  3. 3. It is the goal for all students to leave with a direction and a skill level that will allow a successful transition to community life. Some students need an extended period of time to develop these skills as well as a familiar supportive environment Developing work skills in a realistic environment within the school district allows students to develop basic job skills and behaviors needed to transition outside of school
  4. 4. We use Village School Support of Building Administration E-mail is sent to all Staff to identify In- House Jobs Identify specific Jobs
  5. 5. Key Component Without Transportation we had no way for the students to get to Village and Home Transportation has been provided for all students. Currently all students go home on a bus specifically for them In the past we have utilized Village School buses with elementary school students

Identification of Students who may participate CST meeting takes place with parents, students and STRIVE Coordinators in attendance to explain the program and answer parents questions We encourage them to share information to better assist us in making this a successful program to meet their goals. We have begun to use Im Determined Form with staff and families to assist us with obtaining information about individual students Case manager fills out Referral Form Permission slips sent home Coordination of Nursing Staff Coordination with staff regarding behavior plans if necessary

Program takes place every Tuesday and Thursday Students are Transported to Village by bus Sign In at Main Office Learn to Greet staff appropriately Travel Independently to STRIVE room Put on ID Badges / Uniforms Given Daily Assignments Students travel home on regular Village School buses **Must call into Main Office if they are not coming to work Instill Accountability.

Skill Development Areas Self Determination According to Wehmeyer, Agran, and Hughes (1998) people who are self determined autonomously, self regulate their behavior and are psychologically empowered and self realizing. Throughout our Program we try to touch on the various Elements of Self Determination.

  19. 19. All students will be Interviewed during the first phase of the STRIVE Program. They will practice Interviewing Skills prior to the actual Interview They will be given Assistive Technology to use during the Interview if necessary. They will be rated with the following scale

  25. 25. ESY Program Extended School Year program involves students expanding their skills to more community based instruction -Grocery Shopping Service -Snack Bar -Family Style Lunch
  26. 26. Through our connections within the Community we have received a variety of Donations. The Holmdel Kiwanis donated our STRIVE Uniforms, that we still use. In addition we have received monetary donations from an individual who truly believes in our program and the service we are providing to our students. We are always looking for Community Members to provide us with opportunities for our students in the future. Marshalls We welcome input from staff and parents to help improve our program.

ADDING TIER II Veteran STRIVE students can apply for leadership positions These positions involve orienting, training and mentoring new STRIVE students Once a student has been in the Tier II level for one year they are responsible to encourage Tier I students to request an interview to enter the Tier II level The Tier II students conduct the interviews Tier II students engage in leadership self-evaluation A leadership level allows students opportunities to strengthen self-determination
