Please be advised that a demonstration and fitting by official BLOCH representative will take place during the Program. PREPARING FOR PARTICIPATION IN THE BBASI NY PROGRAM

Successful participation in the BBASI program is a collaborative process that begins in advance at home. The Participant and Parents/Guardians (if Participant is younger than 18yrs) should carefully review, know and follow the guidelines, rules and regulations stated in the Registration Contract, Program Manual and any/all other Program Forms and Materials (Forms & Materials). Reading all in entirety is a requirement of the BBASI Program. Please also discuss with your Participant any individual suggestions and guidelines you have as a family for his/her stay away from home.1

All participants attending the New York Program are expected to be mature, focused, and able to function independently and responsibly at all times.

I. BEING A RESPECTFUL MEMBER OF THE BBASI COMMUNITY

BBASI is a co-ed community and is comprised of hard-working dancers from diverse backgrounds, cultures and countries, and teachers/staff who work tirelessly to make this experience the most valuable it can be. Sensitivity to the rights, property and needs of others is expected from everyone. Some of the general guidelines to ensure a positive environment for all are:

Appropriate and respectful language and behavior is expected at all times. All are here to train hard, have fun and support one another. Bad attitudes and/or behavior will not be tolerated.

Please be sensitive to the fact that some participants may not speak English and come from different cultures/countries.

Every Participant is expected to attend all ballet classes and arrive on time. For the NY Program that is the ONLY mandatory activity.

Every Participant is expected to dress tastefully and appropriately at all times. If there is an issue/problem/emergency at any time, advise BBASI Staff immediately. Do not attempt

to resolve any material situations by yourself. Cell phone/electronics use is prohibited during class, program-organized activities, and after Lights

Out (for Residents at Yeshiva University). All are expected to respect the needs, wishes, privacy and property of others at all times.

II. BBASI LOCATION

New York City is one of the most exciting cities in the world. However, regular awareness, common sense, and caution are expected from participants at all times. BBASI studios are located in Manhattan’s Lincoln Center area on the beautiful Upper West Side.2 Two official housing options are offered for participants needing housing for the duration of their Program enrollment.3 Yeshiva University Beren Residency (YU Residence) is a chaperoned/curfew facility used exclusively by the Program for its participants ages 15-17 years. The Marymount Manhattan College Residence Hall (MMC Residence) is an unchaperoned/non-curfew facility offered to older participants. The residences are a subway ride to the BBASI studios. No meal plan is provided; participants have access to Juilliard Cafeteria conveniently located just minutes away from BBASI studios. For full details on housing options please refer to BBASI Housing section on Page 6. Please note that BBASI cannot ensure an allergen-free environment at any of its facilities.

Supervision

While RAF takes great care to ensure the safety and wellbeing of participants during the Program, our ability to do so depends on serious advance preparation of participants and their families. Program’s schedule outside of ballet classes is open and unchaperoned, giving participants the ability to independently and responsibly take full advantage of everything New York City has to offer. Program Staff and/or volunteer Parent Chaperons will be available to:

Oversee general supervision while classes are in session. Attend Program’s organized activities for general group supervision. Accompany participants for medical visits, if such need arises. Attend to individual cases of emergency, if such need arises. YU Residence

o Group supervision of participants - walk/subway - to and from class/ YU Residence/ dining hall. It is Participant’s responsibility to stay with the group when meeting times/ places are designated.

o Attend to order, curfew, and general supervision. o Assist participants if they are sick/injured.

MMC Residence o No Program staff supervision is offered.

Please be aware that Program Staff / Chaperons will NOT be available for individual supervision, except medical emergencies and/or visits with medical professionals. It is each Participant’s responsibility to be in full compliance with BBASI Program and Residence rules and regulations at all times.

Navigating New York City

While NYC has been consistently ranked as one of the safest cities in the world, a working knowledge of daily city life will enable a Participant to maximize his/her time in this great city in a safe and successful manner. In preparation for your time in NYC, and in addition to your own individual efforts, Program recommends reviewing provided links for useful tips and suggestions.4 Please also note some general guidelines participants should adhere to at all times: Travel in groups of three or more and carry Program badge with emergency numbers.

Be aware of your surroundings at all times and conduct yourself with confidence and control.

If uncomfortable about where you are, or are suspicious of someone near you, leave or stay away.

Take the well-lit and well-traveled paths; avoid shortcuts through dark or isolated areas.

Be aware of your valuables and belongings.

We are pleased to continue our dedication to BBASI Program being in the most premier location within NYC. Program’s studio locations are within walking distance of each other in the Lincoln Center Area.

BBASI Housing6

Yeshiva University Beren Campus Residence (15 yrs – 17 yrs) The YU Residence is centrally located in a residential neighborhood of Midtown East Side at 151 East 36 Street. It is an inviting, modern facility which consists of air-conditioned rooms, two community lounges with TVs, a private outdoor garden and a laundry room. Each floor has common bathroom/shower area. The building has 24-hour on-site security post, but no elevators. Most rooms are single, with a few doubles. All rooms lock individually. The facility will be occupied exclusively by BBASI Program and will serve as its flagship home. The Residence is within a few blocks of a major movie theater, shopping, restaurants, supermarkets and subway stations. Each room typically comes equipped with:

A bed (residents must provide their own bedding) Small refrigerator & microwave Desk, chair, small dresser, closet, sink Wireless internet Bedding is NOT provided

Program’s Sr. Staff and volunteer parent chaperons will reside at the YU Residence and will be available for group supervision, adherence to YS Residence and Program Rules, general guidance and assistance with emergencies. Male and female participants may be housed on same floors, with separate common bathroom facilities. Participants are expected to respect the division and the privacy of others at all times. Curfew, Lights Out, no visitors policy and any/all other Program and YU policies will be strictly enforced.

Marymount Manhattan College 55th Street Residence (18 yrs and older7) The MMC Residence is the unchaperoned/non-curfew BBASI housing option. This modern facility is located in Midtown East Side at 231 East 55th Street, a short subway ride to the BBASI studios. The residence is suite-style, with each suite consisting of 2 bedrooms (doubles, triples or quads), bathroom (number depending on size of the suite), kitchen, and a living room space. Rooms are furnished with twin beds, dressers, desks and wardrobes. Kitchen/living area has table and chairs, full size refrigerator, electric stove and microwave. Bedding and kitchen supplies are NOT provided. Amenities include elevators, common lounges on multiple floors and a laundry room (other building amenities will not be applicable for BBASI participants). The building has a 24-hour security check point with ID card swipe entry. The Residence is within a few blocks of shopping,

restaurants, supermarkets and subway stations. All participants selecting MMC Residence will be required to comply with No Guest Policy and any/all other MMC & BBASI rules and regulations. This Residency option is offered to BBASI’s older participants 18 years or older. Participants of 17 years can request this option, subject to availability, with parental/guardian’s consent. Participants are expected to respect the division and the privacy of others at all times. There are numerous common areas for group activities. General - No visitors policy and any/all other BBASI and Residency policies will be strictly enforced. - All resident participants will be required to sign a Residency Agreement which details relevant policies and expectations in full. - For roommate preferences please complete BBASI Dashboard > “Forms” Tab > Roommate Preference Web Form.

Respect for Property of others

Individual and all common areas (including studios and residency dorms) must be kept in a clean, safe and undamaged condition at all times. All are expected to treat Program’s facilities and the property of others with respect and care, and to be responsible in caring for her/his own property. Breakage, damage, or other repair needs should be reported to Program Staff immediately. A Participant will be held responsible for any damage to property which occurs as a result of his/her action.

Visiting and Temporary Residence Departure Policy

To ensure the safety of all participants, NO GUESTS/VISITORS are permitted at any time in any of Program’s facilities, other than during scheduled visiting days. For additional details about visiting days, please review Section VIII. VISITING DAYS on page 14. BBASI understands that many participants may have friends and family in NYC. In cases of participants residing at YU Residence, BBASI will honor requests by parents/guardians for Temporary Overnight Departures from the Residence. A written request must be submitted to the Coordinating Director with reasonable advanced notice. To complete a request, for those residing at YU Residence, please access a link to the Google Form by clicking HERE. Dancers living in MMC facility are not chaperoned, and hence, the Temporary Residence Departure Policy does not apply.

III. HEALTHY LIFESTYLE

BBASI NY is dedicated to a healthy training environment and we insist on all Participants: Eating, at a minimum, 3 healthy meals daily Staying hydrated Getting sufficient sleep

Meals Juilliard Dining Hall: The Juilliard School Café is located just a few blocks walking distance from the BBASI Studios at 70 Lincoln Center in the Rose Building, 3rd Floor. Participants may purchase meals for breakfast, lunch, and dinner as they please. Meals range from $5.00-$10.00. For more information please click here. Residences:

The Yeshiva University/ Beren Campus Residence does not have a kitchen; each room will be equipped with a microwave and a small fridge.

The Marymount Manhattan College Residence has a kitchen in each of the suites with full size refrigerator, electric stove and microwave.

There is also a wide variety of supermarkets, cafés, delis in the areas of Program facilities. Please refer to a partial/suggested list on your BBASI Dashboard > “Forms” Tab > Local Stores/Restaurants informational form.

Allergies

BBASI cannot ensure a nut, gluten, latex or any other allergen free environment. Participants are required to be aware of their own eating requirements and/or other restrictions, and be responsible enough to follow the protocol developed prior to arrival at the Program. Any allergy related concerns can be discussed in advance with the Coordinating Director and must be clearly noted in the Program’s medical forms accessible on the “Forms” tab of your BBASI Dashboard.

Special Situations

BBASI is dedicated to the belief that in order to be a healthy and successful dancer, one must be a healthy eater, stay hydrated, have substantial body weight and strength, and must get sufficient rest. As such, we do monitor the general well-being of all Participants. If, at any time, BBASI has any concerns about a Participant’s weight and/or ability to participate in class, BBASI reserves the right to require that the Participant is confidentially evaluated by the medical staff. For clarity of all, BBASI adheres to medically accepted BMI for determining safe, healthy and age appropriate weight levels. In the rare cases that the Participant may be determined to have a BMI level outside of healthy levels, BBASI, as in all medical situations, will be guided exclusively by instructions of its Program Director.

IV. TRAINING AS AN EDUCATED AND HEALTHY DANCER BBASI NY Participants come from various training backgrounds. Class training becomes progressively more difficult as the Teacher assesses the capability of participants and works to achieve appropriate progress. A strong emphasis will be placed on proper placement, technique and overall strength development. This may result in dancers feeling soreness and aches. It is of utmost importance that a Participant adheres to the following guidelines at all times:

A dancer knows her/his body best and must, at all times, self-moderate the training. Immediately advise the Teacher and/or Program staff of any discomfort/pain.8 Delay in notification

may aggravate an injury. In most cases, the soreness/ache may just require a dancer to slow down a little and/or take a short

break. Academy’s Teachers/Program Staff will always be mindful of such situations and will work with the Participant so that they do not feel left out.

Class Placement

At the start of each session, all participants are placed in groups based on the audition results. However, during the first 3 days of class, all participants are evaluated by the Academy’s Teachers and adjustments in placements may be made. BBASI Staff make their best effort to make the adjustments, if needed, in the most non-disruptive manner for the Participant. It is important that both the Participant and parents/guardians understand that such changes are made strictly to ensure the most level appropriate and injury free training.

Pain Identification / Communication / Action Procedures

Just as with sports training, discomfort, aches, pains, and injuries are a known occurrence in ballet training. As such, in collaboration with the NYU Hospital for Joint Diseases Harkness Center for Dance Injuries, BBASI Programs have designed an approach to better educate and equip BBASI participants for managing their training. It is of utmost importance that parent(s)/guardian(s) and Participants jointly review, know and follow the information presented below. Participants should report discomfort/pain when it occurs. This will enable BBASI staff to offer support

and to engage in the proper course of action. Delay in reporting an injury/pain may aggravate it. BBASI’s course of action will be a direct response to Participant’s identification of the pain as per the

UNIVERSAL PAIN ASSESSMENT TOOL9. Parent/Guardian must ensure that the Participant is familiar with the UNIVERSAL PAIN ASSESSMENT TOOL below, as well as BBASI’s stated response actions.10

If a class break is taken, Participant will be responsible for individual warm up prior to resuming class

5-6 Participant will be recommended to see a medical professional for evaluation. Parents will be notified.

If a medical professional is seen, completed Training Modification Form will be required to resume class.

Post visit, follow up treatment management and protocol will be the responsibility of Participant.

7-8 Participant will be required to see a medical professional for observation and evaluation. Parents will be notified.

9-10 911 will be called. Parents will be notified as soon as reasonably possible.

Slips, Falls, Contact with another Participant

The ballet training process challenges each Participant to learn new ballet elements and work on developing technique. It is a process during which slips, falls and contact with another participant may occur. Please discuss with your Participant (or self if 18 yrs and older) BBASI’s standard procedures and responses to such incidents as outlined below:

SLIP, FALL, CONTACT WITH ANOTHER PARTICIPANT

BBASI ACTIONS (Ages 15+)

Participant gets up/ continues activity without any assistance

A) BBASI Teachers feel strongly that highlighting such incidents during class brings emotional discomfort to those involved. As such, these type of incidents are ignored and the class continues without any attention by any staff. B) Participant always has the option to approach staff for any assistance.

Participant has difficulty getting up/continuing activity without

assistance

BBASI Staff will be called and protocol stated in Pain Assessment Tool will be followed.*

Head impact/injury occurred during the incident

A) Participant must notify Program Staff immediately if any incident, at any time, involves impact to the head.

B) If no visible/stated signs of impairment, Participant will be asked to notify Parent/Guardian and follow-up actions will be determined by instructions from the call. Suggested protocol to the Parent/Guardian will be to cease activity until Participant is cleared by a healthcare professional trained in concussion management.

C) If visible/stated signs of impairment, Participant will be taken to emergency room.

* Any fall/whiplash/contact could have potential for

concussive injury.

Any symptoms, post such incident, involving: headache, dizziness, fogginess, nausea should be immediately brought to attention of staff and Participant will be taken to nurse or appropriate medical specialist for evaluation.

HARKNESS INJURY-PREVENTION WORKSHOP BBASI is pleased to offer its participants a free injury-prevention workshop which will be conducted by the Harkness Center for Dance Injuries at LaGuardia School exclusively for BBASI. Special Situations

If at any time there is an issue/incident which is not addressed by stated guidelines above, the Participant should advise the staff immediately. If, at any time, the Participant has a condition which prevents him/her from active participation in class, the Participant is invited to observe the class. Learning from observation is very valuable and will ensure that the Participant remains on-pace with the class/Program. At all times, the Participant will be required to abide by respectful behavior while class is in session.

Supplemental Activities

BBASI NY may offer non-ballet activities. Stated guidelines will be applied to any/all incidents.

Class Dress Code

Every participant is expected to dress according to the dress code in class. Girls: Pink tights, black leotard, hair securely fastened and pulled away from face

o No jewelry (small ear studs allowed) o Pink ballet slippers and pointe shoes, black character shoes o Knee length character skirt

Boys: White T-shirt, black tights, white or black ballet slippers o Black character shoes or boots

V. HEALTHCARE POLICIES AND PROCEDURES

All at BBASI take the health of each Participant very seriously. Please discuss with your Participant that s/he should always and immediately notify Program staff of any material problems or concerns. If the need arises for professional medical assistance, parents/guardians will be notified and the Participant will be accompanied by a Chaperon on the initial visit (applicable fees for the professional medical visit will responsibility of the participant/parent)11. There is no medical staff provided by Program and any medical issues are referred to either a nearby clinic, an appropriate specialist or 911 for emergencies.

Medication (Prescription and Over the Counter)

If a Participant will require medication during the Program, following guidelines must be followed: - Participant must be sufficiently responsible to self-carry and trained to self-administer the medication. - It will be the responsibility of the Participant to have a safekeeping device (we suggest something like a

small bag with a lock/Packsafe, etc.) and to keep the medication away from reach of others. - ANY MEDICATION DESIGNATED A CONTROLLED SUBSTANCE MUST BE REPORTED TO THE PROGRAM in

advance. Special arrangements will need to be discussed to ensure reasonable accommodation of such.12

Program maintains limited First Aid supplies in the studios and the YU Residence. If you would like your Participant to have access to it, please visit your BBASI Dashboard >”Forms” Tab> Consent for Medical Treatment & Administration of Medication Web Form.

Communication

RAF will contact parents/guardians if Participant (18 years or older): Has been consecutively absent from classes for longer than 1 full day Has a medical emergency Needs to visit any medical specialist, such a physician, dentist and/or OPT. (Any such visits will be charged

to parent’s/guardian’s insurance/credit card on file) Parents/Guardians may communicate directly with the Chaperon on routine matters. In cases of non-

routine matters, and to ensure proper Program oversight, please contact the Coordinating Director first.

Lice Prevention

Lice have become a common occurrence in places with large quantities of minors. While they are not harmful, they do cause substantial disruptions to all involved. Each family should thoroughly check their Participant prior to arrival to the Program. Please perform this check at least 3 days before Program's start, as sometimes a treatment, if necessary, may need to be repeated after the initial application. Participants will not be screened by the Program, but your adherence to this policy will contribute to the welfare of all. Please remind your dancer not to share any hair products with others.

Immunization

As required by state law, participants must have age appropriate immunizations and records of such. State laws grant exemptions from vaccination for legitimate medical and religious reasons.13 BBASI complies with state laws and understands that there are legitimate circumstances which require special accommodation of such requests for exemption. If your family qualifies for such an exemption, please contact the Program Director to obtain full details and required steps to determine compliance. If no advance clearance for an exemption is obtained from the Program prior to registration, and a request for exemption is not granted at a later time, Program will adhere to its standard Refund Policy. However, and in the case of RESIDENT FAMILIES ONLY, BBASI also believes that each family should be provided the option of voluntarily agreeing or declining to agree to allow their Participant to reside with an unvaccinated participant, regardless of the reason for such decision. Please refer to Roommate Preference Form on your BBASI Dashboard to confidentially advise BBASI of your preference. To respect privacy of all participants and their Families, BBASI will not disclose the names of participants who have been granted an exemption from vaccination, nor will BBASI disclose the decision of any family regarding allowing their Participant to reside with an unvaccinated Participant.

VI. ZERO TOLERANCE POLICY

The following conduct is subject to a Zero Tolerance Policy and in case of a violation, the Program reserves the right to immediate dismiss from the Program without any refunds for tuition and/or housing. Any dismissed Participant must leave the Program immediately. No smoking (including electronic cigarettes) or use of tobacco of any kind on or near the premises of any

Program facility and/or activity. No consumption or possession of alcoholic beverages/drugs/any other illegal substances at any time. Sexually oriented behavior is prohibited, including public displays of affection. 13

No physical / sexual / emotional abuse, threats, intimidation, commission of any violent act, and/or other conduct which threatens or endangers the health, safety or emotional well-being of another.

No harassment of another person, whether physical, sexual, verbal, written or via internet. No vandalism, defacement of property or facilities. No possession, use and/or display of weapons or harmful materials. Participants are expected to eat 3 healthy and complete meals daily, stay hydrated, and get sufficient rest

and sleep. No disrespect to your fellow participants, faculty, staff, and chaperones. No self-harmful behavior including refusal to eat, causing oneself to be sick, or any other bodily harm. No eating or drinking (other than water) in the studios.

VII. DISCIPLINARY ACTION

BBASI knows that all of its participants are dedicated to maximizing this unique opportunity afforded them by their families and BBASI staff. As such, we would like all to be clear on BBASI’s policy in rare cases that there is a need for disciplinary action. Please note that only the Program Directors / RAF Executives have the authority to confidentially discuss any actions/measures with either the Participant and/or the parent/guardian. Zero Tolerance Policy. If a Participant is found to be in violation of the Zero Tolerance conduct, BBASI

reserves the right to immediately dismiss the Participant from the Program. In such cases, the Parent/Guardian will be asked to make immediate arrangements for the Participant to depart.

Attendance & Lateness. After 3 unexcused absences BBASI reserves the right to dismiss the Participant from the Program. Repetitive lateness will not be tolerated, as it is disruptive to class and other participants.

Electronics. Use of oh phones and electronic devices is prohibited during class, organized activities, and after Lights Out. Any violations will result in the electronics being collected.

Other Program Policies. Intentional and/or repeated non-compliance with Program Policies may lead to disciplinary action and/or dismissal.

BBASI NY understands that with so many participants from different cultures, places, and ages some less serious issues may arise. In such cases, Program Director will discuss the issue with the Participant/s and will notify the parent/guardian in writing. If the issue arises again, the Participant and parent/guardian will be given a formal warning that a repeat will lead to dismissal from the Program.

We are thrilled to say that we have found your dancers to be hard-working, respectful, delightful and truly appreciative of this great experience. We want to ensure this continues. If you have any concerns, whether about your own Participant or someone else, we ask you to notify the Program Directors. Please do not contact counselors/teachers/or any other summer staff.

VIII. VISITING DAYS

In order to ensure an enjoyable experience for all, visitors are kindly asked to observe the following rules: Adherence Program’s Zero Tolerance Policy. Program welcomes families taking photos and videos in a non-disruptive manner and from a fixed

location. Program will also be professionally taping the Final Performance and videos will be available for purchase.

IMPORTANT: Designated visiting days are the only days/ times when the Program participants are allowed to have visitors in Program’s facilities.

BBASI NYC 2016 VISITING DAY AND FINAL PERFORMANCE

Resident Participants Check-Out Procedure (Both Residences):

Participants should check out from their residence on Saturday, July 16 or August 6 between 9AM – 12 PM. If you would like to check out on Friday after classes (July 15 or August 5), you must advise Program Staff in advance in writing. Please make sure that:

1. BBASI Staff surveys the room prior to your departure and confirms that NO damage was done. 2. It is Participant’s responsibility to verify any/all of your travel plans.

Non-Resident Participants Check-out Procedure: Non-resident participants leave on their own at the end of classes. If your Participant is not allowed to leave alone please make sure to arrive at least 5 minutes before the end of the classes.

X. PRIVATE LESSONS

Some participants may want to request supplemental lessons in addition to the regular weekly schedule. Private lessons are available after regular class times and on the weekends for an additional fee. Please note that these classes are in no way mandatory, nor will affect the Teacher-Participant relationship during group instruction. Additional information on how to register for private lessons will be sent separately.

XI. RUSSIAN LANGUAGE CLASSES

In the spirit of RAF's mission to expand the personal, pre-professional, and cultural horizons of youth, the Bolshoi Ballet Academy Summer Intensive will be offering FREE Russian language classes to the participants of the BBASI program. This registration is optional for general participants and mandatory for NSLI-Y Finalists. For more information and to register, please fill out the Google Form HERE. XIII. CAMPSITE – BBASI’S CENTRALIZED INFORMATION PORTAL

Using your BBASI Dashboard, you will have access to all program forms and information. Using CampSite, you will be able to manage several aspects of you Participant’s relationship with BBASI using the various tabs under the BBASI banner:

MY ACCOUNT (upper right hand side) This tab will allow you to add or edit your Participant’s basic information including a headshot, email address, or household contacts. Information like your Participant’s DOB and academic grade level can only be changed on the administrative end – your program contact can edit these fields for you.

ENROLLMENT (Second tab from the left) This tab will allow you to add and view the enrollment options (i.e. registering for an audition or program housing) for your Participant.

FORMS (Third tab from left)

This tab will allow you download, complete, and upload the required forms for your Participant’s attendance in the BBASI NYC Program.

FINANCIAL (Fourth tab from the left) From here, you have 24/7 access to your latest statement and invoice information. Using this tab, you may also safely encrypt a credit card number for BBASI Staff to use in the event of an emergency. You can read more about the privacy/safety policy in your registration contract.

Forms

Under “Medical Forms,” you will note that there are 3 sections that must be filled out in their entirety. Please do note that there are additional medical forms/authorizations required. In the Standard Forms sections, there are several supplemental medical forms/consents. Additionally, a Physician’s Report is required to be filled out by your Participant’s primary licensed physician. Please be advised that the BBASI Medical Director mandates that all medical examinations be done within 6 months of the start of BBASI (June 27th, 2016). Should you have difficulty scheduling an appointment with your primary physician by the May 13th deadline, please inform your program contact. Please note that the physician’s report must be filled out by the Participant’s physician under whom s/he has been in the care of for at least 1 year.

In addition to medical forms, you will note that there are two types of “Standard Forms” that may be completed and submitted in different ways:

Printed forms must be downloaded, completed offline, and then scanned/uploaded back in the "forms" tab of your BBASI Dashboard. All forms are labeled to indicate whether or not they need to be resubmitted (COMPLETE).

Optional and Informational forms are READ ONLY and DO NOT need to be re-submitted but should, like all program materials, be reviewed carefully and thoroughly with your participant.

To ensure the safety and well-being of all participants, forms with due dates must be submitted on time, no later than FRIDAY, MAY 13, 2016!
