Consensus statement on Advanced Practice Providers (October 2014)
Introduction and purpose A workforce shortage of 65,000 physicians is projected for both primary care and specialty medicine by the year 2025. Similarly, the supply of urologists per capita in the United States continues to decrease, a trend that started in 1991 and continues to accelerate. In 2009 there were only 3.18 urologists per 100,000 in the population, which is a 30-year low and amongst the most severe specialty medicine shortages. This is compounded by the fact that urology has the second oldest surgical subspecialty workforce with an average age of 52.5 years, of whom greater than 18% are age 65 years or older.

Clinical nurse specialist (CNS): An APRN who integrates care, focusing on the interactions of patients, nurses, and systems. A continuous focus of this role is improvement of patient outcomes and nursing care with the goal of developing evidence-based practice to improve overall patient care within a system. Specific roles can include diagnosis and treatment of health and illness, health promotion, and risk factor prevention among families, groups, or communities.

The PA is a medical professional who is a graduate of an accredited PA program who is also nationally certified and state licensed to work with the supervision of a physician.
PAs undergo training based on the medical model with a focus toward primary care, including courses in pathophysiology, pharmacology, physical diagnosis, laboratory science, and microbiology. Specific rotations in diverse environments such as family medicine, internal medicine, pediatrics, general surgery, and obstetrics comprise clinical rotations for PAs and emphasize primary care, office practice, or long-term care facilities.
Prior to admission to a PA program, students have various undergraduate and graduate degrees, with a minimum of two years of college courses in basic or behavioral sciences.
The academic degree granted after completing a PA program is usually a master’s degree, and they must pass a certification examination for licensure.
Eligibility for certification involves evidence of degree status and 1,000-2,000 hours of clinical practice. (~1 jaar)
In clinical practice, the role of the PA is as part of a team practicing medicine with physician supervision, and is frequently described as delegated autonomy.
The AMA House of Delegates adopted the Guidelines for Physician/Physician Assistant Practice, which includes:
The role of the PA in the delivery of care should be
defined through mutually agreed upon guidelines that are
developed by the physician and the PA and based on the
physician’s style of delegating. The physician must be available for consultation with
the PA at all times either in person or though
telecommunication systems or other means.
The physician is responsible for clarifying and
familiarizing the PA with his or her supervising methods and style of delegating patient care.

Conclusions:
Despite expertise and experience, nurses had a variable, often negative, experience of the MDT. It is necessary to ensure that all participants can contribute and are heard and valued. More emphasis should be given to patients’ nonmedical needs.

3 jaar opleiding algemene heelkunde, naast Frans leren, dokter worden, spoed en IZ draaiende houden1/3-1/2 van wacht, het pré-Colla tijdperk, een stiel leren door noeste arbeid.
Je kon leren via:
1) Je opleider als die er was (vooral overdag en tijdens de week)
2) Zelfstudie voor niet dringende zaken en vooral uit gemaakte fouten, beetje ala guerre,…3) Van de verpleging!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
De verpleging, secretaressen en onderhouspersoneel noemde me “Karle” en samen deden we de zaak draaien

Het personeel noemde me dokter en de taken werden goed afgelijndIk kreeg een MSD-prijs waarmee ik 1 maand naar Iowa en Durham kon waar ik voor het eerst een
nurse specialist aan het werk zag: leek een verpleger die beetje dokter speelt.

1) Functonele urology was noodzakelijk, meer niet en was zeker geen prioriteit2) Er was zeer veel werk in de functionele urologie en neurourologie
3) Ik was gewoon te leren en samen te werken met de verpleegkundigen
4) Ik had een nurse-specialist leren kennen in de US5) Er was een andere snotneus en wijsneus, Ronny genoemd en die leek geïnteresseerd in de functionele
urologie en neurourologie:

Door Ronny zijn specifieke kennis als hoofd nursing op de afdeling, zijn leergierigheid en gedrevenheidkwam capaciteit vrij omdat:
- Katheterbeleid werd overgenomen- Zaalrondes in ziekenhuis werden overgenomen
- Stoma problemen werden opgenomen
- Programmeren en opvolging van neurostimulatoren werden overgenomen
Ik kreeg verder het gevoel dat Ronny plezier beleefde aan zijn meer activeve soms bijna illegalezelfstandigheid en
- Katheter en stomabeleid werd gewoon beter in Ronny zijn handen
- Patiënten tevredenheid verdubbelde niet enkel door zijn kennis maar ook door de extra tijdsbestedingdie bij mij dikwijls tekort schoot

Er kwam tijd vrij voor wetenschap, eerste doctoraat starte in 2011

Weinig diensten kennen onze productie van publicaties, doctoraten en rekruteren zoveel sponsoringOnze dienst is niet meer verlieslatend
Verpleegkundige werd aangenomen om te doctoreren ……. Had haar eigen interesse zoals de pessaria
Is dat allemaal door Ronny?

Nee maar door de intense samenwerking met Ronny en de zelfstandigheid en toelaten van creativiteit van Ronny en volgende verpleegkundigen kregen we :
- De kritische massa aan werkkracht zodat tijd vrij kwam
- Met toename tevredenheid van de patienten en doorverwijzers
Daarnaast werden specifieke topics uitgediept zodat ze eigenlijk zijn toegeëigend aan de verpleegkundigen
en door de artsen bijna niet meer worden behandeld.
Maar vooral maakte dit allemaal mogelijk “dat we konden groeien” met de beschikbare middelen !
