    The College has achieved much in the past 12 months. There have been many developments in family medicine and the College has been in the thick of them, representing the interests of the family physicians and GPs. Our engagement has been wide-ranging; from the fee benchmarking committee, to the NEHR debate, to the training of our FPs, to the Review Workgroup for SMC disciplinary process and informed consent taking standards. Our College representatives have been very busy in these committees, all with the aim of advancing family medicine. I am pleased to report that the 26th Council has done very well and its work is well reported in the pages below.

    Membership The membership of the College as of 31 March 2019 now stands at 2402. This compares with a membership of 1368 in 2009; over the past 10 years our membership has almost doubled! The growth of our membership augurs well for our vision to better represent our family medicine and primary care fraternity.

    Assets Financially, the College continues to be healthy through the prudent fiscal stewardship by the Council, and this gives us the stability and security to continue the mission of our College. Although we continue to generate a healthy surplus, we need to be constantly mindful of the need for fiscal prudence. The College is a not-for-profit organisation and does not receive any government funding, hence we must continue our good financial management to garner the resources to advance family medicine.

    Courses conductedThe GDFM, MMed(FM) College Programme and Fellowship of the College of Family Physicians Singapore (By Assessment) (FCFPS) continue to do well. The curriculum and delivery of these programmes are being constantly reviewed and improved over the years. The enhanced curriculum for the GDFM to cater the needs of our ageing population has also been very well received. This academic year, for GDFM we have an enrolment of 222 doctors, for MMed 41 doctors and for Fellowship 17. We are encouraged by this and are glad the interest in family medicine as a career continues to be strong.

    PartnershipsOur College continues to work closely with our sister professional bodies, the Singapore Medical Association and the Academy of Medicine Singapore. Most recently the three bodies were called to be part of the Review Workgroup for SMC disciplinary process and informed consent taking standards. By working together, we strengthen the ties and collaborations between specialities and settings, and this bodes well for the unity of the profession.

    The College also enjoys a good working relationship with the Ministry of Health and the Singapore Medical Council. We work closely with the various divisions of the Ministry of Health (MOH), such as the Primary and Community Care Division, the Agency for Care Effectiveness and the Agency for Integrated Care, and this has allowed for MOH programmes and policies to be better tailored for our doctors in the community.

    The College has also collaborated in CME events with our specialist colleagues from various institutions and organisations such as the Nephrology Society of Singapore, the National Centre for Infectious Diseases, the Department of TB Control and the Workplace Health and Safety Division of the Ministry of Manpower. The increasing recognition of the important role of family medicine in the health of our nation is timely. We are very proud of our family physicians who have represented the College, speaking at these talks and symposia from a family medicine perspective.

    Major DevelopmentsAs medical education evolves around the world, we in Singapore must evolve along with it. Last year, the College sent a delegation to GPTEC 2018, a GP training conference held in Adelaide Australia. We were well received by our hosts, and were able to learn much about the Australian training methods. We also examined systems being piloted by MOH, and also systems from Ireland, carefully assessing the various options to determine which would be the best fit for our requirements. After careful deliberation, our College has signed a memorandum of understanding with iheed from Ireland, and will be running a pilot of their learning management system for our Masters programme in the coming academic intake. The pilot will see the introduction of EPAs [entrustable professional activities] and programmatic assessment into our training programmes. Our ultimate aim is to unify our training programmes along a single training track.

    We are now at the end of the term of the 26th Council. Our dedicated Council Members have given much of themselves and worked tirelessly for you and the profession! The College academic faculty have also served selflessly in our training programmes. Their commitment and dedication to their craft as educators means that we can mentor and pass on the skills and knowledge to the next generation of family physicians. A huge word of thanks to you all!

    All of this could not have been possible but for our very hard working and efficient secretariat. Their dedication to the task at hand has been commendable, and we are very grateful and glad for their service. Thank you, Jennifer, Anica, Patricia, Joel, Qiyin, Guorong, Chester, Peggy, Jin Fen and Emily!

    It has been my greatest privilege and honour to serve the College and the fraternity. It is by your continued support and mandate that we are able to continue the good work of the College.

    Sreenivasan Orator 2018

    The Sreenivasan Orator for 2018 was Prof Michael Kidd, Professor & Chair, Department of Family & Community Medicine,

    University of Toronto.

    Koh Eng Kheng Gold Medal

    The Koh Eng Kheng Gold Medal was established under the kind donation of the late Dr Koh Eng Kheng’s family. The Medal

    would be awarded to the Best Fellowship Candidate of each academic year.

    The recipient of the Koh Eng Kheng Gold Medal for 2018 was Dr Xu Bang Yu.

    CONTINUING PROFESSIONAL DEVELOPMENT/ MEDICAL EDUCATION PROGRAMMES AND COURSES

    Family Practice Skills Course (FPSC)

    The College promotes Continuing Professional Development (CPD) and Continuing Medical Education (CME) through the Family Medicine Modular Course, Family Practice Skills Course, MMed (Family Medicine) College Programme, Family Medicine Membership and Fellowship Programme.

    The Institute of Family Medicine (IFM), an organ within the College, plays a significant role in developing the contents and syllabi of these programmes and courses.

    During the year the IFM reviewed, updated and edited the notes as well as their e-learning components for Modules 8, 1, 2 and 3 of the Family Medicine Modular Course to make them more relevant and user-friendly to the course participants.

    The Family Medicine Modular Course consists of 8 modules of knowledge acquisition and is a requirement for doctors who are preparing for the Graduate Diploma in Family Medicine (GDFM) Programme and the Master of Medicine (Family Medicine) College Programme. Each year, four modules are covered.

    For the year April 2018 - March 2019, there were 247 trainees registered in the GDFM course, 20 trainees in the MMed(FM) College Programme and 19 trainees in the Fellowship [FCFP(S)] programme.

    During this financial year, the College conducted seven Family Practice Skills Courses.

    E-LearningThe E-Learning programme launched in 2003 has been incorporated into the GDFM training programme. An E-Learning component for each module was introduced, consisting of therapeutic notes, case scenarios, assessment by MCQs and a case review to be put up by a tutorial group.

    Accreditation of core CME pointsThe College is represented by Adj Asst Prof Tan Tze Lee in the Singapore Medical Council (SMC) CME Co-ordinating Commit-tee. The College works closely with the SMC to ensure that the quality of the CME events is acceptable. It continues to advise SMC on the accreditation of core Family Medicine CME events through its panel of Family Medicine (FM) CME advisors appointed by the College.

    Commencement of Academic Year 2018

    2018 had been a smooth year for the College Mirror (CM) as we have successfully implemented 4 sub-editor teams and covered much of the big news in our Family Medicine fraternity.

    SIGNIFICANT COLLEGE EVENTS

    The Commencement Ceremony was held on 28 July 2018 at Singapore Chinese Cultural Centre (SCCC).

    There were 247 trainees in the GDFM course, 20 trainees in the MMed(FM) College Programme and 19 trainees in the FM Fellowship [FCFP(S)] Programme.

    Annual General Meeting 2018The College AGM was held on 28 July 2018 at Singapore Chinese Cultural Centre (SCCC). The minutes have been circulated.

    Convocation 2018The Family Medicine Convocation Ceremony was held on 17 November 2018 at Capella Singapore and the Guest-of-Honour was A/Prof Benjamin Ong, Director of Medical Services, Ministry of Health.

    30 doctors were conferred the MCFP(S). Of which, 12 were awarded the MCFP(S) by Election on 4 January 2018, 16 were awarded the MCFP(S) by Election on 17 November 2018, 1 was awarded the MCFP(S) by Assessment on 7 February 2019 and 7 March 2019 respectively. 134 GDFM graduates and 63 MMed(FM) graduates were recognised.

    Family Medicine Review Course 2018The Family Medicine Review Course 2018 was held on 19 May 2018 at the Health Promotion Board (HPB).

    World Family Doctor Day 2018The World Family Doctor Day Dinner 2018 was held on 19 May 2018 at the Park Royal on Pickering.

    COMMENCEMENT OF THE ACADEMIC YEARThe Commencement of the academic year for 2019 will take place at the Academia, SGH on 27 July 2019.

    GRADUATE DIPLOMA OF FAMILY MEDICINE (GDFM)

    The Graduate Diploma in Family Medicine (GDFM) is a vocational training certification for primary care doctors. The aim of this 2-year part-time trainee programme is to train primary care doctors to practise Family Medicine at an enhanced level to meet the needs of the child, the adolescent, the adult and the elderly. The course emphasises on basic clinical, diagnostic and management skills essential to general practice.

    The programme is planned to accommodate the busy doctor’s schedule as almost all courses that require in-person attendance are conducted outside regular office hours. The course consists of the following: • 8 modules of Family Medicine Modular Course (FMMC) with each comprising of 4 workshops, 1 tutorial, online case studies and multiple choice assessments • 3 Practice Management Courses and 1 elective Family Practice Skills Course

    The examination is conducted by Division of Graduate Medical Studies (DGMS), National University of Singapore (NUS). The standard set is a pass in all three segments:• Written paper I - Applied Knowledge Test: 100 MCQ questions (2 hours)• Written Paper II - Key Features Problems (KFP): 10 questions (1 hour)• Skills Assessment in the Objective Structured Clinical Examination (OSCE) - 10 stations of 11 minutes each, comprising 3 minutes for reading and 8 minutes for assessment (2 hours)

    The GDFM(NUS) is a title or qualification approved by the Singapore Medical Council for display by Registered Medical Practitioners.

    The 17th GDFM Examination was held on 29 June 2018 for the written papers and 7 & 14 July 2018 for the OSCE. A total of 134 of the 182 candidates passed, giving a passing rate of 73% for the GDFM Examination 2018.

    MASTER OF MEDICINE (FAMILY MEDICINE) [MMed(FM)]

    There are currently two programmes to prepare doctors for the MMed(FM) examinations.

    Family Medicine Residency ProgrammeThe Family Medicine Residency Programme aims to equip the resident with educational and clinical experiences in accor-dance with ACGME-I standards, which focuses on structured training and close supervision. The resident will undergo clinical rotations in the hospitals and polyclinics, call-back sessions and teaching sessions that combine foundation teaching, continuity clinics, skills workshops/attachments and community medicine.

    MMed(FM) College Programme The programme is a structured training programme designed for mature practising family physicians in Singapore. The programme is jointly organised by the College of Family Physicians Singapore and the DGMS, NUS. This 16-month part-time course provides a comprehensive and structured training programme for doctors working in primary care who have completed the 8 modules of the Family Medicine Modular Course (FMMC) to prepare them to sit for the MMed(FM) exam-inations.

    The course participants from both the Programmes sit for the same examination – namely, the MMed(FM) examination conducted by the DGMS, NUS.

    This examination consists of:• Written 250 multiple-choice questions • Clinical • Logbook-based viva assessment • Slides-based short answer questions • Clinical Skills Assessment Stations

    The Fellowship Award by assessment is the highest qualification for family physicians in Singapore. This programme was started in 1998. Potential candidates for this award must successfully complete a demanding two year assessment programme and an exit viva voce examination.

    The criteria for the award were as follows:

    The Family Practice Skills Courses (FPSC) continue to serve the continuing professional development needs of the General Practitioner/ Family Physician. Each skills course includes one or more of the following components in which the family physician can choose to participate in:

    • Distance Learning with verification (6 CME points upon attaining minimum pass grade of 60% in online MCQ Assessment) • Seminars (2 CME points per seminar) • Workshops (1 CME point per workshop) • Small group activities including clinical attachments, tutorials, ward-rounds (for some courses)• Assessments which can be written, oral, demonstration and/or clinical (for some courses)

    The IFM is encouraged by the big increase in the number of doctors enrolled in the GDFM modular courses year on year. The Family Practice Skills Courses also saw an increase in some of the courses.

    The enrolment numbers in the 2018 intake was 247 compared to 236 in the 2017 intake. The 4 Family Practice Skills Courses had attendances ranging from 122 to 225 in the seminars and 110 to 220 in the workshop; the number of doctors who did the CME reading and passed the MCQs ranged from 186 to 516. Newer topics attracted bigger attendances e.g., the Extended Consultation, and Updates in Rheumatology.

    We look forward to another year of serving our members.

    PurposeTo provide guidance and oversight on financial and governance activities in the Secretariat and the various committees.

    Duties and Responsibilitiesa) To review financial reports from the ED and Secretariat staff.

    b) To review budgets prepared by Secretariat staff and the various committees.

    c) To ensure regular and accurate monitoring and accountability for funds, and report to the Council on any financial irregularities and concerns.

    d) To make financial recommendations to the Council, and to provide feedback on any financial matter specifically designated by the Council.

    e) To oversee short and long-term investments of the College.

    f) To advise the ED and other appropriate staff on financial priorities and information systems, depending on committee member expertise.

    Observations and Decisions by the Committee for the Financial Period under ReviewThe committee advised the secretariat on the fixed deposit placements in terms of the selection of the tenure and the financial institution. Fixed deposits were placed with: • Bank of China • HL Bank

    The committee, upon reviewing the Governance Evaluation Checklist of the Charity Portal, refined the current procedures for handling Conflicts of Interest. The framework for Sponsorship of Overseas Travel was also refined. Both these documents were discussed and approved by the council.

    To provide guidance and oversight on administrative and human resource processes in the Secretariat and the various committees.

    Report for the period under review

    1. We are pleased to report that there were no staff resignations for the last financial year, and would also like to take this opportunity to welcome Ms Peggy Poh to our secretariat team.

    2. The medical education programmes and training activities undertaken by College remain strong and a core function. The enhancements made to both the GDFM and MMed(FM) programmes are working well and to better the needs of the trainees’ future. This has led to an increased work load for the secretariat.

    3. Thus the committee will be looking into increasing the manpower of the secretariat to strengthen the team to meet these challenges and to carry out its mission better and more effectively in the future.

    In the financial year from 1 April 2018 to 31 March 2019, the Group derived its income mainly from course and seminar fees, sponsorships and subscriptions. The 3 items together accounted for 85.8% of its total income.

    Fees from courses and seminars accounted for 67% of the total Group income, sponsorships accounted for 8.5% of the total income and subscriptions accounted for 10.3% of the total income.

    In the financial year from 1 April 2018 to 31 March 2019, the Group’s consolidated total income was $2,948,682; the consolidated total expenditure for the same period was $1,827,940. The consolidated net surplus for the financial year from 1 April 2018 to 31 March 2019 was $1,108,799.

    I am pleased to report that the Group continues to be on a firm financial standing for the financial year 1 April 2018 to 31 March 2019.
