    Robert J. Simione is managing principal at Simione Healthcare Consultants, LLC and a frequent contributor to home care, hospice and financial management publications. He is the chair of the Advisory Board for the Home Health and Hospice Financial Managers Association and a member of the National Association of Home Care and Hospice (NAHC) Board of Directors. A graduate of Villanova University, Simione has served on a number of federal and state committees and conducted numerous workshops on various topics for NAHC, the National Hospice and Palliative Care Organization and state associations throughout the country.

    Amy Boutwell, MD, MPPMarket forces are driving collaboration in 2015 with a central role being played by case management professionals working across settings and sectors. This presentation will focus on achieving the core of todays healthcare reform promoting better health, providing better care and reducing healthcare costs. Dr. Boutwell draws upon her

    extensive experience with best practices and new ideas to define effective, practical and efficient ways to enhance transitions in care. She applies a comprehensive approach to reducing hospitalizations and improving care across settings and over time without exclusive adherence to any specific model.

    Amy Boutwell, MD, MPP is a graduate of Stanford University, Brown University School of Medicine and the Harvard Kennedy School of Government, where she received a masters degree in public policy. She is a practicing physician at Newton-Wellesley Hospital, attends on the medicine teaching service at Massachusetts General Hospital and is an instructor in medicine at Harvard Medical School. She founded Collaborative Healthcare Strategies which, most recently, has focused on the incentives and opportunities created by the Affordable Care Act and the Centers for Medicare and Medicaid Services (CMS) Innovation Center. Dr. Boutwell is the co-founder of the STAAR (State Action on Avoidable Rehospitalizations) Initiative of the Institute for Healthcare Improvement (IHI) as well as serving as a senior physician consultant to the National Coordinating Center for the CMS QIO Care Transitions Theme.

    Helen Riess, MDStudies have demonstrated that the degree of clinician empathy plays a significant role in improving outcomes in medicine, predicting quality of care, patient safety and satisfaction, and in decreasing malpractice claims. Negative consequences of uncompassionate communication threaten the effectiveness of

    the overall healthcare experience for patients and their families. Dr. Riess has developed an empathy training approach designed to promote respectful, compassionate and effective communication at all levels of healthcare. The result is greater trust, safety and satisfaction for patients and medical professionals.

    Helen Riess, MD, is co-founder, chief scientist and chairman of Empathetics, LLC, Associate Clinical Professor of Psychiatry at Harvard Medical School and Director of the Empathy and Relational Science Program at Massachusetts General Hospital. She is also a member of the Harvard Academy for Teaching and Learning, the Consortium for Research on Emotional Intelligence in Organizations, and faculty of the Harvard Macy Institute. Dr. Riess has devoted her career to teaching and research in the art and science of the patient-doctor relationship.

    Matt Zavadsky, MS-HSA, EMTIn several communities across the country, emergency medical service (EMS) providers are transforming their care delivery model from the traditional you call, we haul approach to a Mobile Integrated Healthcare (MIH) model. MIH service can effectively cover a geographic service area while adjusting the location

    and number of ambulances in service to compensate for the ebb and flow of call demand. The system has proven to be the most cost-effective and cost-efficient system found in EMS today. Zavadskys presentation will highlight the synergistic ways EMS, home health, and hospice agencies can work together for the benefit of patients.

    Matt Zavadsky, MS-HSA, EMT is from MedStar Mobile Healthcare in Forth Worth, Texas. He has guided the implementation of a robust mobile healthcare program which has partnered with organizations such as Klarus Home Care and Vitas Healthcare on ways to improve patient care and the use of healthcare resources. Lead author of Mobile Integrated Healthcare: Approach to Implementation, Zavadsky has developed and launched several functioning, successful and sustainable MIH programs that collaborate with community partners and other healthcare specialists. He also consults on numerous EMS topics including high performance EMS system operations, public/media relations, costing strategies and public policy.
