Background of the ProblemTheoretical Framework

Application of Evidenced Based Research StudyIntroduction of the ProgramChemotherapy drugsare a heterogeneous group of drugs in which many are known mutagens, teratogens and/or carcinogens. Allergic reactions have also been reported. Studies show that there is a health riskduring exposure of chemotherapy drugs and there are guidelines for minimizing exposure of health personnel to these chemicals.Workers in daily contactwith these drugs will be more at risk due to the increasing use of chemotherapy. Chemotherapy spills refers to spills during preparation and leakage from infusion bags. (Oslo University Hospital 2012)Target PopulationThe target population of the program would include the Doctors, Pharmacists and Nurses of the Cagayan Valley Medical Center, health professionals involved in the preparation of chemo drugs.Goals and Objectives of the Program The main goal of this program is to ensure the safety of the health professionals involved from exposure to cytotoxic chemicals in the preparation of the chemo drugs. To avoid contamination and preserve the sterility of the drug. To ensure that spills of chemotherapy drugs or waste materials that contain these chemicals are handled in a safe way to protect health and safety. To protect the people and surroundings from exposure.Activities/TimelinesOrganization/AgenciesCagayan Valley Medical Center will be utilize as the agency for the program proposalTotal Cost of the Program

Significance and Applicability of the Program ProposalThe applicability of the program proposal will benefit the nurses, patient and the administration.Sustainability of the Program1. Mag oorder yung onco fellow sa chart kung anu yung chemotherapy treatment ng px. Including the meds needed and materials to be sent sa laminar flow. 2. Secure chemo meds from the pharmacy or from the px. 3. Secure CAOS (chemotherapy administration order sheet) form - ginagawa ng onco fellow. Nakasulat dito yung desired tx. E.g. doxorubicin 900mg in 500ml D5W to run for 4 hrs. Of course may name ng px yung form rm/ward etc. 4. Call laminar flow and inform them na magpapadala ka ng chemo meds na ipapatimpla. 5. Ask nursing aid to bring chemo meds, materials (iv fluids, macroset, syringes, needles, gloves, mask, chemo cover), and CAOS form to laminar flow. 6. Mixing of chemotherapy done by pharmacist at laminar flow. Pag tapos na, ihahatidnlng nila sa ward yung chemo. 7. Pagdating ng pharmacist/nurse sa ward, ieendorse nia yung mga natimplang chemo meds sa nurse in charge. Reading the label sa ivf ng chemo and CAOS form then ipaparcv sa ward nurse yung isang copy ngCAOS. 8. Chemo to be administered by ward nurse na. *** minsan kasi ndi lang isa yung nakatimpla na gamoT para sa px, so kunwari lahat ng pang day1 chemo (3 diff.meds), ipapatimpla na sa lminar flow. Tapos naka label na individually yun, idodouble check nlng ng ward nurse sa chart which to administer first
