NOTE: The 4th quarter must be certified and approved in order to move forward in reporting. The cells will be automatically populated. If 4th quarter is certified and approved, click “Save” then click on “Next” button.

NOTE: All cells that give a “count” number must be filled in. TEST COLLEGE CCD

NOTE: You can correct individual cell entries, but if you “clear data” all data is deleted and you must start again. Once you have entered all the data, click “Save” and move to “Next”.

NOTE: If the school has used grant funds for Equipment purchases, you will need to fill in the form and send it via email separate from this report.

At this point you can click “Next” and go back to the beginning of the reporting sections, or click “Certify Final Report”, which takes you to the page to certify and submit the final report.

14Once you have pressed the “submit” button, the system will take you to this page, which displays the entire report. The report must be printed out and saved in the grant’s file as a hard copy.

Next Steps

• District CBO needs to certify the Final Report by August 31

• Submit 3 copies of Final Claim forms with original signatures, in blue ink, to Project Monitor

• Chancellor’s Office approves the Final Report• Final payment to District through apportionmentNOTE: No Grant Extensions - will be granted on a case by case basis, and only under exceptional circumstance
