increasing the likelihood of patients being able to remain in their preferred place of care towards the end of their life, rather than having to be admitted to hospital. To tackle potential symptoms that may occur as disease progresses, a short list of medicines can be prescribed and kept within a JIC box in the patient’s home for use by an attending health professional. This is potentially of most benefit during the Out-of Hours period. JIC boxes are distributed through the 22 NHS Fife Palliative Care Network Community Pharmacies. The Prescription Kardex allows Community Nursing staff to administer the medicines prescribed and acts as a signal to the pharmacy that a box is needed. JIC Boxes contain medicines for the following symptoms:

The overall aim of this service is to provide a network of community pharmacists throughout Fife, who are able to meet the pharmaceutical care needs of palliative care patients.
The following key services are provided:
Dispensing of specialist palliative care medicines, according to an approved list .
Provision of advice and information on the use of these medicines to patients/carers and healthcare professionals.
Liaison with the patient’s usual community pharmacist (chemist) and primary health care team to ensure continuity of supply of medicines and advice.
Just In Case Boxes
Palliative care network pharmacists receive specialist training in palliative care, with regular updates.
Enquiries are welcomed regarding drug compatibility in syringe drivers; side-effects of treatment; problems in administration of medicines.
Specialist support is provided by Fife Palliative Care Service.
If a prescription for a palliative care medicine is needed when the pharmacy is closed, contact the PCES healthcare professional line on .
Some palliative care medicines listed are also stocked at the local PCES centre. If a medicine is not stocked at PCES, then the PCES centre will contact a network pharmacy on your behalf.
For medicines not available from the PCES centre or network pharmacy, PCES will contact the hospital pharmacist on-call for advice.
All medicines below (except Octreotide and Levomepromazine) are held in each of the pharmacies listed in this leaflet.
Levomepromazine 6mg is unlicensed and is used in patients with nausea and/or vomiting where the 25mg strength causes adverse effects (e.g. drowsiness) even when the tablet is halved.
Octreotide is used in a syringe driver, and can be useful for patients vomiting with GI obstruction. As both can be required urgently, for patients in the community, and in order to ensure access to and availability of these drugs, 3 pharmacies (1 in East, West and Central Fife area) routinely stock a supply of each.
When a patient is prescribed a specialist palliative care medicine, which is
: the patient’s usual community pharmacy may not stock that medicine. If it is included in the stock list then refer the patient to the nearest Palliative Care Community Pharmacist (see list overleaf) who will:
Dispense the initial prescription, and give the carer/ patient advice on the medicines (where appropriate).
Refer the patient back to their usual pharmacist for further supplies (if required), once they make an assessment of their ongoing pharmaceutical needs. (This may require liaison with the patient’s GP/district nurse).
