Fourth Street Clinic operates on a nearly 50:50 cash to in-kind donation ratio. For every $1 given, nearly an additional $1 is leveraged in the form of in-kind donations and volunteer hours.

Discounts and off-site service donations further expand services and funding:

n Intermountain Healthcare donated $244,000 worth of radiology services

n Standard Optical discounted lens grinding by 89%

n Ameripath discounted cervical cancer screenings by 41%

n Quest Diagnostic discounted laboratory services by 60%

n Audiology Associates of Salt Lake City discounted audiology services by 80%

n Family Dental Plan, Salt Lake Donated Dental and Premier Dental Care discounted dental care services by 26%

Fourth Street Clinic helps homeless Utahns improve their health and quality of life by

providing high-quality health care and support services. By increasing homeless Utahns’

access to primary care delivered in a coordinated clinical setting, Fourth Street Clinic

is a major partner in promoting community health, ending homelessness and achieving

across-the-board health care savings.

HISTORY

The mid-1980s brought a dramatic rise in Salt Lake City’s homeless population. During

this time, Salt Lake City’s downtown was revitalized and several substandard, single-room

occupancy hotels (SROs) that offered rooms for just a few dollars a night and required

no leases were razed and redeveloped. In total, 800 low-income housing units were torn

down, displacing 1,000 very poor residents from their homes and jobs. With no public plan

to rapidly re-house or support these residents, homelessness moved from a sporadic,

temporary experience to endemic and intergenerational.

To fill the service gap, nonprofits such as Fourth Street Clinic began to form. Founded in

1988, Fourth Street Clinic began as a triage clinic staffed with one, part-time nurse who

relied heavily on hospitals for patient treatments.

Today, with a staff of 50 and a volunteer network of more than 150, Fourth Street Clinic is a

comprehensive health care home that serves 3,747 homeless men, women and children with

integrated primary care, behavioral health, pharmacy and specialty care services.

Fourth Street Clinic received HealthInsight’s 2012 Physician

Office Quality Award as the first primary care clinic in

Salt Lake, Tooele and Summit counties to meet quality

improvement benchmarks for diabetes management. Diabetes

is the top diagnosis among Fourth Street Clinic’s homeless

patients and is among the most common, costly and

preventable health problems in the United States (Centers for

Disease Control and Prevention). Below are the percentages

of Fourth Street Clinic’s diabetic patients who received the

recommended screenings and achieved benchmarks for

improved health over time.

HOME is an elusive concept for those living in extreme poverty and unstable housing.

However, in 2012 Fourth Street Clinic created a “home” for homeless Utahns by adopting a

Patient Centered Medical Home (PCMH) model where each patient regularly sees their own

medical team who compassionately diagnoses and treats the underlying chronic diseases that

so often trap people in poverty and homelessness.

Through providing a PCMH for patients, Fourth Street Clinic works to eliminate the fragmented

care that does little to improve long-term health and drives up community health care costs. This

comprehensive approach is effective in reducing the frequent 911 calls and emergency room

visits that are the traditional treatment options for those living in extreme poverty. Providing

appropriate care at the appropriate time and in the appropriate setting improves quality and

contains costs.

By embracing a focus on quality, Fourth Street Clinic has seen an impact on patients’ lives.

What PCMH means to patients like Sandy is that her medical provider talked with her about the

high risks of her becoming diabetic. Sandy now understands how her medications and obesity

affect her health, and she has decided to lower her risk by engaging in weight management and

nutrition counseling. When Sandy finally moves out of the homeless shelter, she will leave with

a whole new understanding of how to take care of herself.

Stories like this remind me that by keeping comprehensive, quality care core to our mission—

our patients benefit, our health care systems benefit and our community benefits. I look forward

to building upon a proven model that manages the root causes of homelessness and not just

the symptoms.
