The guage will normally be stored in a lockedwooden cabinet in the equipment storage room ofthe headquarters office at 2900 Boniface Parkwayin Anchorage, Alaska. This office building is atwo story concrete block structure 55 feet by 100feet in dimension. Suite 200, the firm's office,occupies the south half of the second floor. Ithas a separate rear entrance and a front entranceto a central hall and stairway. Exterior doorsand the interior door to the central hallway areheavy duty steel or glass and steel with dead boltlocks. The building has an electronic securityand alarm system. Security for Suite 200 includesdoor switches at both entrances and an interiormotion detector,

Item 15 RADIATION PROTECTION PROGRAM

A. SAFETY PROCEDURES

1. Do not operate or attempt to operate the guageunless you have been authorized to do so.

2. Do not attempt to repair, modify or open thescaled sources under any circumstance.

3. Wear a film badge at all times while operating ortransporting the guage.

4 Follow operating arocedures, when using the guage,in accordance with the Troxler instruction manual,the radiation control regulations and this safetyprogram.

5. Keep unauthorized persons away from the guage.

6. Do not leave the guage unattended when in use oroutside of the storage enclosure or locked vehicle.

7. Keep the guage in the " SAFE" or storage positionwhen not in use.

8. Be sure that the guage is locked within an authorizedenclosure when it is not in use. Security againstthe theft of a radioisotope is of utmost importanceand must not be neglected. The storage enclosuremust be labeled with a radiation warning signbearing the symbol as described in 10 CFR 20.203and the words " CAUTION RADIOACTIVE MATERIALS".

9. The guage may be only transported by authorizedgersonnel in approved vehicles. The guage may notse transported on the front or rear seats of anyvehicle. When carried in a suburban or station-wagon, the unit must be placed in the rear-mostarea of the vehicle. If a pickup truck is used,the guage must be locked in its shipping case andthe case secured within the locked canopy or toolbox bolted to the truck bed in order to preventloss or theft.

B. EMERGENCY PROCEDURES|

a. In the event of possible damage to the sourceor source control mechanism, the operator will

! keep unauthorized persons at least ten feet| from the gauge and prevent removal from the

site until authorized by the RadiationProtection Officer or appropriate authority,

b. If there is any possibility that the sourcecapsule might be ruptured, the location mustbe covered by a tar) or sheet of plasticmaterial held down ay weights to preventscattering of radioactive material by theelements.

c. The operator must then immediately notify the| Radiation Protection Officer of the incident

and give an appraisal of the probable con-dition of the source.

d. The Radiation Protection Officer will thenimmediately notify the Nuclear RegulatoryCommission at the following location.

2. Source Stolen or Lost

a. The operator must immediately notify localpolice or other law enforcement agency withinwhose jurisdiction the incident occurred.

b. The operator must also notify his RadiationProtection Officer who will notify theNuclear Regulatory Commission office.

C. DUTIES OF THE RADIATION PROTECTION OFFICER

1. Assure compliance with all pertinent parts of theNucicar Regulatory Commission regulations.

2. Assure compliance with the conditions in theRadioactive Materials license and amendments anditems in this safety program.

3. Perform the guage leak tests at the intervalsrequired by the Radioactive Materials License.The wipe sample will be collected using a TroxlerModel 3880 leak test kit. The leak test measure-ment on the wipe sample will be performed byTroxler Electronic Laboratories, Inc., P.O. Box 12057,Research Triangle Park, North Carolina 27709.

4. Maintain the following items in a radiation fileand keep available for inspection by the NuclearRegulatory Commission, if requested.

(a) Current Radioactive Materials License.

(b) Copies of the license application, attachmentsand all pertinent correspondence referred toin the conditions of the license and amendments.

(c) Guage Source Certificate issued with theguage by the manufacturer.

(d) Film badge reports.

(e) Leak test reports.

(f) Records concerning disposal, inventory anduscage of nources.

(g) Copies of this safety program.

(h) A current copy of the Nucicar RegulatoryCommission regulations.-
