Program At-a-Glance
Dates: January 1, 2019 – December 31,2019 Submission Deadline: January 7, 2019
Reward: Get a 20% rebate via Reward Card after submission when you purchase up to eight (8) new tires for
personal use on the employees registered vehicle. Rebate amounts are based on 20% of the tire(s) invoice price only.
Costs associated with mounting, balancing, valve stems, applicable taxes, etc. are excluded.
Purchase Requirement: Only includes new tires in the currently active MAST Flag brand, Michelin®, BFGoodrich®,
and Uniroyal® portfolio.
Eligible participants: Offer valid for current employees, and their spouse/qualified domestic partner only of Active
Michelin Fleet located in the fifty (50) United States, District of Columbia (“U.S.”) and U.S. territories (Puerto Rico,
Guam and U.S. Virgin Islands).
Redemption: All claims must be entered online at www.ONEPerks.org

How to create a profile
Visit www.oneperks.org. Click on “Create Profile” at the top of the page.
Fill in the email address and password you would like to use when logging in.
Fill in your name, address, phone number and preferred method of contact.
In the Ship To/AAN section, fill in your appropriate HO/BT number (1304192) Click “submit” once you have filled in the required information.
You now have created a login and can access your personal dashboard!

How to submit a perk
After logging in, go to the “Dashboard” and click “Submit Perk”.
Fill in the Tire Claim information.
Include a copy of the invoice/receipt. This documentation must clearly show the name and address of the dealer/outlet where the tire purchase was made, who purchased the tire(s), the tire brand, line, size, and quantity purchased.
Include a copy of the vehicle registration (not title) for the vehicle using the tires. Vehicle registrations must be in effect at the time of purchase.
Include proof of employment – a copy of a pay stub from your employer dated within 30 days of the purchase date. Any confidential information such as Social Security Numbers should be concealed before submission.
Accept the Terms & Conditions and click “Submit Perk”.
EMPLOYMENT VERIFICATION
Black out any confidential or personal information such as Social Security Numbers Ensure your name, the
pay stub date and your company name/logo are legible
After submitting your tire purchase, you will receive a confirmation e-mail or text message. Eligible Requests with correct paperwork will typically be processed in approximately 4-6 weeks from the time you submit. You can click on the “Perks” tab at the top of the page to view the status of your submission.
