The amendment request involves Section 12, Radiation Safety Officer. Please replace Jerad J. Grove with Susan J. Taylor as Radiation Safety Officer. Please see the attached paperwork for credentials and Radiation Safety Officer training certification.

Taylor, of Co no co Inc., has attended and successfully completed a 40 hour Industrial Radiation Safety Training course, conducted by TN Technologies Inc. on February 7-12, 1999 and described in the attached course outline. The participant received scores of 96.2% on the homework assignments and 89% on the exam, resulting in a final course grade of 91.9%. A course grade of70% is considered a passing score. The class average was 90.5%.
The course covers fundamentals of radiation, units of dose and quality of radiation fields, hazards of radiation exposure, detection devices, regulatory controls, industrial devices and specific training on installation and leak testing of TN Technologies density, level, and weigh gauges. It is designed to meet the requirements of me Nuclear Regulatory Commission and Agreement States for Radiation Safety Officers at companies using industrial gauging devices.
This course is structured to qualify persons who complete it to understand and safely perfonn various operations involving nuclear devices including the installation, relocation, and leak testing of such equipment. The operations are to be performed in accordance with the mles and regulations of the United State Nuclear Regulatory Commission and/or Agreement States, and are in all respects subject to such rules and regulations.

management meetings with contractors whose safety performance was not adequate 3) providing training to management in the refinery who are responsible for contractor oversight to insure they are managing contractors according to the processes and procedures.
Developed and integrated new tools and processes into the Billings Refinery in order to prepare the organization for the 2011 spring turnaround. These tools include: 1) A contractor bridging process which bridges the gap between COP and the contractor safety procedures. This process
normally been practiced in the Project Organization; however, I believe it is something entirely new to a refining turnaround process. 2) Developed a screening tool that is used to determine turnaround work that will require a detailed Job Safety Plan. 3) Developed a new Job Safety Plan document that will be completed by COP Safety, the COP Area Lead, and the contractor for work being executed during the turnaround. The purpose is to insure that safety items.
Emergency Response Coordinator; HSE; ConocoPhilips; Billings Refinery
-Responsible for the site emergency response functions, including: Emergency Response Plan,
equipment, training and exercises. -Facilitate training and management of the Emergency Response Team (ERT) and allied functions (Fire Brigade, Rescue, Hazmat, Spill Response and Emergency Medical Technicians) ·Responsible for ensuring proper maintenance and testing is conducted on the refinery fire water
system and emergency response equipment. ·Assist in S&OH programs and permitting. 'Serve as the Facility Security Officer. Turnaround - The 2009 Turnaround was the most successful TA in the history of the Billings Refinery. It was executed ahead of schedule, within budget, and zero recordable injuries.
Security - Developed and executed the Security Special Operations LogistiCS Plan, Training Plan, and rotation schedule. Worked with Corporate Security and site personnel to complete an extensive SVA report. The report was completed, documented and filed prior to the deadline.
Emergency Response - Was responsible for the emergency response portion of a successful H&S Audit conducted the first week of June, in which minimal findings were issued. Also assisted in the Industrial Hygiene and Health portion of the audit.
Discovered out of date and out of compliance issues regarding the emergency response service and maintenance, programs and processes, and have worked relentlessly to correct the issues and bring these back into compliance. Examples of these include: fire extinguishers, fire hydrants, hazmat response equipment, and ERT training. Purchased new bunker gear for the Emergency Response Team.
Managed multiple improvements to the Emergency Response Team including advanced planning, monthly training, responsibility assignments, communications, and team travel. I am currently
working on continuous improvement of the EMT and Rescue Teams.
Updated the SPCC Plan and the ERP Plan in time to prevent out of compliance issues with the Worst Case Discharge Scenario for the Billings Refinery. The plan was submitted to the EPA for
their files. Mutual Aid Fire School planning and participation. A mutual aid fire school was conducted with ExxonMobil, Billings Fire Department, Billings Airport Fire, Lockwood Fire Department and
ConocoPhillips. Currently completing a revision of the ERT Bylaws, exercise and drill schedule, as well as an ERT operations procedure. Will complete with the help of the ERT Captains a training, exercise and
drill schedule for 2010.
Completed the Incident Command Worst Case Tabletop Drill for the Billings Refinery.
