INSTRUCTIONS

• Please attach a copy of your previous year tax return if it was not prepared by our company.

• Complete the sections pertaining to your tax reporting requirements. Please include copies of any issued tax documents (ex. W-2’s, 1099’s, etc.)

• Please check the organizer to make sure you are furnishing all the information needed to complete your return correctly and accurately.

• Use the last page of the organizer to write down questions you may have and we will address them during the preparation of the tax return.

• Please bring the completed organizer with you along with all accompanying documents and questions for your scheduled tax preparation appointment.

• If you have trouble adding your information to the PDF, please make sure you are using the latest version of Adobe Reader (https://get.adobe.com/reader/).
