Requirements: The permit holder (permittee) authorized to use dangerous weapons in commercial motion picture, television production, or other commercial entertainment events must maintain records of those activities conducted in California. (Cal. Code Regs., tit. 11, § 4130, subd. (d).) Current records must be maintained on form BOF 048, or a computer generated printout using the same format. (Cal. Code Regs., tit. 11, § 4130, subd. (d)(1).) The permittee must make these records available to any peace officer or other persons designated by the Attorney General upon request. (Cal. Code Regs., tit. 11, § 4130, subd. (h).) Permittees authorized to use dangerous weapons in commercial motion picture, television production, or other commercial entertainment events must provide an annual report of all activities. The annual report shall be submitted on form BOF 048 or a computer generated printout using the same format as form BOF 048. The annual report shall be continuous from the prior year's renewal application annual report. The annual report shall present the date(s), location, production company name, contact information and a description of the dangerous weapon(s) used. The annual report must be submitted to the Department of Justice (DOJ) with the Annual Dangerous Weapons License/Permit(s) Renewal Application, form BOF 031. Or, upon termination of the permit, a final report detailing all remaining unreported activities must be submitted to DOJ. Completing the Report of Commercial Motion Picture, Television Production or other Commercial Entertainment Event form: This form provides two entry sections for reporting two instances of up to three separate dangerous weapons (use additional sheet(s) of paper to report more than three weapons). Make as many copies of this form as required to complete your reporting requirement. Place a sequential entry number in the indicated space for each reported activity.
