The new approach includes accurate evaluation of the coupling between implant, abutment and other prosthetic components after osseointegration. In this area new features of the implantable osteointegrated systems have been developed. The clinical follow-up of GTB implant system is characterized by commons evidences maintained over time:

O ver the past ten years many changes occurred in the field of dental implants and concepts that seemed to have become axioms were revised. After having focused almost exclusively on the concept of

osteointegration, recently other key factors have been studied: the maintenance of perimplant bone tissue, especially at the crestal level, the improvement of implant system functionality after loading, i.e. the biomechanics of the implant.

T o easily obtain those clinical results the implant system should provide both surgeon and prosthodontist a perfectly tuned package of products and protocols. The protocols have to be

simple, fast and cost effective. On the other hand, the products have to be easy to use and grant to the dentist effectiveness and efficiency. For those reasons the GTB implant system has united in the same product the following critical success factors.

Abutment with a slim and concave profileTo guarantee a lot of available volume to peri-implant soft tissues so that they can create a new biological seal, generating a biological condition quite similar to a healthy natural tooth.

High precision conical connectionTo avoid dynamic micro-movements and the resulting pump-effect that allow bacteria to infiltrate into the gingival channel.

Implant bioplatformA brand new concept that uses the horizontal surface of the fixture to obtain an integration with biological tissues. A complex design of the implant platform that enhance the amount of integrable surface (30% more), lower the transfer distribution of the masticatory load and create an annular space to mechanically protect the blood clot (boosting the predictability of bone regrowth over the platform itself ).

High biological performanceThe GTB fixtures are design using the Finite Element Analysis to evaluate the behavior of both implant system and bone tissue during mastication. The masticatory load is distributed in a completely different and innovative way respect traditional implant fixtures: instead of transmitting most of the load trough the implant neck the GTB fixture use the apical portion of the implant discharge the most of the load and this load is transmitted to the bone as a compressive stress, according to bone biology and avoiding marginal bone overloading. That’s the reason why with GTB implant system it’s possible to use thinner and shorter fixtures.

Progressive blood clot dissipation channelThis feature allow the blood clot to mechanically wet the entire fixture surface without using high hydrophilic surface treatments. The blood will raise up to the implant BioPlatform avoiding blood isostatic compression: this incredibly affects the patience comfort during and, moreover, after the implant surgery.

GFA Gingival Former Abutment

T he Gingival Former Abutment allows the dentist to use a prosthodontics tissue level approach even when the implant’s platform is positioned under the marginal bone

level. This clinical protocol allow the dentist to perform a true one time abutment protocol, a condition that grant the best way to preserve the soft tissues attachment: that’s the perfect condition to maintain the biological seal created during the healing time.

Simplified proceduresMoreover, having a new platform at tissue level makes simple and fast every prosthodontics procedure. The double GTB conical connection allow to use the GFA modular implant concept for full arch rehabilitations but mostly for the single crown rehabilitations using a time&cost effective protocol. Finally, the GFA modular implant concept can be performed both with screwed and cemented retained prosthesis.

Radiographic control

One stage surgery using GTB TZERO implant and GFA prosthetic component

Patient comes to the clinic after an orthodontic treatment in 1.4. The implant GTB TZERO is positioned and at the same moment the GFA is placed. After three months of treatment and after checking the implant osteointegration, the final screw retained ceramic-metal crown is placed on the GFA.

One time abutment prosthodontic approach and control of tissues levels

A 47-year-old patient who came to the clinic with a fracture of 2.5. The extraction was performed and an immediate post-extraction GTB TZERO implant was placed in the same surgical act. After the implant was placed the GFA was positioned on the implant, impression was taken and, a few hours after the surgery, an immediate loading provisional crown is placed on the GFA. Three months after, with the osteointegration of the implant, the screw retained ceramic-metal crown was placed onto the GFA.

Even when the immediate load is not necessary or not recommended, the one time abutment surgery is an important way to obtain predictable stability of the soft tissues and assure the best quality of the treatment for the patient

A 62-year-old patient came to the clinic to rehabilitate the edentulous area of the third quadrant, placing two GTB implants in 3.4 and 3.6 and the MUAs in the same surgical act. After three months, with the osteointegration of the implants, the three screw retained crowns was placed in position 3.3, 3.5 and 3.6.
