2018 BENEFITS OPEN ENROLLMENT GUIDE
What IS new for 2018?
Open Enrollment Dates: November 6, 2017 through November 21, 2017
The City conducted a competitive procurement process for pharmacy, dental and vision benefits. As a result,
new vendors were selected and the new rates and vendors will be effective February 1, 2018. Employee Self
Service (ESS) will reflect the 2018 rates for your February through December benefits coverage. Please
note:
Express Scripts is our new Pharmacy Benefit Manager effective February 1, 2018. You will receive a separate
Pharmacy card before February. This replaces the Pharmacy benefit formerly provided as part of the Optima Health
Insurance. Express Scripts will share data with Optima so your out-of-pocket expenses are tracked.
Anthem is our new Dental Plan provider effective February 1, 2018, replacing Delta Dental. Rates for 2018 will
remain unchanged and new cards will be mailed to your home before February 1.
Davis Vision is our new Vision Plan provider effective February 1, 2018, replacing UniView/UniCare. Vision rates
have decreased slightly for the new plan year. New cards will be mailed to your home before February 1. The basic
vision coverage through the Optima Health plans remains unchanged.
Deductibles under the High Deductible Health Plan
(HDHP) have increased to meet IRS regulations.
The new deductibles are $2,700 for the employee only
coverage tier and $5,400 for all other coverage tiers.
City contributions for the Health Savings Accounts for
eligible employees enrolled in the HDHP, are $500 for
the employee only HDHP coverage tier and $1,000 for
all other coverage tiers. One-half of the City
contribution will be made and be available in January
and the other half will be made in July.
Election changes to Medical, Dental, Vision or Legal
coverage made during the Open Enrollment period will
be effective January 1, 2018.
Deductions for your coverage in January of 2018 will
be taken in December using the current 2017 rates.
Employee premiums for the HDHP, HMO, POS and
PPO Plus plans have increased approximately 7%.
Employees covered by the City’s health plan are
eligible to earn a Wellness incentive. The Wellness
incentive for 2018 provides $300 for completion of a
routine physical exam, a routine eye exam and a
routine dental exam. The $300 wellness incentive will
be added to your WageWorks card if enrolled in the
FSA. If you are not enrolled in the FSA but
participating in the HMO, POS or PPO Medical Plan,
you will receive a WageWorks card. If you are enrolled
in the HSA, the $300 will be deposited to your HSA.
The Diabetes Disease Management program has been
expanded to include covered children. Each program
participant can earn an incentive of $200!

What IS NOT new for 2018?
Optima Health HMO, POS and PPO plan designs are not changing. The co-payments and deductibles are not
changing for 2018 and the PPO plan will remain closed to new entrants.
Employees who want a Flexible Spending Account (Medical and/or Dependent Care) or Health Savings
Account must actively make elections in ESS every year. Due to IRS regulations, these benefits do not
roll over from one year to the next. Deductions will begin in January.

Open Enrollment Checklist: View your dependent information in ESS to ensure it is correct. Verify all
Social Security numbers for accuracy.
Make sure you have selected the best coverage options for you and your
family. Once Open Enrollment ends, changes can only be made within 31
days of an IRS “qualifying family status change” (i.e. employee marriage,
divorce, birth or adoption of a child, gain or loss of other coverage, etc.)
and require documentation.
Make any election changes to add/drop dependents, change medical
plans, or add/drop coverage under any of the benefit plans through ESS.
Make sure you select the submit choices button and get a green .
Ensure only eligible dependents are covered on your insurance plans.
Eligible dependents include your current spouse and children under the
age of 26 unless they are disabled. Former spouses are not eligible.
Grandchildren are not eligible dependents unless the employee/spouse
has legal custody, and provides court documentation.
If you have a medical or dependent care flexible spending account, make
your elections for 2018. If you do not re-enroll, the plans will not be
available to you in 2018 and remaining medical FSA funds will be lost.
If you are eligible to elect the Long Term Disability buy-up benefits from
The Standard, new enrollees must submit an “Evidence of Insurability”
form (link available at www.optimahealth.com/ches). Only VRS Plan 1 or
VRS Plan 2 full time employees are eligible for this benefit. VRS Hybrid
employees are automatically covered for employer paid short and long
term disability through the Reed Group.
Important Reminder
Employees must re-enroll every year in the medical and dependent care flexible spending accounts (FSA).
If you do not re-enroll in the medical FSA and have 2017 funds left over at the end of the calendar year,
you will lose those funds. If you enroll in the 2018 medical FSA, you can roll over up to $500 of unused
2017 FSA monies. You can submit receipts for 2017 expenses through March, 31, 2018. Any remaining
funds will be added to your 2018 fund balance after March 31, 2018.
How do I update my enrollment choices?
All enrollments are made online through ESS found at https://ess.cityofchesapeake.net. You can log-in
from home or work. If you have difficulty logging in to ESS from work, notify your payroll clerk. If you have
a City computer logon, you will need to contact the IT Help Desk to have your password reset. If you do
not have a City logon (e. g. CIBH, Library, Health Department, Circuit Court Clerk), use your employee
number as your user name and select the “forgot your password” link on the ESS logon screen to reset
your password. You will receive an e-mail to select the link and reset your password.
Dependent Audit will be Conducted During 2018
During 2018 the City’s benefits will be audited to ensure that covered dependents meet eligibility crite-
ria. Employees will be required to provide proof of dependent eligibility (e.g. birth certificate, marriage
certificate, court ordered custody documents) for anyone covered as a dependent.

Get your 1095 electronically so you can file your taxes sooner! Under the Affordable Care Act, the IRS requires the City to provide all employees and retirees who are eligible for medical insurance a tax form called a 1095 form. If you wish to receive your 1095 electronically (work e-mail) so you can file your taxes sooner go to:

* If you participate in Wellness or diabetes programs, your contributions need to be decreased accordingly.
2018 City contributions to the HSA are $500 for employee only and $1,000 for all other coverage tiers. One half of the 2018 City contributions will be made in January and July (prorated for new hires and life event changes).
