The skin is often taken for granted. However, it is the body’s largest and one of the most vital organs. Without it,

we cannot survive. The skin functions to provide protection from ultravio-let light, infection, and dehydration. It maintains body temperature regulation and produces vitamin D. The skin helps us communicate with our surroundings as it is the organ of touch and reception. The skin also provides clues about our internal health.

Severe stress and illness raise insu-lin levels. These conditions are associat-ed with multiple skin findings, including acne. Uncontrolled acne may also be a clue that dietary choices should be im-proved. Recent studies report that acne is associated with the intake of processed and sugary foods with a high glycemic index. Traditional acne treatments in-clude topical retinoids which help to normalize the skin’s exfoliation cycle and topical antibiotics to rid the skin of bac-teria that contribute to acne formation. More severe cases of acne may require oral therapies. Cosmetic procedures such as Aerolase Neo laser and chemical peels also are effective acne treatments. To enhance the effectiveness of medical and cosmetic therapies, a diet consisting mostly unprocessed foods with a low glycemic index is also recommended.

Excessive hair shedding, called tel-ogen effluvium, is associated with mul-tiple medical conditions. Thinning hair may be due to hormonal changes, ane-mia, thyroid disease, lupus, nutritional deficiencies, eating disorders, medi-cation, depression, and severe stress. Successful hair loss therapy includes treating the underlying condition. To help combat hair loss, preparations con-sisting of minoxidil and corticosteroids are often used. Platelet rich plasma in-jections is a natural and promising hair regrowth therapy that provides a nutri-ent enriched environment that supports folliclular health. Hair loss can result in

emotional distress. Therefore, the whole person must be treated. Adequate sleep, rest, exercise is also included in an effec-tive treatment regimen aimed to regrow hair.

Diabetes is also associated with skin changes. One common finding is acanthosis nigricans. This is described as thick and darkened skin on the pos-terior neck, underarms, inner thighs, and cheeks. This condition is rarely ge-netic but is most commonly due to ex-cess weight leading to increased insulin production and resistance. Treatment for this condition includes eating foods with a low glycemic index and exercise. Diffi-cult to control bacterial and fungal skin infections can be a sign of elevated blood sugars. Scleredema, hard and thick skin of the back, is a sign of diabetes. Granu-loma annulare, itching, and skin changes on the legs and feet can also be seen with diabetes. Ulcers on the legs and feet can develop due to poor circulation associat-ed with long standing diabetes.

Thyroid disease can cause increased sweating or skin dryness, hair loss of the scalp and eyebrows, skin changes on the legs, itching, and changes of the nails. Signs of lupus includes skin sensitivity to the sun, a rash, changes around the nails.

Vasculitis, or inflammation of blood vessels, can also be seen in autoimmune disorders such as lupus. Skin findings can also be seen in metabolic conditions such as gout, high cholesterol, and nutri-tional disorders. Skin and nail changes can provide a clue to dysfunction of the liver, kidneys, pancreas, and lungs. In rare cases, internal malignancies can be associated with skin findings.

In the United States, breast cancer is the most common cancer among women of all races. Unfortunately, it is also the second leading cause of cancer death in this gender population. In 2013, it is estimated that over

230,000 new cases of invasive breast cancer and over 64,000 new cases of non-invasive breast cancer (an early form of breast cancer) will be diagnosed in this country. This will lead to about 40,000 deaths from breast cancer. This makes breast cancer the most commonly diagnosed cancer in women, and the second leading cause of cancer related death in women. In their lifetime, about 1 in 8 (12%) women in the US will develop invasive breast cancer. This is a very chilling statistic, which makes early screening and detection only that much more important. Breast cancers that are discovered at earlier stages have a much higher chance of cure and survival. The key is to find cancer before any symptoms can occur.

According to the American Cancer Society, breast ex-ams by your doctor should be every 3 years for women in their 20’s and 30’s. The frequency of these breast exams by your health care provider should increase to every year at the age of 40. Yearly mammograms are recommended start-ing at age 40. These should be continued as long as the per-son is in good health.

Self breast exam is an option for women starting in their 20’s. If any suspected abnormalities are noted, this should be brought to the attention of your doctor.

There are some women who have a very strong family history and certain genetic mutations that run in their family. These patients may need a MRI in addition to mammograms.

The genetic mutation linked to development of breast and ovarian cancer is called the BRCA mutation. Women carry-ing this mutation have up to an 80% risk of being diagnosed with breast cancer during their lifetimes. The BRCA muta-tion is detected through a blood test collected at your doctor’s office or a lab. Not every women needs to be tested for this mutation. Possible women who qualify for BRCA mutation testing include those with a personal or family history of:

• Breast cancer diagnosed at age 50 or younger • Ovarian cancer at any age • Two separate breast cancers in the same person, or two

family members with breast cancers on the same side of the family

• Male breast cancer • Triple negative type of Breast Cancer (meaning breast

cancer carrying no receptors to target) • Pancreatic cancer and a breast or ovarian cancer in the

same person, or on the same side of the family• Ashkenazi Jewish ancestry with breast, ovarian, or pan-

creatic cancers• Two or more relatives with breast cancer, one under age

50 • Three or more relatives with breast cancer at any age • A previously identified BRCA mutation in any member

of the family.■

Time and time again I see men and women in their 40s and 50s with a similar story: Increasing stress at home and work, raising children, not sleeping well, gaining weight, less energy, less time for

pleasure and the list goes on. Along with this come aches and pains, lessened libidos, burgeoning intestinal and ab-dominal complaints, and often a growing list of medica-tions to combat these.

Fortunately there is a fairly simple approach to dealing with this kind of situation. First and foremost is to make sure there are no serious illnesses either happening or on the horizon. A good history and physical exam as well as appropriate comprehensive testing should be able to sort that out. Once that is handled you can begin to deal with getting well.

First and foremost is getting good restorative sleep. Over time we develop habits and patterns of sleep that do not promote health. Overuse of caffeine, cutting back time allotted for sleep and the development of medical sleep disorders such as sleep apnea (marked by snoring, gasp-ing, daytime fatigue and difficulty with losing weight) are all issues to consider. There are simple non pharmaceutical methods available to help promote the sleep we need for our bodies to heal.

Next is diet. As we get older we become less able to manage sugar and easily digested simple carbohydrates as found in breads, pastas, grains and some fruits. We tend to become somewhat insulin resistant, which creates a vicious circle of obesity causing more insulin resistance and inflam-mation, etcetera.

Weight gain and inflammation are often due to food al-lergies, a phenomenon known as leaky gut, inflammation and insulin resistance. One way to gauge your body’s meta-bolic activities is to have your doctor check your c-reactive protein level to measure inflammation, and if blood sugar is an issue, consider having a glucose tolerance test and check insulin levels. I have found food allergy testing to be very useful for many.

Evaluating and optimizing hormone status can be in-strumental in maintaining youth. Some say our hormones decline because we age. Others say we age because our hor-mones decline. Areas to focus on are thyroid issues which can produce fatigue and dulling. A phenomenon called “ad-renal fatigue” is becoming more recognized as a real issue and testing and treating this can make a big difference for people.

Finally optimizing hormone levels in men and women (testosterone, estrogen and progesterone) can afford great changes in energy and vitality.

As one begins to deal with these issues and others (such as toxins in our food and environment, micronutrient lev-els, exercise and stress management) it becomes obvious that they are all interconnected like a web. A simple exam-ple is that when one is stressed, you are less likely to sleep well, and data shows that people who are not well rested make poorer eating choices. Working with a health practi-tioner and examining your life and habits from these per-spectives can give one an access to new habits, new actions, and real changes.■

Restoring your hormones to optimal levels is an excellent way to prevent

Two million years of evolution suggest that the best solu-tion to staying healthy is to copy our body’s own natural hor-monal protocols. In other words, only a few people in their 20’s and 30’s develop cancer. The vast majority of women discover breast and uterine cancer after menopause and men get prostate cancer after their testosterone levels drop as they age.

A healthy body and mind defends against many chemicals (carcinogens) that cause abnormal cells to multiply (cancer). We all know that a wholesome diet, low stress and maximal body movement are important to keeping the body and mind healthy. What many of us are unaware of is that having the op-timal level of estrogen, progesterone, and testosterone is critical in preventing cancer. The protective benefits that these natural hormones provide when they are kept in balance is just begin-ning to be fully understood.

With regard to breast cancer, follow-up research in the Women’s Health Initiative (WHI) has shown that synthetic, chemical progesterone is the main reason for the increased risk of breast cancer. But natural progesterone is protective.

Natural estrogen can be protective when it is carefully balanced and monitored. However, if cancer cells are already present in the breast tissue, the estrogen can stimulate them to grow faster. Even natural estrogen use must be monitored with frequent mammograms and self- breast exams. That be-ing said, when these hormones are monitored and properly balanced:

There are times in our lives that give us pause, and we think to ourselves: “What happened to the happy-

go-lucky college graduate out to show the world? I’m struggling with strange feelings, moods and emo-tions that I don’t understand.” Others may say, “How did our relationship change? We used to have fun and en-joy each other’s company and now we are living like two strangers.” Still others may think “I had such high hopes for my child, so smart and full of energy. Now he lacks focus, doesn’t listen the way he used to, sometimes talks back to me and gets into trouble at school.”

For individuals and families the decision to enter therapy can be a dif-ficult one. Someone struggling with anxiety, depression, serious relation-ship conflict, a trauma disorder or a child’s behavior problem may not seek services right away for a variety of reasons.

When a family finally realizes that they need to seek profession-al help from a licensed counselor or therapist, they may not know where to turn. Who do I call? How do I find out who is the right counselor for me or my child? What is counseling and how can it help?

Mental health counseling or “therapy” is a process where you work with a professional, licensed counsel-or towards addressing problems that you may be having in your life or re-lationship.

When you find a counselor and explain your concerns to them, they will first develop a Treatment plan—steps that need to be taken to deal with the issue at hand. Sometimes a ther-apist may recommend a psychiatric evaluation or a rehabilitation program.

Once therapy begins, the thera-pist may guide your journey, but in most settings the “client” or “patient” defines the journey and is generally asked to set an agenda for each ses-sion. As you are guided through a process of self-reflection, you will learn more about yourself and the be-haviors, conflict or situation that you would like to change. You may discuss when or how your problem began, the skills you used to cope before you be-gan therapy and your strengths and areas for growth. Often times, thera-py helps a person to understand that while they may be struggling, they are competent in many areas and may even have skills or strengths that they never realized or tapped into. At times painful, therapy will lead you to a bet-ter understanding of yourself and pro-vide you with a set of tools with which to tackle your unique situation. Trust that you will emerge stronger and more self-aware, now able to reclaim the special, essential “You” that you always knew was there but couldn’t quite seem to get in touch with.

Estate planning is one place in life where you have the power to alter the outcome. Taking the time to decide who, when

and how your money will be handled and who will manage your health care when you are unable to make those de-cisions for yourself are important choic-es that you have the power to make.

A well-rounded estate plan includes five documents: a Revocable Living Trust, a General Durable Power of At-torney, a Health Care Power of Attor-ney, a Living Will and a Will. A Will is only effective at death; the other four documents have an impact during your lifetime.

In order to be effective, a Will must be admitted to probate. Probate is a process to transfer clear title of property from the person who died to the people designated in the Will. As with most legal processes, probate can be costly, time-consuming, and public. The most important role of the Will is the appointment of guardians for mi-nor children.

A “Revocable Living Trust” is a trust, which is created while you are living, and which you can revoke or amend at any time. A well-drafted trust can be extremely flexible and can facilitate management of your assets while you are well, during a period of incapacity, and long after your death. While you are alive and well, you would be the trustee and manage the assets in your trust. Upon your inca-pacity or death, someone designated by you would take over as trustee and would manage the trust, making in-

vestment and distribution decisions in accordance with the instructions you set forth in the trust, providing con-tinuity in asset management. Assets which are in a trust do not have to go through the probate process.

In a General Durable Power of At-torney, you appoint someone to make decisions for property not controlled by the trust such as IRAs, 401(k)s and other retirement accounts. This pro-vides for a smooth transition during periods of incapacity.

In a Health Care Power of Attor-ney, you appoint someone to make medical decisions for you in the event you can no longer do so for yourself. You may also express any wishes you have regarding life-prolonging mea-sures in a Living Will.

Founded in 1990, MENTOR Maryland is a leading treatment foster care agency, serving youth with complex behavioral, medical or mental health challenges. We offer a separate and distinct TFC program designed uniquely for Medically Fragile children. Since 1993 we have helped over 500 medically frag-ile children receive vital medical care and support in foster homes throughout the state. More than 150 medically fragile children have been adopted by

their MENTOR foster parents, while over 250 have success-fully reunited with their parents or relatives. This program needs dedicated families willing to partner in its success! The children have conditions ranging from pre-natal drug exposure to the most complex conditions such as ventilator

dependence. Other common health issues include seizure disorders, cerebral palsy, shaken baby syndrome, and HIV/AIDS. Many of the children placed with us also have de-velopmental delays, autism, or some degree of intellectual disability. ALL of them have the potential to thrive by be-coming part of a loving family!

MENTOR’s Medically Fragile program specializes in providing treatment foster care for the State’s most medical-ly vulnerable children. This highly comprehensive service line offers children from birth to 21 a home setting as an alternative to hospital or group care. In our foster homes, each child benefits from one-to-one nurturing, medical care and social stimulation provided by foster parents who are specially equipped with support from our program’s team of nurses and social workers.■

1/4 cup green cabbage 1 teaspoon onion

3 tablespoons quinoa or rice 1 teaspoon coconut sprinkles 1 tablespoon avocado diced

PreparationBoil 1/2 sweet potato, 1/2 cup quinoa, combine organic canned black beans. Mash sweet potato and quinoa togeth-er and mix in black beans. Add finely shredded raw cabbage and all other ingredients. Use romaine lettuce leaves.■

Ingredients1/2 cup spinach

1/2 cup corn meal 1/2 cup of black beans

PreparationIf grilling, preheat an outdoor grill for high heat, and lightly oil a sheet of aluminum foil. If baking, preheat oven to 375 degrees F (190 degrees C), and lightly oil a baking sheet.

In a medium bowl, mash black beans with a fork un-til thick and pasty. In a food processor, finely chop spinach. Then stir into mashed beans. Mix in corn meal until the mixture is sticky and holds together. Divide mixture into four patties. If grilling, place pat-ties on foil, and grill about 8 minutes on each side. If baking, place patties on baking sheet, and bake about 10 minutes on each side.

Create an onion, garlic and green bell pepper paste in your blender.■
