; 95% confidence interval, 1.01-1.25; P = .03). A similar but less pronounced dose-
response-type adherence-mortality association was observed for beta-blockers. Mortality
was not associated with adherence to calcium channel blockers. Moreover, sensitivity
analyses demonstrated no relationships between drug adherence and cancer-related
admissions, outcomes for which biological plausibility do not exist.
CONCLUSION: The long-term survival advantages associated with improved drug
adherence after AMI appear to be class-specific, suggesting that adherence outcome
benefits are mediated by drug effects and do not merely reflect an epiphenomenon of
"healthy adherer" behavioral attributes.

paradox”, ovvero i pazienti che presentano i rischi
più elevati a causa delle loro molteplici patologie
croniche sono anche quelli che meno
probabilmente ricevono il trattamento

Solo il 19.1% dei pazienti assumeva una statina in prevenzione secondaria . Nei pazienti
di età compresa tra i 66 ed i 74 anni, la probabilità di prescrizione di una statina era del
37.7%, 26.7% e 23.4% rispettivamente in caso di basso, intermedio o alto rischio. La
probabilità della prescrizione di una statina era del 6.4% in meno per ciacun anno di età
in più e per ogni incremento dell’1% del rischio di mortalità a 3 anni
Conclusioni
• Il paziente anziano è generalmente un paziente complesso in
relazione alle modifiche fisiologiche età-correlate ed in
considerazione della frequente comorbidità che lo
contraddistingue
• La comorbidità richiede spesso una polifarmacoterapia che, a
sua volta, espone il paziente a rischio di interazioni
farmacologiche e di eventi avversi a farmaci
• La complessità del trattamento, oltre a fattori di natura socio-
economica ed assistenziale, può condizionare l’aderenza al
trattamento
Conclusioni
La scelta di continuare/instaurare un determinato trattamento
farmacologico non può e non deve dipendere solo dall’età e/o
dal numero di farmaci già in atto
