Many of our members are participating in the Centers for Medicare and Medicaid Services (CMS) Acute Hospital Care at Home program, either expanding or starting new hospital at home initiatives. The 2021 Hospital at Home Users Group Annual Meeting—What’s Happening, What’s Next—comes amid a period of transformational growth for hospital at home programs across the country. Spurred by the CMS waiver, health systems and hospitals are exploring how acute care for a wide range of patients and diagnoses can be delivered safely and effectively at home. Looking beyond the public health emergency, all hospital at home programs must consider how to both improve their patient care and respond to an uncertain future.

The Annual Meeting consists of morning and afternoon sessions, featuring presentations from experts in delivering high quality acute care in older people’s homes. Additionally, it includes a poster session,

innovation presentations, awards, results from a hospital at home photo contest, and even some hospital at home trivia.

Psychopharmacology for Agitation and Psychosis in Dementia

Neuropsychiatric symptoms of dementia are common and progressive in dementia. The session will discuss the off-label use of psychotropic medications for the treatment of agitation and psychosis in dementia. Bridging the Digital Divide in Telehealth

The COVID-19 pandemic led to a rapid expansion of telehealth capabilities across all practice types. Providing telehealth services to certain populations (elderly and others who face technological or financial barriers) presents distinct challenges and opportunities. This session will focus on identifying barriers to telehealth among patient populations and some strategies for

addressing these barriers. These strategies include both technologic and personnel resources. The session will focus on the experiences of three different home telehealth programs for primarily older adults.

Recent growth in Home Care Medicine creates ethical dilemmas due to changing financial incentives. We offer three case studies, including one of discharging a patient from the practice, to highlight tensions between a desire to “do the right thing” and financial or safety concerns. There is no easy answer, given the fact of “no margin, no mission.” We aim to promote debate about ethical issues that weigh on daily decisions of HCM leaders and clinicians.

Advance Illness Management for Home Based Patients Led to Improved Outcomes and Better Experience

This session will present an innovative clinical practice with an interdisciplinary care team including home health nurses, nurse practitioners, physicians, social workers, and pharmacists that implemented a home-based advance illness management (AIM) program for patients with multiple chronic conditions. The program led to improved outcomes including lower ED and hospitalization rates (decrease by 50%), higher hospice use (increase by 48%) with goal-concordant care, and lower total cost of care.

Home-based Medical Care: Evolving from Fee for Service to Global Risk

Home-based medical care has been around in the US for decades. In fact, the father of medicine, Hippocrates, practiced home-based medical care. Such models in the US have struggled because of the inability to have a sustainable business model in a fee-for-service payment system, in particular, Medicare fee-for-service. In this session, we discuss the conditions needed to allow a home-based medical care model to thrive and grow.

Social Media: A Guide to Moving Home Care Medicine into the Digital Space

There is need for Home Care Medicine to increase its presence in the digital space to promote and advance the field. This session will be led by Dr. Shreya Trivedi, an academic hospitalist at Beth Israel Deaconess Medical Center and Editor-in-Chief of the CORE IM Podcast. In her role at Harvard Medical School, she co-directs iMED (Innovations in Media & Education Delivery), an effort to educate and innovated within the digital space. In addition to Dr. Trivedi, the panelists will highlight the role of social media in medical education, advocating for our vulnerable homebound patients, and increasing the impact of Home Care Medicine nationally for health care professionals, trainees, and the public.
