Three dishwashers started working at 8:30 am. By 1:00 pm workers were involved in washing the dishes since there was a huge banquet the previous night, leaving lots of dirty dishes.The dishwashing procedures consisted of:

1- Pre-rinsing the dirty dishes by hand in a water tank.2- Washing the pre-rinsed dishes in the automatic dishwasher3- Draining the washed dishes and placing them in cupboards.

Starting around 2:00 pm, workers began complaining of such symptoms as headaches and dizziness. By 2:30 pm, all workers showed the same symptoms. Two seriously affected workers were taken to a local hospital by ambulance and the other nine workers were also eventually taken to the hospital for treatment.On arrival at the ER is a 54 y/o woman- her last memory was washing some asparagus off of a plate. Her next memory was riding to ER.

PMHX: PUD, fibromyalgiaMeds: Unknown anxiolytic, estrogenFmHx: UnknownSHx: Divorced. Lives with her boyfriend in a trailer park. Smokes 3 packs/cigarettes/day, drinks six-pack of beer daily.ROS: Positives for headaches, dizziness, weakness, nausea, difficulty concentrating, dyspnea and visual changes.

PE: BP 126/91, HR 86, R 30, T 37.1MS: AAAO x 1(name), speech without dysarthria. 2/3 recall at 5 min. Abdomen: +BS, soft, nontender.Neuro: CN strongly intact, reflexex equal and symmetrical.Motor: Full strength throughout with normal muscle tone and bulk.Sensory: Unremarkable.ABG: 7.41/30/370 with 100% FiO2, carboxyhemoglobin level is 26%
