One passport size family photograph comprising of mother, father, the selected child and his/ her real brother and sister (if any). 3. Passport size photograph of the selected child. 4. Photo Copy of Municipal Birth certificate of the child. 5. Photo Copy of Proof of Residence (copy of Driving Licence/Ration card/ Passport/ Election ID/ Bank passbook/Telephone Bill/Electricity Bill/Adhar Card/Registered lease deed) Only Provisional Fee Structure Nursery :- Without Transport – Bank DD of Rs. 36000/- for the period April to July 2015. With Transport - Bank DD of Rs. 42000/- for the period April to July 2015. In addition to the above documents, following are required for Selected First child, Neighborhood /School Transport Criteria Candidates only, in accordance with draw of lot held on 30-01-2015 Original documents for verification. Copy of the Online form Original Municipal Birth certificate of the child. Proof of Residence (copy of Driving Licence/Ration card/ Passport/ Election ID/ Bank passbook/Telephone Bill/Electricity Bill/Adhar Card/Registered lease deed) Children need not accompany parents for the verification procedure .
