BOARD OF EDUCATION The Board of Education establishes policies and adopts regulations for

the conduct of the public schools. Its seven members, who serve without pay, are elected by the qualified voters of the District for a term of three years. Responsibility for administering Board policies is vested in the Superintendent of Schools.

BOARD OF EDUCATION MEETINGS There are usually two regular meetings of the Board of Education each

month. The public is invited to all the meetings, which are held in the James Baxter Board Room, Tuesdays at 8:00 pm, except where noted. The 2013-2014 regularly scheduled meetings are:

September 17 March 11,18 and 25 October 8 and 22 April 8 and 29 November 12 May 13 and 20* December 10 June 3 and 17 January 14 and 28 July 8 (Organizational Meeting) February 11 and 25 *Meeting will commence at 10:00 pm

A Note from the

Superintendent of Schools Dear Parents and Community Residents, Welcome to the 2013/2014 school year! We look forward to an exciting year for all of our students, as we implement new technology and programs designed to enhance educational opportunities for them. During the past year, our students achieved

at many levels, earning awards in academics, arts, athletics and extra-curricular venues. Our graduating students were accepted to many prestigious colleges and universities with 97% enrolling in institutions of higher learning. Our district continues to be committed to providing varied and exceptional learning opportunities for all of our students. In doing so, we encourage parents and community residents to join us for special events and programs listed on the school calendar, which will allow you to experience first-hand our wonderful students and staff. This calendar, which includes a Parent Guide, is one avenue of communication with our community. In addition, we ask that you frequently visit our revised district website and Facebook page for the most up-to-date information regarding student accomplishments and district events. You might also consider registering for our E-mail Blasts via the home page of the district website. Our Parent Portal is another primary means of communication with parents of school-aged children. On the Portal, you can learn about your child’s school, programs and see what your child is learning. You can also monitor your child’s grades and assignments and assist him/her in maintaining good grades through on-time completion of assigned tasks. If you have not already done so, please be sure to sign on to the Parent Portal this school year. We also encourage you to become involved in our school community through participation in the PTA, SEPTA, Athletic Booster Club and/or supporting your child’s involvement in extra-curricular activities. Finally, please be sure to attend Parent Back-to-School Night and parent/teacher conferences, as we can assure excellence for all children in our community by working together for their common good. BEFORE-SCHOOL & AFTER- SCHOOL PROGRAMS

The Hauppauge School District has made arrangements with SCOPE Educational Services to operate before-school and after-school programs at each of the District’s three elementary schools contingent upon adequate enrollment. The before-school program begins at 7:00 am and ends in time for the start of the regular school day. The after-school hours are from dismissal time until 6:00 pm.

The days of operation will follow the Hauppauge School District calendar. For more information about fees and other aspects of the program, please call SCOPE at 360-0800.

NEW STUDENT REGISTRATION New student registrations take place in the District Office,

495 Hoffman Lane, Hauppauge, NY, BY APPOINTMENT ONLY. Registration packets need to be completed before the registration appointment since forms MUST BE NOTARIZED. Packets can be requested for mailing by calling 761-8808, or picked up in the reception area at District Office from 8:00 am to 3:30 pm.

All required documentation is thoroughly explained in the registration packet and will include the following: Each child’s original birth certificate (raised seal),

baptismal record, or passport. Each child’s complete immunization record with a

doctor’s signature or stamp. All records are reviewed by the school nurse to ensure compliance with NYS Public Health Law Regulations. Any students whose immunizations are not in compliance may not be allowed to attend school until compliant.

Photo ID of parent or guardian registering the student. Proof of residency documents are required based on

type of residence in the district.

Enrollment is conditional upon District verification of legal residency in the district.

Change in Address All residents with students enrolled in Hauppauge

Schools who change their address must notify district registration at 761-8808 immediately. There will be a request to verify residency at the new address.

“FEBRUARY IS KINDERGARTEN REGISTRATION MONTH”

The Hauppauge School District offers a full-day kindergarten program. Those students who will reach their 5th birthday on or before December 1st of the current school year are eligible to register. Registration packets can be picked up January 13 through January 24 at each elementary school. Bring your calendar to arrange for a registration appointment. Registrations are scheduled by appointment only and arranged as follows: February 3 – 7 Forest Brook Elementary School February 10 – 14 Pines Elementary School February 24 – 28 Bretton Woods Elementary School

BOARD OF EDUCATION PARENTAL INVOLVEMENT POLICY #1900

The Board of Education believes that parent involvement is essential to student achievement, and thus encourages such involvement in school educational planning and operations. Parental involvement may take place either in the classroom or during extracurricular activities. However, the Board also encourages parental involvement at home (i.e., planned home reading time, informal learning activities, and/or homework “contracts” between parents and children).The Board directs the Superintendent of Schools to develop a home-school communications program in an effort to encourage all forms of parental involvement. To assure compliance with federal Title I guidelines, the Board further directs the administration to implement programs, activities, and procedures to achieve parental involvement in planning, designing, and implementing such programs pursuant to requirements contained in Title I and NCLB (No Child Left Behind) law and regulations. All such programs, policies, and guidelines are intended to strengthen the relationship between home and school.

THE EDUCATION PROGRAM The administration of the District’s educational

program is the responsibility of the Superintendent. She is supported by a staff that includes an Assistant Superintendent for Curriculum, Instruction and Technology; Assistant Superintendent for Personnel and Administration; Assistant Superintendent for Business and Operations, and other administrators.

The district provides courses of study in art, business, dance, health, home & careers, English Language Arts, mathematics, music, physical education, reading and study skills, science, second languages, social sciences, special education, technology education, and theatre. The curricula in each department is routinely reviewed and revised to reflect changes in New York State standards and instructional practices. Pupil personnel, psychological, and testing services are also available to students in addition to a host of co-curricular and athletic activities.

STUDENT SERVICES

TRANSPORTATION Transportation is provided to the children in the school

district in the following manner: • Grades K-5: More than one-quarter mile from school. • Grades 6-12: More than one-half mile from school. The State of New York requires transportation be

provided as follows: • K-8: More than 2 miles from school. • Grades 9-12: More than 3 miles from school.

Transportation is also provided for students attending parochial and private schools within a fifteen-mile distance from the determined point of origin. ALL REQUESTS FOR SUCH TRANSPORTATION MUST BE FILED BY APRIL 1 FOR THE FOLLOWING YEAR. Any requests submitted after April 1 will not be accepted. Bus schedules are provided to all parents prior to the beginning of the school year. Further information can be received by telephoning the Transportation Office at 870-5787.

NO CHILD LEFT BEHIND ACT COMPLIANCE

In accordance with the No Child Left Behind Act, New York State Commissioner’s regulations require that districts provide Academic Intervention Services (AIS) to eligible students to support them in meeting NY State Standards. Accordingly, students in grades K-12 recommended for AIS support based upon the district adopted criteria will be scheduled to receive academic intervention. These services are provided through flexible grouping, pull-out, or push-in models to prepare students for the English Language Arts (ELA), math, social studies and/or science assessments. Parents are informed in writing of their child’s need for AIS and of the requirement that students be enrolled in those classes. Continued communication with parents regarding students’ progress occurs at least four times per year.

High school students receive their academic support

through the AIS class which provides additional classroom instruction in core content areas. Students must attend AIS classes, if assigned, in courses resulting in a Regents exam.

As required by NCLB legislation, the district has outlined a formal process for resolving complaints alleging violations of our District’s administration and implementation of the Title I program or Academic Intervention Services. SCHOOL LUNCH Each of the District’s schools maintains modern kitchen

facilities in order to provide lunches as prescribed by the NY State Education Department. Although the purchase of lunch is voluntary, it has been generally recommended that students take advantage of the opportunity because of the variety and balance maintained. The purchase of milk is made available to all students.

The School District utilizes a computerized point-of-sale system, Nutrikids®, in all of its cafeterias to provide greater service and a more accurate accounting of student meals for parents. All students may purchase their meals with their ID card or number instead of cash. This program features an easy and secure online prepayment system and allows parents to monitor their child’s food choices. HOMEBOUND INSTRUCTION Students who are unable to attend school for an

extended period of time due to illness or accident are eligible for limited homebound instruction by a tutor provided by the School District when it is possible to supply such service. Parent/guardians must submit a medical physician’s note stating that the student is unable to attend school for at least 10 school days from the date the note is received by the school nurse.

INSTRUCTIONAL SUPPORT TEAMS

The Hauppauge School District operates Instructional Support Teams (IST), comprised of members of the teaching, administrative, and support staff in each school building. The purpose of the IST is to support instructional staff and students by planning effective mainstream-based instructional strategies for students who are not attaining State Standards and/or are having difficulties in mastering core content materials. The IST model uses a collaborative approach to problem solving.

TUTORIAL ASSISTANCE Students who have particular learning difficulties may

request assistance from their teachers for after-hours tutoring. Parents or students should contact the teacher in advance to make satisfactory appointment arrangements.

SPORTS AND ATHLETICS

The School District provides an assortment of athletic programs for all middle and high school students. A program of intramural sports is open to children in the upper elementary grades. In addition to the sports physicals required to participate in intramural sports being administered by the school doctor, they may also be administered by family physicians utilizing the form obtained from the school nurse.

GUIDANCE SERVICES The Guidance Department is committed to ensure that all

Hauppauge students are provided with the opportunity to discover their own unique strengths, and to create a personal vision for the future. The program is sequential in scope and includes activities to help students acquire competencies in career planning, knowledge of self and others, and educational development. Our goal as a school community is to educate all students to manage change and become lifelong learners.

All schools have guidance counselors available to provide support and resources for all students. Student success is linked directly to continued communication between staff, students and parents. Please do not hesitate to contact the Guidance Department at the school your child attends.

CAREER CENTER The Career Center is located in Hauppauge High School

opposite the Counseling Center in Room 255. Students and parents are invited to use the college and career resource materials available to plan for the future.

For students enrolled in the General Education Work Experience Program (GEWEP), the Career Center offers a job placement service to match a student’s career goals with partnering employers. For students not enrolled in GEWEP who have indicated a desire for after-school employment, the Career Center maintains a list of participating local employers. The School to Work coordinator in the High School coordinates both programs; detailed information may be secured from Mr. Hufnagel in Room 255 throughout the school year.

HOMEWORK Homework provides an opportunity for students to

increase their fluency and confidence through practice of skills taught in school. Incomplete assignments, and supplementary or research work which cannot be completed in school should be finished at home. The experience of doing homework independently is valuable to children. The amount of homework necessary will be dependent on the subject, type of class, and the individual child. Parents can

assist by providing a distraction-free place to work, establishing a time schedule for homework, sharing an interest in the assignment, and an expectation that homework be completed.

PARENT-TEACHER CONFERENCES

Parents are invited and urged to contact individual schools to discuss the progress of their children throughout the year.

Regular parent-teacher conferences may be scheduledthrough the building teacher in the elementary schools. Such conferences may take place after school hours, at a time which is mutually acceptable to the teacher and parent. Similar conferences can be scheduled at the secondary level by contacting the Counseling Center of the Middle School or the High School.

INFINITE CAMPUS PARENT PORTAL

The Infinite Campus Parent Portal gives access to the parent(s)/guardian(s) of all students at all levels to attendance, health, special reports, and notices from the building PTAs etc.. At the middle and high school levels, access includes teacher gradebook, report cards, and transcripts. The parent portal works to keep parent(s)/guardian(s) informed about their child’s progress in school.

PUPIL PERSONNEL SERVICES CHILD FIND In order to plan appropriately, and consistent with Part 200 of the Commissioner’s Regulations, please notify Claire M. Restaino-Hochheiser, Director of Pupil Personnel Services, 761-8342 if you have a child 0-21 years of age with a disability who may require special education services. All information is kept confidential. PUPIL PERSONNEL SERVICES Growing and learning are complicated activities. Parents and teachers often need advice and support regarding how to deal with problems. The schools maintain a staff of professional personnel for these purposes, each of whom may be contacted through the school principals or the PPS Department. Pupil Personnel Services include the areas of psychological support, speech therapy, special education, English language learners, nurses, and social work services. SPECIAL EDUCATION Children who require alternative instructional support based upon an identified disability may receive special education services in an in- or out-of district placement according to recommendations made by the Committee on Special Education (CSE) in conjunction with parent/ guardian agreement and approval from the Board of Education.

COMMITTEE ON PRESCHOOL SPECIAL EDUCATION (CPSE)

Any child between the ages of 3-5 years old residing in the District and suspected of having a developmental delay can be referred to the CPSE. The CPSE will arrange a comprehensive evaluation and appropriate services for children upon approval. The program is funded jointly between the Suffolk County Health Department and the New York State Education Department. Referrals can be made in writing to the Office of Pupil Personnel Services.

COMMITTEE ON SPECIAL EDUCATION (CSE)

When a student is suspected of having a disability, a referral is made to the Committee on Special Education (CSE). A referral for special education programs and services may be made by a parent or person in a parental relationship to a child, by the student who is 18 years or older, an emancipated minor, or a professional within the student’s school district, and the Commissioner and/or a designee of a public agency responsible for providing services to the student. A request for a referral may be made by a physician or designee of a public agency with responsibility for the welfare, health, or education of the child. Parents are members of the Committee that makesrecommendations regarding their child. They must be provided notice of proposed actions, a description of other options considered, an explanation of why an action was selected or a reason for rejection, and a description of the evaluations or procedures upon which the action is based.

PSYCHOLOGICAL SERVICES The school psychologist serves the school in an advisory capacity and observes students in class, consults with teachers, and sees students individually in a confidential setting. He/she utilizes a variety of measures to evaluate and help students understand their intellectual potential, academic achievement, classroom functioning and personality development.

SOCIAL WORK SERVICES Social Workers provide services at all grade levels for children who may have difficulty in educational, social or emotional areas. These specialists act as consultants to parents, teachers, counselors, and administrators. They function as

an integral part of a referral diagnostic team and make recommendations regarding the student based upon identified needs.

SPECIAL EDUCATION INSTRUCTION/PROGRAMS

A staff of qualified Special Education teachers provides special education instruction, as determined by a child’s IEP. These specialists serve at each school; therefore, contact should be made through the principal. For further information, call Claire M. Restaino-Hochheiser at 761-8342.

SECTION 504 ACCOMMODATION PLAN

Accommodations are developed and provided to students who have mental or physical impairments that significantly impact their health or learning. Contact your building principal for further information.

SPEECH AND LANGUAGE SPECIALISTS/TEACHERS OF THE DEAF

Specialists are employed to help children whose speech and/or language processing interferes with communication. Children with articulation delays, stuttering, receptive or expressive language difficulties, or loss of hearing receive special assistance on a scheduled basis in each building.

ENGLISH AS A SECOND LANGUAGE (ESL)

The purpose of the ESL program is to ensure that all English Language Learner (ELL) students acquire the English language. This instruction is provided by certified ESL teachers who use methods and techniques designed to teach a second language. Parents will be notified if their child is eligible to participate in a language instruction educational program.

SECTION 504/ADA REGULATION The Hauppauge School District does not discriminate on the basis of disabling conditions in the educational programs or activities which it operates. It is required by Section 504 of the Rehabilitation Act of 1973 not to discriminate in such a manner. This policy of non-discrimination includes the following areas: recruitment and appointment of employees, employment pay and benefits; counseling services for students, access by students to educational programs, course offerings, and student activities. 504 Accommodation Plans are developed between parents and the school district.

The District official responsible for the coordination of activities relating to compliance with Section 504/Title II is Claire M. Restaino-Hochheiser, Director of Pupil Personnel Services (761-8342). Her office is located at 495 Hoffman Lane, Hauppauge, NY 11788. Information, including compliance procedures, will be provided to any student or employee who feels that his or her rights under Section 504/Title II may have been violated by the District or its officials, by contacting the appropriate coordinator. GRIEVANCE PROCEDURES If you wish to challenge the actions of the School District and the decision of the 504 Committee with regard to your child, you should file a written Notice of Appeal with the District’s 504 Compliance Coordinator, Claire M. Restaino-Hochheiser, within 30 calendar days from the time you received written notice of the 504 Committee’s action(s). The full grievance procedure is provided as an avenue for the expeditious processing of complaints. All grievances will be reviewed in accordance with the following procedures.

If an individual believes there has been a violation of his or her rights under Section 504 or ADA, the complainant shall submit in

writing fully setting out the circumstances giving rise to his or her grievance. Such claims must be filed with the District’s Section 504/ADA School Compliance Officer: Claire M. Restaino-Hochheiser at 495 Hoffman Lane, Hauppauge, New York 11788.

As soon as possible, but within (5) school

days, the Section 504/ADA Compliance Officer will meet with the complainant to review all aspects of the complaint. If possible, the Section 504/ADA Compliance Officer will attempt to resolve the complaint at this initial meeting.

If an investigation becomes necessary to gather additional information regarding the circumstances of the complaint, the Section 504 Compliance Officer will conduct such an investigation within ten (10) school days. A meeting will be held shortly thereafter with the complainant to inform him/her of the outcome of the investigation and the steps to be taken to resolve the matter.

If the complainant does not feel his/her

complaint has been satisfactorily resolved, a written request for review by an impartial hearing officer should be submitted to the Section 504/ADA Compliance Officer.

The hearing officer shall give the complainant a full and fair opportunity to present evidence relevant to the issues raised under the grievance. The complainant may, at his/her own expense, be assisted or represented by individuals of his/her own choosing, including an attorney.

If you disagree with the decision of the

impartial hearing officer, you have the right to a review of that decision by a court of competent jurisdiction.

On 504 matters other than your child’s

identification, evaluation, and placement, you have a right to file a complaint with the 504 Compliance Officer, Claire M. Restaino-Hochheiser, who will investigate the allegations to the extent warranted by the nature of the complaint in an effort to reach a prompt and equitable resolution.

The best community relations program a school district can offer is one where the children enrolled receive an optimum educational program. Hauppauge Public Schools are dedicated to this goal. Specific questions or issues that arise throughout the year are addressed at the first possible opportunity. The Board of Education encourages residents to seek information. Read “Finding Answers” on page 36.

The School District provides avenues of communicationthrough the District newsletter Hauppauge Horizons, informational releases to the media (with a special emphasis on the local newspapers), the calendar, and the annual Budget Brochure. To have a continually improving school district, it is important for the community to know about its schools, the educational program, and the people who are responsible for the process.

CARE OF BUILDINGS The maintenance of the interior and exterior of our District

buildings and grounds is a cooperative effort involving the District employees, parents, and residents of the District. Every effort is exerted to maintain our buildings and to prevent vandalism, both during school hours, and when school is not in session. However, no amount of security precautions can be effective without the continued vigilance of parents and residents, particularly on weekends or at other times when the schools are not open.

A concerted effort is made to prevent unnecessary and costly vandalism from damaging our facilities. Please call the emergency number, 265-8511, to report vandalism or other incidents in or on any school property. Your message will be taken and dispatched to Security.

SWIM PROGRAM An extensive swim program for adults and children is

offered year round in the District’s 25-yard pool. Children’s courses are held on Saturdays during the school year and include all levels of American Red Cross (ARC) swimming. An exciting multi-level children’s summer program runs from the end of June to August. All levels of adult swim lessons are offered on Monday evenings during the summer. Life-

guard training is offered during the winter and spring programs. Family swim is offered on most Tuesday, Thursday, and Friday evenings from 7:00-8:15 and 8:30-9:45 during the academic year and on Tuesday and Wednesday evenings from 7:00-8:15 and 8:30-9:45 during the summer. Fees vary. Registration dates are entered on the calendar in September, November, March, and June. A flyer is delivered to all residents. USE OF SCHOOL FACILITIES The Board of Education wishes residents of the

District to have the benefit of the use of school facilities during after-school hours while feasible and without interfering with the regular school program. Thus, the District’s facilities are used for many activities which are educational, cultural, recreational, and civic in nature, all of which primarily benefit District residents.

Organized groups may use school facilities only after completion of application and approval of usage.

All outside playgrounds are closed after dusk. All school grounds and facilities are closed to the public

when school is in session.

HAUPPAUGE EDUCATIONAL FOUNDATION New York State’s rising educational standards have created a need for additional resources for students in Hauppauge. The Foundation is a nonprofit, tax exempt entity with a Board of Trustees committed to attracting, managing, and distributing monies and resources to implement educational programs. The funds raised by the Foundation will be used solely for educational activities within the Hauppauge Public School District. Donations will help fund projects and programs that are not possible within the school’s annual budget. The Foundation’s funds are utilized for those special “extras” which are not allocated in the school budget.

Anyone can join the Foundation. We look to include as many people as possible. Those interested are asked to volunteer their time, talent, and resources, and make a commitment to excellence in our schools.

WORKING PAPERS The District cooperates with the New York State

Labor Department in the issuance of working papers, required by law, for students under the age of 18 who seek employment. All children residing in the District who need such papers may obtain them by contacting the High School Counseling Center.

To secure working papers a student must: • obtain and fill out an application • undergo a medical examination • submit proof of age. This can be in the form of a birth or

baptismal certificate, hospital record, or passport.

HEALTH INFORMATION PESTICIDE NOTIFICATION

Initial Notification to Persons in Parental Relation and Staff Pursuant To Section 409-H of the NY State Education Law.

New York Education Law Section 409-H, effective July 1, 2001, requires all public and nonpublic elementary and secondary schools to provide written notification to all persons in parental relation, faculty, and staff regarding the potential use of pesticides periodically throughout the school year.

The Hauppauge School District is required to maintain a list of persons in parental relation, faculty, and staff who wish to receive 48-hour prior written notification of certain pesticide applications. The following pesticide applications are not subject to prior notification requirements: a school remains unoccupied for a continuous 72 hours following an application; antimicrobial products; nonvolatile rodenticides in tamper-resistant bait stations in areas inaccessible to children; silica gels and other nonvolatile ready-to-use pastes, foams, or gels in areas inaccessible to children; boric acid and disodium octaborate tetrahydrate; the application of EPA-designated biopesticides; the application of EPA-designated exempt materials under 40 CFR 152.25, the use of aerosol products with a directed spray in containers of 18 fluid ounces or less when used to protect individuals from an imminent threat from stinging and biting insects including venomous spiders, bees, wasps, and hornets.

The Hauppauge School District has utilized an Integrated Pest Management (IPM) program over the past ten years. The very purpose of an IPM program is to restrict or eliminate entirely, the use of pesticides in or around District facilities.

The District’s IPM program includes the services of a contracted firm that is an expert in integrated pest management. This firm conducts monthly inspections of all District facilities and takes appropriate remedial measures to eliminate or reduce the presence of insects or other pests.

While the District does not anticipate any situation where there will be a need to provide notification of a pesticide application 48 hours in advance, if you would like to review such notification, if one were required, please contact the Hauppauge UFSD Buildings and Grounds Office at the Whiporwil School, 495 Hoffman Lane, P.O. Box 6006, Hauppauge, New York 11788.

ASBESTOS NOTIFICATION In compliance with the U.S. Environmental Protection Agency (EPA) and the Asbestos Hazard Emergency Response Act (AHERA), the District has performed the six-month asbestos re-inspections of each of our school buildings for asbestos-containing building materials. A six-month Periodic Surveillance Inspection Report is on file in each administrative office.

HEALTH SERVICES EMERGENCY CONTACT PROCEDURE In the event of an accident, first aid is administered immediately. Every effort is made to contact the parent/guardian. If parents/guardians are unavailable, the student’s emergency contacts will be called. STUDENT ACCIDENT INSURANCE

The students in the Hauppauge School District are covered under a limited coverage school-time plan of insurance against accidental injury. Benefits will be.

MEDICATION Prescription and over-the-counter medication may not

be administered in school unless accompanied by a physician’s note regarding frequency and dosage and a parent’s written request. All medication must be brought to and from school by an adult. Medication should never be sent to school with a student.

IMMUNIZATIONS & COMMUNICABLE DISEASES New York State Public Heath Laws require that all children entering school must present a physician’s certificate indicating that the student has been immunized against Polio, Measles, Mumps, Rubella, Diphtheria, Pertussis, Tetanus, Hepatitis B Series, Varicella, and Tdap booster. (Varicella is needed only if born on or after

1/1/98 or entering grade 6 after 9/1/05. Tdap booster is needed if born on or after 1/1/94 and entering grade 6 on or after 9/1/07). This certificate must be submitted to your school prior to the opening day of school in September. By law, no child may be admitted to school unless the certificate is on file. PHYSICAL EXAMINATIONS New York State Education Law requires all students to have a physical examination upon entering the school district and in grades Kindergarten, 2, 4, 7, and 10. If the cost of the exam is prohibitive, contact the school nurse’s office to avail your family of any of the several scheduled examinations with the school physician during the school year. BMI INFORMATION As part of a required school health examination, a student is weighed and his/her height is measured. These numbers are used to figure out the student’s body mass index or “BMI”. The BMI helps the doctor or nurse know if the student’s weight is in a healthy range or is too high or too low. A sample of school districts will be selected to take part in a survey by the NYS Department of Health. If our school is selected to be part of the survey, we will be reporting to the NYS Department of Health information about our students’ weight status groups. ONLY SUMMARY INFORMATION IS SENT. No names and no information about individual students are sent. Please notify your child’s school nurse if you do not want your child’s BMI information included in the survey.

WELLNESS POLICY Throughout the school year, there may be

opportunities for your child to purchase or consume food that is prepared in private homes.

If your child has any food allergies that may restrict his or her ability to buy or consume these products, please be sure to notify the nurse in your child’s school. Additionally, if you have any concerns, please speak with your child’s teacher.

unsafe 3. Religious observance 4. Sickness requiring attendance at a medical clinic 5. Approved school-sponsored trips 6. Quarantine 7. Required court appearances

Absence or tardiness for any other reason is considered unexcused. Parents are requested to advise the school by telephone when a child is absent. A written excuse, signed by the parent, is necessary when the student returns to school from an absence or tardiness. Students are expected to be in school on time every day for the full day. Studies show that poor attendance is the reason most students fail or drop out of high school. Parents, please help us to develop this lifelong, valuable worth ethic. It is important!

Parents should emphasize the harmful effects of frequent absence or tardiness. Along with careless work habits, loss of instructional time occurs. Parents are urged to give this matter serious attention.

CODE OF CONDUCT NOTICE A summary of the Hauppauge School District’s Code

of Conduct will be distributed to parents annually as a part of the August school mailing. The complete Code of Conduct is available on the District website at http://www.hauppauge.k12.ny.us.

VISITORS You are always welcome in our schools. Please go directly to school security personnel and report your presence and reason for being in the building. This request is made to aid you, to prevent the interruption of classes, and to prevent unauthorized individuals from entering our schools.

DIGNITY FOR ALL STUDENTS ACT

The Dignity for All Students Act took effect on July 1, 2012. The ultimate goal of the Dignity Act is to provide public elementary and secondary school students with a safe and supportive environment free from discrimination, intimidation, taunting, harassment, and bullying on school property, on the school bus, and/or at school functions/zones. All building Principals have been designated as the Dignity Act Coordinators. Required updates to reflect the Dignity for All Students Act have been incorporated into Board of Education Policies and the Code of Conduct.

SMOKING AND TOBACCO USE In accordance with Board of Education policy and State

and Federal laws, smoking by students or adults in school buildings or on school grounds is prohibited. Students shall abide by the appropriate regulations regarding smoking at public events and out-of-school social events. There shall be no smoking on school buses. There shall be no smoking by students at any school function (on or off campus).

NOTICE OF NON-DISCRIMINATION

To ensure compliance with Federal and State regulations, the Hauppauge Union Free School District has identified the Assistant Superintendent for Personnel & Administration, Michele M. Rothfeld, and James J. Stucchio, Assistant Superintendent for Business & Operations, as the Title IX Compliance Officers. Title IX deals with areas of discrimination involving age, sex, religion, national origin, and disability. Principals are designated Compliance Officers at their buildings.

USE OF DRUGS, ALCOHOL & WEAPONS

1. Any student attending school in the Hauppauge Union Free School District who possesses, uses, dispenses, or sells drugs, alcohol, or weapons on District property, or while otherwise under the jurisdiction of said District shall, upon evidence that is satisfactory to the school authorities, be suspended immediately for five days by his or her building Principal. (Weapon possession carries an automatic one-year suspension from school).

2. During this suspension, a plan of action to deal with the student’s problem will be developed with the Building Principal. The parents or guardian will be invited to participate in formulation of the plan.

If the same student should be involved again in possessing, using, dispensing, or selling drugs and/or alcohol, upon evidence that is satisfactory to the school authorities, such behavior will be grounds for disciplinary charges against the student in accordance with the law, with a view toward suspending that student from the District for a period ranging from a minimum of six (6) weeks to the balance of the school year via a Superintendent’s Hearing. Evidence of out-of-school psychiatric or psychological evaluation or other appropriate counseling may be considered in determining an appropriate period of suspension.

4. When a student is suspended, the District will supply a bona fide program of home instruction for that individual for the time that he or she is suspended, as may be required by law.

5. Suspension shall be explained to the parent at the time of the initial offense. Suspension for the remainder of the school year means that the child shall no longer be allowed to attend school in the Hauppauge School District. It must be made clear that if a student appears on school property during the duration of the suspension, he or she may be arrested and criminally charged.

STUDENT ACCESS TO INTERNET AND NETWORK RESOURCES

All students who are enrolled in the Hauppauge Union Free School District will enjoy the privilege of having access to the Internet and network resources, provided that they comply with the District’s Acceptable Use Policy (4526-R).

Each student will be provided with a confidential username and password to the District network, including Internet access. In September, each student will be required to login to the network. During this initial login process, each student will be required to register his or her personal password. Successful completion of this process will include acceptance of the terms and conditions of the Hauppauge Union Free School District Acceptable Use Policy (4526-R).

As a parent or guardian, you have the right to deny your son or daughter access to Hauppauge Union Free School District Internet and network resources. Parents or guardians may submit such written request to:

TITLE 45 DIRECTORY INFORMATION PUBLIC NOTICE

Unless objection to any of the specific items of information following is submitted in writing by parents or legal guardians, or by those students themselves who are 18 years of age and older, the Hauppauge School District herewith gives notice of intention to provide, release, or publish in the Hauppauge Horizons Newsletter, school or student newspapers, magazines, yearbooks or other publications, daily or weekly newspapers, athletic programs, musical or theatrical programs, and news releases, any and/or all of the following information pertaining to students as may be appropriate under the circumstances: name of student, names of parents, address, age, height, weight, grade, major field of study, participation in recognized school activities, extracurricular activities and sports programs, academic honors, achievements, awards, scholarships, and similar information. Under Title 45, U.S. Code, Part 99, Privacy Rights of Parents and Students, parents who do not desire the release of any of the above directory information must make a specific request in writing to the Superintendent of Schools by September 30, 2013. Failure to make such a request shall be deemed consent to release, provide, or publish the directory information during the school year.

STUDENT BICYCLES AND AUTOMOBILES

Bicycles may be ridden to and from school, but not on school grounds. They are to be parked in areas designated by the Principal.

Automobile parking permits are issued to high school seniors who possess a Class D license. In order for high school juniors to obtain a parking permit enrollment in a work-based learning program and possession of a Class D license are required. In no case, either for seniors or juniors, will a parking permit be issued to a student with a junior license.

Parking permit applications may be picked up from the security office located within the high school. Please note that permits are a privilege and may be revoked if the student does not comply with the following regulations of the Board of Education:

• Cars must be driven carefully and in accordance with all signs and traffic directions on the school grounds.

• No stopping or parking is permitted except in the area designated for student parking.

• Students may not use cars between arrival and departure time.

• Students may not loiter in parked cars. • Juniors must maintain good standing within the

work based learning program.

FINDING ANSWERS Under no circumstances will the School District or

Board of Education consider or take action on any letters or communications that are unsigned. Anonymity of individuals registering complaints or allegations in writing will be respected if requested, but such communications must bear the name, address, and signature of the writer.

When parents have a question or complaint about a child’s classwork or progress in a specific course, they should contact the teacher. The teacher, who works with the student, knows the child and can generally help to resolve the difficulty.

At the elementary level, a parent should contact the

Building Principal if the teacher does not offer a satisfactory resolution. At the secondary level, the department director should be contacted or the parent should schedule an appointment with the student’s guidance counselor. If satisfaction is not achieved, then the parent should call the Building Principal.

If a resident’s concern is of a more general nature and involves the educational program or activities of a school, the Building Principal should be contacted directly.

If the question still remains unresolved, the parent may choose to contact the Office of the Superintendent of Schools.

In the case of a complaint against a professional staff member, parents are urged to:

VOTER INFORMATION

VOTER REGISTRATION The Hauppauge Board of Education has adopted

permanent personal registration procedures in accordance with State Law. This means anyone who registered and voted in a school election or general (local, county, state, or federal) election during the four (4) years prior to a school vote or election, and has not subsequently been removed from Suffolk County Board of Elections registration by registering in another district, can vote without registering.

Residents who have not previously registered or have moved into the District may register to vote in School District elections daily at the District Clerk’s Office between 9 am and 4 pm any day school is in session, and before regularly scheduled Board of Education meetings during April and May (not less than seven days preceding any District election). EMERGENCY SCHOOL CLOSING/DELAYED OPENING

In the event that schools are closed due to inclement weather, hazardous road conditions, or in the event of delayed openings or early dismissal, announcements will be made on the District website: http://www.hauppauge.k12.ny.us and over the following radio/TV stations:

Whenever you are in doubt about the schools being open, you are urged to listen to these radio/TV stations; please do not telephone the school office on these occasions in order to avoid congestion of vital open lines during the crucial early hours.

When the schools are closed due to emergency conditions, the before-school and after-school programs are canceled.

If weather predictions indicate that early morning storm conditions will clear shortly, delayed opening procedures may be implemented. Depending on weather conditions, bus pickups for all schools will begin one or two hours later than the regular schedule. The delayed opening schedule will only be used when road and weather conditions are expected to improve after the very early morning hours.

At times, early dismissal of schools may be necessary due to threatening weather conditions or other emergencies such as loss of heat or electrical power. When there is a need to close schools early due to an emergency situation, parents/guardians will be notified by a computerized phone call system. It is imperative that parents/guardians develop plans for their children in the event of such an emergency closing. Specifically, children should know the names of designated adults who can meet them at the bus stop. At the elementary level,

it will be the parents’ responsibility to notify the school to keep the child at school in the event that an adult will not be available to pick the child up at the bus stop. In the event of inclement weather which does not require early dismissal, all parents should

anticipate that buses will be delayed. Parents will NOT be notified by the school of these delays. Please feel free to contact your child’s school or check the District’s website at:

requires you to enter the name and code number of the public school district where you were a resident on December 31 of the tax year. You must enter this information even if you were temporarily absent, or if

the school your children attended was not in the School District, or even if you had no children attending school.

Adjusted gross income is assigned to school districts and is used as a wealth measure in various formulas for the apportionment of State aid, based on tax codes entered on tax returns.

In other words, when the school district tax code is entered incorrectly or not at all, the Hauppauge Public School District receives less State aid than it is entitled to receive.

The Hauppauge Union Free School District, which consistently maintains its reputation for outstanding academic, athletic, and artistic achievements, is the twenty-seventh largest of Suffolk County’s sixty-nine school systems educating students in kindergarten through twelfth grade. There are approximately 4,100 students in the public schools and more than 150 students in private and parochial schools.
