I am inspired every day by the stories of the women, men, and children that we serve – their determination to overcome obstacles, their strength, and their faith. As you can see from the timeline below, 2012 was a year of action for Community of Hope. Serving more than ever before in our 33 year history, we worked to end homelessness for over 260 families and provided compas-sionate healthcare to over 6,700 patients.
In addition to working every day with people in need, we are also working to transform both housing and healthcare systems throughout the city. With a grant from the Freddie Mac Founda-tion, we will assess up to 500
families who are homeless in order to provide the right assistance at the right time – an accurate, personalized approach to ensure that families are placed in the program that best fit their needs. Among our standout successes in 2012 was our Rapid Re-Housing program, where families had a 92% success rate. With the DC government and a nonprofit partner, Transitional Housing Corporation, Inc., we are work-ing to fundamentally change the system so that families may move out of homelessness faster. In the area of healthcare, we are prepar-ing for the implementation of the Affordable Care Act, including focusing on patient-centered care, achieving third party certification and accreditation, and expanding
to provide care to 10,000 more families in the city with the open-ing of the new Conway Health and Resource Center in the very underserved Ward 8.
We are on track to serve even more families in 2013. Of course, this work would not be possible without our dedicated Board of Directors, donors, volunteers, staff and partners. Together, we are making this city a better place for all families and creating brighter futures for our children.

Ending HomelessnessImagine that you’ve got no place to call “home.” Bouncing from couch to couch, or crammed in DC’s family
shelter, you have no stability or routine. Setting and achieving goals is tough, if not impossible. Guided by the Housing First model, Community of Hope starts by providing families with the stability of a home so they can
focus on increasing their income, putting family first, and reaching self-sufficiency.
Community of Hope’s programs are designed to build your strengths and will help you so that you can stand on your own. You choose what your outcome will be and Community of Hope will help you get there.

“Healing our City
The Marie Reed Health CenterCommunity of Hope’s Marie Reed Health Center in Adams Morgan provides primary medical, dental and behavioral health services. We serve all patients regardless of insurance status or ability to pay with quality care. This year, we reviewed every aspect of the patient experience to identify ways to make our care more patient-centered. We received the highest level of recognition as a Patient-Centered Medical Home and are the first community health center in the District of Columbia honored with this distinction.
The Family Health and Birth CenterCommunity of Hope’s nationally accredited Family Health and
Birth Center (FHBC) provides holistic care for moms, babies and the entire family. The only free-standing birth center in the
District of Columbia, FHBC midwives and staff provide personalized prenatal and maternity services. Using a group
model of care, FHBC patients benefit from a supportive community. Last year, FHBC midwives helped deliver 265
babies. In addition to specialty birth services, FHBC offers complete primary care for the entire family.
The Conway Health and Resource Centercoming soon
Across the Anacostia River in Ward 8 lies the District’s most impoverished and medically underserved region. Ward 8 residents face the worst health outcomes in the District of Columbia in areas such as chronic disease, maternal and pediatric health, and HIV/AIDS. Without access to affordable primary medical, dental or behavioral health services, many people wait until it’s too late, often ending up in emergency rooms. Community of Hope’s Conway Health and Resource Center will work to change that.
With the ground broken in July 2012, the Conway Health and Resource Center, named for the generous support of Bill and Joanne Conway, is set to open in late 2013. This 50,000 square-foot
space will be a catalyst for change by providing residents with access to affordable medical, dental and behavioral services. It will also be the new home to Community of Hope’s administrative and hous-
ing staff, bringing us closer to the families we serve. Ultimately, this new space will serve 10,000 more District of Columbia residents, bringing brighter futures to our neighbors in need.

District of Columbia by providing quality health services. Serving more patients than ever before, 2012 was a landmark year for Community of Hope. Our dedicated providers served over 6,700 patients in

Our positive net income on the operating side is a result of accounting practices for
grants, as well as continued improvement in our billing practices for Health Services. We are reinvesting in our programs already and this strong trend is preparing for signi�cant
growth in 2014. On the capital side, we have also made great progress on construction of the Conway Health and Resource Center, which accounts for the large growth in net assets. Full audited �nancial statements are
