Bristol Community Health Role Profile

Job Title: End of Life Care Co-ordinatorSalary Band 6 - £25,528-£34,189Reports to: Service Manager- PCHSJob Purpose: To Coordinate care for patients at end of life stage, working in

partnership with St Peters Hospice at Home Service on a ‘test and learn’ basis.

Key Responsibilities:

Provides a palliative care nursing assessment which is individualised and holistic and to plan for a package of care via the Coordination Service.

Coordinate care for the EoL Coordination Service and the PCHS service, taking the lead for resourcing care as required.

Works collaboratively with patients, their key worker, their families, carers, other services and agencies. Meets agreed goals to plan complex packages of care, as a key co-ordinator for the service.

Supports patients, their families, carers, team members and other agencies involved in service provision in response to and in anticipation of changing needs as the illness progresses.

Frequently manages extremely distressing situations including the emotionally challenging care of dying patients, bereaved family members and carers.

Liaises effectively with the key worker and with a range of services in order to influence any change required in current prescription for patient in response to changing symptoms.

Works closely with the service manager to ensure service continuity and appropriate levels of staff supervision.

Demonstrates leadership to the health care assistant team in the delivery of a high standard of care for both patients and their family members.

Responsible for forward planning of workload, staffing levels and staff development.

Influences policy developments and changes as appropriate across the area covered by Bristol Community Health. Takes a lead role in working collaboratively with patients, carers and relevant stakeholders i.e. GPs, secondary care, commissioners, adult community care and voluntary agencies.

Initiates and participates in training programmes for qualified and unqualified staff, including other health care professionals, statutory and non-statutory bodies and volunteers.

Initiates and participates in induction and orientation of new staff. Participates in working groups involved in research programmes and

clinical audits. Takes prompt and appropriate action in receipt of complaints as per

Bristol Community Health policy, and participates in official investigations where required.

Records and reports all clinical and non-clinical accidents and incidents according to Trust policy, taking appropriate action.

Responsible for Health and Safety within the work environment, formulating safe systems of work and carrying out risk assessments in conjunction with service manager/End of Life Lead.

To coordinate the provision of care and/or equipment and facilitate access to support services, allowing those with a life limiting illness to achieve a good death.

Skills Specification:

Able to work autonomously and as part of a team, Able to work without supervision to achieve the objectives of the

post. Able to prioritise tasks according to changing needs and service

capacity. Able to communicate clearly, succinctly and sensitively using

appropriate method and means, to include personal computer, telephone and mobile phone (for text and voice).

Able to present complex information clearly at meetings/ teaching sessions as required.

Able to build and maintain effective working relationships with a range of key services and organisations within the NHS, social services and the voluntary sector across the area served

Experience: Experience of supervising staff and co-ordinating patient care within a clinical team setting

Experience of managing a pay or non-pay budget Experience of carrying out regular staff reviews or appraisals,

including working with staff to identify development needs. Experience of working across organisational boundaries within

health, social care and the voluntary sector. Experience of working as part of a team, including that of leading a

team. Experience of implementing change in own organisation and

influencing change across organisational boundariesKnowledge and qualifications:

Level 1 Nurse, RGN, with an active NMC PIN. Evidence of continuing professional development. Evidence of study in palliative care. An understanding of the role of this service (Palliative Care Home

Support) within the wider context of palliative care service provision involving NHS, voluntary sector and other health and social care service providers.

An understanding of the range of factors which affect accessibility and appropriateness of service provision, and steps that need to be taken to provide appropriate care to all groups
