Calderdale Council is committed to offering a wide range of high quality social care and wellbeing services. We understand that everyone is different and we want to make sure that you receive the care you need. We are dedicated to helping people maintain their independence and quality of life, where possible in their own home.

We work closely with independent care providers, the NHS, the Care Quality Commission (the independent health and social care regulator) and other partners to provide a wide range of choice and a high standard of care. With so many care services available, it can be difficult to decide which is right for you. The answer is Gateway to Care. This single point of contact is here to give you clear information and guidance about the health and social care services available in Calderdale. Simply contact Gateway to Care during office hours when a trained professional advisor will be pleased to talk you through the options and answer your questions.

NHS Continuing Healthcare is available from the NHS for people who are not in hospital but have ongoing healthcare needs. You can receive this service in any setting, including in your own home or a care home. There is no charge for NHS continuing healthcare.

NHS Continuing Healthcare is not dependent on a particular disease, diagnosis or condition, nor

The Support and Independence Team can help you adapt to life back at home when you leave hospital after an operation or illness. You might be referred by a Social Worker or District Nurse if you need support at home after an illness or physical injury.

The Support and Independence Team can help you to regain independence in your own home by supporting you to re-learn lost skills or find new ways of doing things, instead of relying on other people to do things for you.

For example, you might want to focus on improving your memory, re-learning old skills, doing your own shopping and cooking or feeling safe in your own home. If you need help with taking medication when you come out of hospital, we can offer you automatic medication reminders or dispensers which really make a difference in allowing you to stay independent.

This is just one example of a whole range of services that can assist you in keeping your independence. We can involve your carer or family as well so that they can continue to help you recover.

The service can last up to six weeks, during which time you will become more independent as you make progress.

If you are finding it difficult to manage at home, family and friends may be able to help. If this is not possible you could consider using the services of a home care provider who employs care assistants to provide help in your own home. They can assist with personal care such as bathing, washing and dressing as well as practical support such as shopping, laundry and cooking meals. They will be trained in personal care and safety procedures, moving and handling and hygiene. Some home care providers employ registered nurses.

All home care providers are registered with the Care Quality Commission (CQC) which inspects services based on its standards for quality and

Shared LivesThe Shared Lives scheme is for older people, adults with learning disabilities or mental health needs and their families by providing a helping hand. People who need a bit of extra help can do the things they enjoy through short breaks, long term care and daytime support. Shared Lives carers provide this help in their own home or in the homes of the people being cared for. The care provided will be tailored to your individual circumstances and assessed needs.

To take advantage of the scheme, your needs will be assessed by the Council. Your assessment will include any special requirements you have such as personal care needs, dietary requirements, religious and cultural needs and preferred method of communication. You can telephone Gateway to Care on 0845 11 11 103 for further information.

Careline Community alarm serviceThis is a care alarm service offered via Calderdale Council. It allows you to summon help in an emergency, by pressing an alarm button carried on a pendant around your neck or on a wristband. This will raise an alert with the 24 hour response centre. The care alarm can be attached to a range of telecare detectors, sensors and alarms to help maintain independence. If you would like to know more, please contact Gateway to Care on 0845 11 11 103.

Frozen meals service Gateway to Care can arrange for meals to be delivered direct to you following a short telephone

Personal Budget You may wish to arrange your own care at home. Following an assessment you can ask your care manager about using a Personal Budget to help pay for your care (see page 6). Your assessment will show what help you need – and then you can decide whether you would prefer to arrange the services for yourself. This can include employing a personal assistant to help you around the house or support you to do the things you enjoy.

assessment. This service costs £3.25 for a two course meal (April 2013). You can choose from a number of meals. Frozen meals are delivered to your home fortnightly or monthly. The meals are then reheated in a conventional oven, microwave or steamer. Many diets are catered for, including diabetic, vegetarian, low fat, low salt, low carbohydrate, gluten free, soft diet, Asian vegetarian, Halal, Kosher and Afro Caribbean.

Occupational therapyIf you are an adult with a physical disability, a Community Occupational Therapist can assess your situation at home and help you to overcome any difficulties you may be having with day-to-day life. These might include getting in and out of the house, getting up and down stairs, using the bathroom or taking a bath.

We have specialist knowledge of equipment, alternative ways of doing daily tasks and ways to adapt your home. If we recommend specialised equipment, we can arrange for you to try it out at home free of charge.

You can contact Gateway to Care to ask to have your care needs assessed and to be referred to the Community Occupational Therapy Assessment Service. Assessments are carried out in your own home. For those who are able and wanting to fund their own equipment or adaptations, advice and information is available. For more information, contact Gateway to Care on 0845 11 11 103.

An assessment is just a discussion about your needs, considering you as an individual and focusing on what you want to achieve. Whatever your financial circumstances, the Council can offer you a free assessment of your care needs. We will encourage and assist you to get involved with your assessment, which means discussing with you how we can best meet your needs. We will take into consideration the things that you enjoy doing, your likes and dislikes and the people you love, as well as your abilities and care needs.

The Care Quality Commission (CQC) checks and regulates all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

All care providers must be registered with the CQC which assesses adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety.

The CQC holds and publishes up to date information about the results of their inspections.

Following an inspection, each care home, care

Self-Directed Support aims to replace the traditional system of social care, by putting people in control of the care they receive so that you can live the life you choose. It recognises that some people can manage their support on their own whilst others may need help – from family, friends or paid carers.

A Personal Budget is an allocation of funding given after an assessment. You can have your Personal Budget paid directly into a bank account (a direct payment), or have the money paid directly to a provider as an individual service fund or let your local authority use it to arrange the services for you. Alternatively, a combination of a number of methods is possible.

You may have to make a contribution towards your Personal Budget. To make sure that this is as accurate as possible we will arrange for someone from the Council’s Fairer Charging Team to visit you at home to offer you a benefits check and financial

We will look at your financial circumstances and decide how much money you will be expected to contribute to any care you receive. For more advice on paying for care, please see below. who has social care needs (including carers) to purchase care.

The payments are given to people after an assessment and are provided instead of community care services. The payment will be enough to enable you to purchase services to meet your needs and must be spent on services you need. The Council’s Support Planning Team will help you decide what to spend the money on. People receiving Direct Payments take on the responsibilities of an employer, as they are commissioning their own care and support. For some people there are other options where they are supported to manage their direct payment through an Individual Service Fund. This is where the Personal Budget is paid directly to a provider of their choice who will take on the

If you have capital or savings of more than £23,250 (April 2013) you will have to pay for any care services you require but you are still entitled to an assessment of your needs by your local authority.

If your capital and savings amount to less than £14,250 (April 2013), your contribution is based only on your income and the local authority meets the shortfall, subject to local funding limits. Telecare equipment can also be used in conjunction with the Careline alarm service so that some types of equipment can be set up to call, or send an automatic alert to, the 24 hour response centre.
